Myringotomy in the Junbo mouse model of chronic otitis media alleviates inflammation and cellular hypoxia.

OBJECTIVES/HYPOTHESIS: Ventilation of the chronically inflamed middle ear is a key outcome in functional middle ear surgery. Grommets eliminate middle ear effusion, but there is also evidence that they downregulate inflammation. The reason for this is not understood, but there is little to suggest alteration in eustachian tube ventilatory capacity. Previous work has shown that the Junbo mouse model of chronic otitis media has hypoxic middle ear mucosa and bulla fluid leucocytes. Here we explore whether surgical ventilation may alleviate chronic otitis media through downregulation of hypoxia.

Aspiration of middle ear effusions prior to grommet insertion an etiological factor in tympanosclerosis.

Fifty-five children with bilateral middle ear effusions were entered into a prospective randomized double blind trial to determine whether aspiration of middle ear effusions prior to ventilation tube insertion influences ventilation tube extrusion time or the development of tympanosclerosis. Following myringotomy, aspiration of the left or right ear only was performed randomly prior to ventilation tube insertion. The tympanic membranes were inspected six-monthly to determine whether ventilation tube extrusion had occurred, and at 24 months after surgery for the presence of tympanosclerosis. Re

Correlation between tympanometry volume and three-dimensional computed tomography mastoid volumetry in tympanoplasty candidates.

OBJECTIVE: Ventilation of the middle ear and mastoid air cells is believed to play an important role in the pathogenesis of chronic ear disease. Traditionally, ventilation is assessed by computed tomography. However, this exposes patients to cumulative radiation injury. In cases with a perforation in the tympanic membrane, tympanometry potentially presents a non-invasive alternative to measure the ventilated middle-ear and mastoid air cell volume. This study hypothesised that total tympanometry volume correlates with ventilated middle-ear and mastoid air cell volume. METHOD: Total tympanometr

Temporal bone pneumatization. A planimetric study.

This present study is based on the results of a planimetric investigation of temporal bone pneumatization in 73 children (with and without middle ear pathology). Radiologic and clinical follow-up studies indicate a definite inhibition of pneumatization due to chronic secretory or recurrent suppurative otitis media. This process is partially reversible if the underlying pathologic ventilation of the middle ear spaces is cured by an adequate treatment, such as direct adenoidectomy and/or long-term middle ear ventilation (6 months indispensable).

Experimental background for the use of tympanostomy tubes.

Already within 24 h, splitting of the soft palate in rats caused production of effusion in the attic space. During the next couple of days the effusion material filled the meso- and hypotympanon. The production of a serous fluid in the attic space appeared concomitant with an elevated histamine content in the middle ear cavity, irrespective of whether the ears were ventilated or not. In the artificially ventilated middle ears no effusion appeared in the meso- and hypotympanon and the hypotympanal mucosal lining remained normal. The serous effusion filling the middle ear cavities with the intac

Comparison of Two Different Scoring Systems in Encrusted Ureteral Stent Management: A Single-Center Experience.

BACKGROUND: Nasal obstruction is a common condition caused by various aetiologies, including deviated nasal septum, nasal polyps, and chronic rhinosinusitis. This condition can adversely affect the Eustachian tube function, leading to impaired middle ear ventilation, negative middle ear pressure, and associated symptoms such as ear fullness, discomfort, and hearing loss. Surgical intervention to correct nasal obstruction has been shown to improve Eustachian tube function and middle ear ventilation potentially, but comprehensive studies on this impact remain limited. OBJECTIVE: To evaluate the

Interactive effects of the middle ear pathology and the associated hearing loss on transient-evoked otoacoustic emission measures.

Sixty-three children aged 4 to 17 years were examined by tympanometry, pure-tone audiometry, transient-evoked otoacoustic emissions (TEOAEs), and otoscopy to evaluate the effects of middle ear pathologies and the associated hearing loss on TEOAEs. TEOAE measures were highly specific (93.8%) in identifying normal ears that passed both audiometric and tympanometric criteria. The sensitivity for identifying abnormal ears was also reasonable (83. 3%). The effects of the middle ear abnormality were most significant, regardless of the degree of hearing loss, when the tympanogram was type B with norm

Endoscopic Ear Surgery: Redefining Middle Ear Anatomy and Physiology.

