Methoxyflurane for Relief of Procedural Pain in Burn Patients: A Prospective Single-Centre Evaluation Study.

Background: Procedural pain in burn patients continues to be a major problem. Frequently used analgesics, such as opioids, may have various side effects, including respiratory depression, nausea, and vomiting. Inhaled methoxyflurane has been used in the pre-hospital setting for trauma-related pain. This pilot study aimed to investigate the feasibility of using methoxyflurane for pain relief during dressings changes for burns in the hospital setting. Methods: In this investigator-initiated pilot study, we included burn patients undergoing dressing changes in the burn ward. The primary outcome w

Doping Control Analysis of Methoxyflurane in Equine Plasma by Headspace Gas Chromatography-Tandem Mass Spectrometry.

To control the potential misuse of methoxyflurane in equine sports, a simple and sensitive method for detecting methoxyflurane in equine plasma by gas chromatography/tandem mass spectrometry using negative chemical ionisation with methane as reagent gas has been developed and validated. The method shows good sensitivity and robustness, with a limit of detection of 0.05 μg/mL could be achieved by the method, demonstrating the potential to employ this technique in the detection of this volatile and electronegative compound in biological matrices.

Inhaled methoxyflurane for fracture reduction in prehospital extremity trauma: an observational review of HEMS clinical practice.

BACKGROUND: Prehospital manipulation and reduction (M&R) of traumatic limb injuries may be required before transfer to restore alignment, reduce pain and bleeding, and limit secondary neurovascular compromise. In many systems this requires intravenous procedural sedation, which is resource-intensive and not universally available. Inhaled methoxyflurane (IMF) is a portable, self-administered analgesic used within UK HEMS, but its utility as the primary agent for prehospital M&R is not well described. METHODS: We performed a retrospective observational review of trauma patients attended by East

Service Evaluation of Methoxyflurane Versus Standard Care for Overall Management of Patients with Pain Due to Injury.

INTRODUCTION: Inhaled methoxyflurane is an analgesic used for the emergency relief of moderate to severe pain in conscious adult patients with trauma and associated pain that is increasingly being used in hospital emergency departments to provide rapid analgesia. It is widely accepted that effective pain relief can facilitate patient care and flow through the emergency department (ED). The main aim of this evaluation was to assess the impact of inhaled methoxyflurane on patient length of stay (LOS) in the ED compared with standard care. METHODS: Adult patients with moderate to severe trauma p

Inhaled methoxyflurane for pain and anxiety relief during burn wound care procedures: an Australian case series.

Pain is a common and significant feature of burn injury. The use of intravenous opioids forms the mainstay of procedural burn pain management, but in an outpatient setting, the demand for novel agents that do not require parenteral access, are easy to administer and have a rapid onset are urgently needed. One such agent is the inhaled anaesthetic agent, methoxyflurane (MF). The aim of this study was to conduct a pilot investigation into the clinical effectiveness of MF inhaler on pain and anxiety scores in patients undergoing burn wound care procedures in an outpatient setting. A prospective c

Methoxyflurane in early analgesic therapy by ski patrol members on Swiss ski slopes - an observational cohort study.

BACKGROUND: Pain therapy is an important first-response measure in the pre-clinical care of trauma patients. Injured individuals on ski slopes are usually given first aid by members of the ski patrol. The early implementation of adequate pain therapy by these paramedical rescuers can increase patient satisfaction and have a positive effect on the entire treatment process. In this context, we analysed the administration of methoxyflurane by ski patrol members on Swiss ski slopes. METHODS: In this retrospective observational study, we evaluated 172 datasets, of which 149 concerned patients who

Pain Self-Management with Inhaled Methoxyflurane by Emergency Department Trauma Patients: A Prospective, Interventional, Single-Center Study.

The coronavirus disease 2019 (COVID-19) pandemic has led to overcrowding in many emergency departments (EDs). The present single-center, prospective, interventional study (conducted at Bichat University Medical Center (Paris, France)) was designed to assess the impact of self-administered, inhaled, low-dose methoxyflurane on trauma pain in a pre-ED fast-track zone dedicated to the management of lower-acuity non-COVID-19 patients. In the first phase of the study, the control group consisted of patients with mild-to-moderate trauma pain, for whom the triage nurse initiated pain management (based

Methoxyflurane Instead of Morphine in Acute Coronary Syndrome Patients: Results of the Randomized Controlled METHANE-SIRIO 4 Study.

