Efficacy and safety of microvascular decompression versus gamma knife surgery in patients with trigeminal neuralgia: an updated meta-analysis.

INTRODUCTION: Trigeminal neuralgia(TN) is a debilitating condition often requiring surgical intervention when medical therapy fails. Microvascular decompression (MVD) and Gamma Knife surgery (GKS) are the main treatment options, but their relative efficacy and safety remain debated. This meta-analysis aimed to compare outcomes between MVD and GKS in patients with TN. METHODS: We systematically searched PubMed, Embase, and Cochrane without temporal restrictions, in July 2025, for randomized controlled trials and observational studies that had as an intervention MVD versus GKS in patients with

Microvascular Decompression with Selective Use of MVD-Plus for Classical Trigeminal Neuralgia: Outcomes, Predictors, and Role in Redo Surgery.

BACKGROUND: Classical trigeminal neuralgia (TN) is a severe facial pain disorder most commonly caused by neurovascular conflict (NVC) at the trigeminal root entry zone. Microvascular decompression (MVD) provides durable pain relief by addressing the underlying vascular compression. However, Indian data evaluating predictors of failure, early relapse, redo surgery, and the role of adjunct nerve combing (MVD-plus) remain limited. OBJECTIVE: To evaluate functional outcomes after MVD for classical TN and identify predictors of early failure, pain relapse, redo surgery, and the role of MVD-plus.

Retrospective study of the combined application of the blink reflex and lateral spread reflex in microvascular decompression of facial nerve surgery.

BACKGROUND: The blink reflex (BR) can be used as a routine monitoring method during facial nerve microvascular decompression. This study aimed to investigate whether the use of the BR in hemifacial spasm (HFS) surgery is comparable to that of the lateral spread reflex (LSR), and to explore its significance for guiding intraoperative neurophysiological monitoring (IONM). METHODS: Patients undergoing facial nerve microvascular decompression from 2016 to 2018 were included in the study. According to the results of IONM, the intraoperative monitoring items of the BR and two conventional facial ne

Microanatomy Around the Facial Nerve Pathway for Microvascular Decompression Surgery Investigated with Correlative Light Microscopy and Block-Face Imaging.

BACKGROUND: Microvascular decompression for hemifacial spasm is performed at the root exit zone. More proximal segments of the facial nerve, defined as the root emerging zone (REmZ), may also be susceptible to neurovascular compression. Consequently, detailed knowledge of the microanatomy around facial nerve fibers at the pontomedullary junction is essential for consistent success of microvascular decompression. METHODS: Five human brainstems obtained from cadavers were investigated using correlative light microscopy and block-face imaging, which obtains arbitrary two-dimensional light micros

Endoscopic versus microscopic microvascular decompression for trigeminal neuralgia: equivalent pain outcomes with possibly decreased postoperative headache after endoscopic surgery.

OBJECTIVE Endoscopic surgery has revolutionized surgery of the ventral skull base but has not yet been widely adopted for use in the cerebellopontine angle. Given the relatively normal anatomy of the cerebellopontine angle in patients with trigeminal neuralgia (TN), the authors hypothesized that a fully endoscopic microvascular decompression (E-MVD) might provide pain outcomes equivalent to those of microscopic MVD (M-MVD) but with fewer complications. METHODS The authors conducted a single-institution, single-surgeon retrospective study with patients treated in the period of 2006-2013. Before

Enhancing surgical precision: a novel electromyography finding for confident identification of the root exit zone during microvascular decompression surgery.

OBJECTIVE: The lateral spread response (LSR) is an important electrophysiological sign that predicts successful decompression in patients undergoing microvascular decompression (MVD) for hemifacial spasm (HFS). However, LSRs do not consistently correlate with clinical outcomes, and there are cases in which LSRs are absent. In this study, the authors identified a unique pattern on facial nerve electromyography (EMG) when the root exit zone (REZ) is touched. This distinctive EMG pattern, which the authors coined the "Sang-ku sign" (SKS), could aid in identifying the offending vessel on the REZ,

Surgical Outcomes of Revision Microvascular Decompression for Persistent or Recurrent Hemifacial Spasm After Surgery: Analysis of Radiologic and Intraoperative Findings.