Middle ear anatomy and physiology is highly complex, yet familiarity is important to perform middle ear surgery and understand surgically relevant ventilation pathways of the ear compartments. The middle ear is divided into five subspaces: the mesotympanum, the retrotympanum posteriorly, the epitympanum superiorly, the protympanum anteriorly, and the hypotympanum inferiorly. The Eustachian tube plays a crucial role in maintaining middle ear aeration and atmospheric pressure. There are two independent aeration routes of the epitympanum. Thanks to the advent of the endoscope, this anatomic and p

P-100 in the treatment of Ménière's disease: a clinical study.

Patients suffering from Ménière's disease are particularly sensitive to negative pressure in the middle ear. For example, attacks of vertigo can be triggered by a descent in an aircraft when ventilation of the middle ear can become critical. Positive-pressure pulse treatment has been shown to have a beneficial effect on the symptomatology and is now a true alternative in the treatment of Ménière's disease. In this study, we compared two devices that produced positive-pressure pulses delivered to the inner ear via the external ear canal and after the insertion of a middle-ear ventilation tube.

Effect of static middle-ear and intracranial pressure changes on differential electrocochleographic response.

In an animal model, we examined the extratympanic electrocochleographic response to static pressure changes (middle-ear pressure and intracranial pressure [ICP]) with attention to the summating potential (SP), action potential (AP), and the SP/AP ratio. With a closed middle ear, raised or lowered middle-ear pressure and raised or lowered ICP resulted in congruent increases of the SP and the SP/AP ratio, while the AP remained at a steady voltage. With a closed middle ear, raising or lowering ICP by positioning also had the effect of raising or lowering middle-ear pressure. With an open middle e

Middle ear pressure during brief pathological conditions of the nose and throat.

Middle ear ventilation was investigated by repeated tympanometry in different groups of patients with impaired air current in the upper airways. Among patients with total blocking of the nose by bilateral anterior packing, 46% had a negative pressure of -100 mm H2O or less, which quickly returned to normal after removal of the packing. Unilateral anterior packing produced no significant changes in middle ear ventilation. Unilateral posterior packing produced changes in 50% of ears, tonsillectomy in 60% of ears, amounting to a negative pressure of -100 mm H2O or less. Patients with infectious m

Analysis of factors associated with multiple ventilation tube insertions in children with otitis media with effusion.

OBJECTIVE: To determine the factors related to multiple ventilation tube insertions in children with otitis media with effusion. METHODS: A retrospective review was performed of 126 ears of 81 children aged less than 12 years who had undergone insertion of a Paparella type 1 ventilation tube for the first time between August 2012 and March 2018. RESULTS: Mean age at the first operation was 4.0 ± 2.2 years, and the mean duration of otitis media with effusion before the first ventilation tube insertion was 5.4 ± 4.5 months. Among 126 ears, 80 (63.5 per cent) had a single ventilation tube inser

[The CO2 laser otoscope. A new application device for paracentesis].

The CO2 laser myringotomy is a current low-pain procedure for middle-ear ventilation for secretory otitis media (SOM) that can be performed under topical anesthesia, even in children. The duration of middle-ear ventilation is essentially determined by the size of the perforation created. Perforations with a diameter greater than 2 mm ventilate the tympanic cavity for approximately 3 weeks, thus avoiding tympanic ventilation tubes. To date, the CO2 laser beam can only be reliably applied to the tymapanic membrane via a micromanipulator system coupled to an ear microscope. The CO2 laser otoscope

A correlative manometric and endoscopic study of tubal function in dry central perforation of the tympanic membrane.

Since good ventilation of the middle ear is a pre-requisite for successful myringoplasty, it was our policy to investigate the tubal function in dry central perforation of the eardrum and to correlate the manometric and endoscopic findings. Nasal endoscopy proved to be indispensable in diagnosing mechanical tubal obstruction and in localizing and even treating 'hidden' lesions in key areas, with probable normalization of the tubal function. The correction of the mechanical tubal obstruction must precede ear surgery. Whenever tubal obstruction is diagnosed as functional or idiopathic, the venti

Risk factors for persistent otitis media with effusion in children: a case-control study.

BACKGROUND: Otitis media with effusion (OME) is defined as middle ear effusion without acute signs of infection. OME usually resolves spontaneously; however, persistent OME may require the insertion of a ventilation tube. This study investigated risk factors for persistent OME in children who undergo ventilation tube insertion. METHODS: Children who were admitted to undergo ventilation tube insertion at Jeju National University Hospital between August 2015 and July 2016 were enrolled as the case group. Healthy children without persistent OME from August 2016 to July 2017 were enrolled as the