AIM: Morphine is known to negatively influence the pharmacokinetics (PK) and the antiplatelet effect of oral P2Y12 receptor inhibitors administered due to an acute coronary syndrome. Worldwide studies of the potential approaches to overcome the undesired morphine effect have been only partially successful. The aim of the METHANE-SIRIO 4 study was to evaluate the PK and pharmacodynamics of ticagrelor and its active metabolite in unstable angina (UA) patients treated with morphine or methoxyflurane. METHODS AND RESULTS: Consecutive patients diagnosed with UA, randomized into the study arms, und

MetoksyKval: the extent of pre-hospital methoxyflurane administration for acute traumatic pain: focus on economic impact and rationale for use.

BACKGROUND: Effective pain management is critical in pre-hospital care, yet many patients still receive suboptimal treatment. Methoxyflurane, a handheld self-administered analgesic, is increasingly used in European ambulance services. Its effectiveness and safety are established, but the reasons for its use in acute pre-hospital settings have received limited attention. This study aimed to explore patterns of use, effectiveness, and economic implications of methoxyflurane in traumatic pain, with particular focus on decision-making and patient satisfaction. Efficacy and Safety of Methoxyflurane for Treatment of Acute Traumatic Pain by EMTs during Alpine Rescue Operations: The "PainDrop" Trial.

Background: Treatment of acute traumatic pain is a core task for mountain rescue services. Intravenous access, however, is often difficult, and the vast majority of missions are carried out without a physician at the scene. The spectrum of analgesics available for use by non-physician personnel is limited. Inhaled analgesics, such as methoxyflurane, might prove useful, but currently no data exist on their application by non-physicians in the alpine setting. Methods: This prospective observational alpine field study was conducted over a period of 15 months. New insights into the mechanism of methoxyflurane nephrotoxicity and implications for anesthetic development (part 1): Identification of the nephrotoxic metabolic pathway.

BACKGROUND: Methoxyflurane nephrotoxicity results from biotransformation; inorganic fluoride is a toxic metabolite. Concern exists about potential renal toxicity from volatile anesthetic defluorination, but many anesthetics increase fluoride concentrations without consequence. Methoxyflurane is metabolized by both dechlorination to methoxydifluoroacetic acid (MDFA, which may degrade to fluoride) and O-demethylation to fluoride and dichloroacetatic acid. The metabolic pathway responsible for methoxyflurane nephrotoxicity has not, however, been identified, which was the aim of this investigation

[The effect of hypothermia and methoxyflurane-anaesthesia on sympatho-neuronal and sympatho-adrenal activity in the course of cardiac surgery (author's transl)].

The concentrations of adrenaline and noradrenaline, and dopamine-beta-hydroxylase in the plasma, and certain haemodynamic parameters, were determined in 14 children undergoing surgical correction of congenital cardiac defects under hypothermia at 30 degrees C and methoxyflurane anaesthesia. During the pre-operative phase of hypothermia at 30 degrees C, the adrenaline levels rose to about 300% of the inital levels, and the noradrenaline levels to about 200%. During the postoperative phase of re-warming at 34 degrees C, a further dysregulative release of catecholamines led to an increase in adre

Serum fluoride levels in ambulance staff after commencement of methoxyflurane administration compared to meta-analysis results for the general public.

OBJECTIVES: Ambulance officers administering methoxyflurane as an inhalational analgesic may be exposed to trace vapor. Fluoride is a methoxyflurane metabolite, and has been associated with acute renal failure in anesthesia patients and skeletal fluorosis with chronic elevated serum levels from other sources. However, there has been no direct measurement of serum fluoride in occupationally exposed ambulance officers. Thus, this study directly measures serum fluoride over a prolonged period in order to determine renal toxic and skeletal fluorosis risk to ambulance officers who are administering

Analgesic efficacy of inhaled methoxyflurane during transforaminal epidural steroid injection for lumbar radiculopathy: a randomized, double-blind placebo-controlled trial.

STUDY DESIGN: Randomized, double-blind, placebo-controlled trial. PURPOSE: To evaluate the efficacy and safety of inhaled methoxyflurane for pain relief during transforaminal epidural steroid injection (TFESI) in patients with lumbar radiculopathy. OVERVIEW OF LITERATURE: TFESI is a common intervention for lumbar radiculopathy, but procedural pain remains a concern. Methoxyflurane is a rapid-onset, inhaled analgesic with a favorable safety profile. While its effectiveness in procedural pain relief has been demonstrated in other medical settings, its role in TFESI remains underexplored. METH