OBJECTIVES: Microvascular decompression (MVD) is the treatment of choice for hemifacial spasm (HFS), due to the high rate of complete resolution associated with MVD. However, some patients experience recurrent or persistent symptoms after surgery. In this study, we evaluated the causes of recurrence or failure based on our surgical experience with revision of MVD for HFS and analyzed the relationship between surgical outcomes and radiologic and intraoperative findings. METHODS: Among more than 2500 patients who underwent MVD surgery for HFS, 23 patients received a second MVD in our hospital f

Microvascular decompression surgery for trigeminal neuralgia and hemifacial spasm: naunces of the technique based on experiences with 100 patients and review of the literature.

BACKGROUND: Microvascular decompression (MVD) surgery for trigeminal neuralgia and hemifacial spasm offers a relatively low-risk opportunity to treat cranial nerve hyperactivity-compression syndromes, which are associated with severe, disabling facial pain and spasm. Although a number of publications have described the technique in detail, combining the technical nuances from different schools of thought or neurosurgical training in an effort to increase the safety and efficacy of this procedure would be beneficial to the surgeon. METHODS: The nuances of technique and operative findings from

Comparison of endoscope, exoscope, and microscope visualization during microvascular decompression for trigeminal neuralgia: a single-center experience of 135 surgeries.

OBJECTIVE: Microscopic microvascular decompression (MVD) is an effective, long-lasting treatment for trigeminal neuralgia (TN). Endoscopic MVD has shown comparable outcomes while identifying neurovascular compression in up to 28% of cases otherwise missed with the microscope. The extracorporeal telescope (exoscope) represents the newest visualization technique available to neurosurgeons, offering enhanced ergonomics and trainee education. The authors compared their institution's experience with endoscope-, exoscope-, and microscope-assisted MVD for TN. METHODS: The authors retrospectively rev

The alteration of the sensorimotor network in trigeminal neuralgia after microvascular decompression surgery: a follow-up study using independent component analysis.

INTRODUCTION: Trigeminal neuralgia (TN) is a chronic neuropathic pain disorder characterized by spontaneous or triggered electric shock-like facial pain. Microvascular decompression (MVD) is the most effective surgical intervention for classical TN that is refractory to medication. Recent advances in neuroimaging have enhanced visualization of the trigeminal nerve's vascular anatomy, deepening insights into TN pathophysiology and paving the way for improved diagnostics and therapies. Resting-state functional magnetic resonance imaging (rs-fMRI) has been extensively applied in studies of TN, un

Nervus Intermedius Section to Mobilize the Anterior Inferior Cerebellar Artery in Microvascular Decompression Surgery for Hemifacial Spasm: A Technical Case Report.

BACKGROUND: The course of the anterior inferior cerebellar artery (AICA) in the cerebellopontine angle may affect the technical options in microvascular decompression surgery for hemifacial spasm. A complex relationship between the AICA and the nervus intermedius has rarely been discussed in patients with hemifacial spasms. CASE DESCRIPTION: A 74-year-old woman presented with left facial spasms for 8 years. Intraoperative endoscopic observation confirmed that the left nervus intermedius crossed over the left AICA and limited its mobilization from the root exit zone (REZ) of the left facial ne

Using strength-duration analysis to identify the afferent limb of the lateral spread response in hemifacial spasm patients during microvascular decompression surgery.

Strength-duration analysis has been used to identify excitability differences between motor and sensory axons in human peripheral mixed nerves. The trigeminal and facial nerves have both been suggested to play a role in mediating the lateral spread response (LSR) in patients with hemifacial spasm (HFS). We sought to investigate this hypothesis by analyzing strength-duration properties of spasm side mentalis M wave and o. oculi LSR in 22 patients undergoing microvascular decompression surgery for HFS. Comparison of sagittal angle of trigeminal nerve at porus trigeminus between responders and non-responders to microvascular decompression surgery in patients with trigeminal neuralgia.

BACKGROUND AND PURPOSE: The sagittal angle of the trigeminal nerve at the porus trigeminus (SATNaPT) has been described as abnormally hyperacute in a subset of patients with trigeminal neuralgia and proposed as a potential marker for the likelihood of poor microvascular decompression (MVD) outcomes. The purpose of this study is to replicate these results and evaluate this hypothesized association by comparing the SATNaPT between MVD responders and non-responders. MATERIALS AND METHODS: At a single institution, 80 patients with a clinical diagnosis of trigeminal neuralgia, surgical interventio
