Encountering children and child soldiers during military deployments: the impact and implications for moral injury.

Background: During a deployment, soldiers must make seemingly impossible decisions, including having to engage with child soldiers. Such moral conflicts may continue to affect service members and veterans in the aftermath of a deployment, sometimes leading to severe moral distress, anguish, and personal crises. Service providers have increasingly argued that as a diagnosis, Post-Traumatic Stress Disorder (PTSD) cannot account for these deeply personal and painful moral conflicts. In light of this, the concept of moral injury has been introduced to better capture the profound forms of guilt and

Effects of military service and deployment on clinical symptomatology: The role of trauma exposure and social support.

The Marine Resiliency Study-II examined changes in symptomatology across a deployment cycle to Afghanistan. U.S. Servicemembers (N = 1041) received clinical testing at two time points either bracketing a deployment (855) or not (186). Factor analyses were used to generate summary and change scores from Time 1 to Time 2. A between-subject design was used to examine changes across the deployment cycle with deployment (low-trauma, high-trauma, and non-deployed) and social support (low vs. high) as the grouping variables. Insomnia increased post-deployment regardless of deployment trauma (std. eff

Survival of U.S. Military Service Members with Hodgkin Lymphoma by Post-9/11 Deployment Status.

UNLABELLED: Classic Hodgkin lymphoma (CHL) commonly affects adolescents and young adults (AYA), including service members (SM). The impact of deployment on CHL outcomes remains unknown. TRICARE beneficiaries diagnosed with CHL from 2001 to 2022 were identified through the Department of War cancer registry. Deployment history came from the Defense Manpower Data Center, and mortality data came from the National Death Index. Mortality was compared between SMs with and without post-9/11 deployment, and associations with deployment-specific variables were evaluated using Cox models adjusted for dem

The Relationship between Allostasis and Mental Health Patterns in a Pre-Deployment French Military Cohort.

(1) Background: While a number of studies among military personnel focus on specific pathologies such as post-traumatic stress disorder (PTSD), anxiety, and depression, they do not address the cumulative impact on mental health of stressors related to the profession. The present study aims to determine the relationship between allostatic load and mental health status in a cohort of fit-for-duty soldiers prior to their deployment to Afghanistan. The aim is to better-define the consequences of stressor adjustment. (2) Methods: A cohort of 290 soldiers was evaluated in a cross-sectional study wit

Travelers' Diarrhea: An Update on the Incidence, Etiology, and Risk in Military Deployments and Similar Travel Populations.

Travelers' diarrhea (TD) has historically been a common illness among visitors to developing nations. Although recent studies indicate decreasing incidence of TD among short-term travelers, a systematic review of illness among long-term travelers, including deployed military personnel, has not been conducted in more than 10 years. We conducted a literature search of studies published between 2005 and 2015 that evaluated TD in populations of deployed military personnel or similar long-term travelers (travel ≥1 month) to developing nations. Our literature search identified 28 studies for inclusi

Military sexual trauma is associated with post-deployment eating disorders among Afghanistan and Iraq veterans.

OBJECTIVE: Evaluate the association of military sexual trauma (MST) screen status with eating disorder diagnoses among veterans within 1- and 5-years after initiating Veterans Health Administration (VHA) care, and whether the association varied by sex. METHOD: Retrospective cohort study of US Afghanistan/Iraq veterans who used VHA services between FY 2004 and 2014 (N = 595,525). This study used VHA administrative data to assess the presence of eating disorder diagnoses in medical records within 1- and 5-years of initiating VHA care, and whether a positive screen for MST was associated with ea

Military Occupational Outcomes in Canadian Armed Forces Personnel with and without Deployment-Related Mental Disorders.

OBJECTIVE: Mental disorders are common in military organizations, and these frequently lead to functional impairments that can interfere with duties and lead to costly attrition. In Canada, the military mental health system has received heavy investment to improve occupational outcomes. We investigated military occupational outcomes of diagnosed mental disorders in a cohort of 30,513 personnel who deployed on the Afghanistan mission. METHODS: Cohort members were military personnel who deployed on the Afghanistan mission from 2001 to 2008. Mental disorder diagnoses and their attribution to the

Influence of military component and deployment-related experiences on mental disorders among Canadian military personnel who deployed to Afghanistan: a cross-sectional survey.

OBJECTIVE: The primary objective was to explore differences in mental health problems (MHP) between serving Canadian Armed Forces (CAF) components (Regular Force (RegF); Reserve Force (ResF)) with an Afghanistan deployment and to assess the contribution of both component and deployment experiences to MHP using covariate-adjusted prevalence difference estimates. Additionally, mental health services use (MHSU) was descriptively assessed among those with a mental disorder. DESIGN: Data came from the 2013 CAF Mental Health Survey, a cross-sectional survey of serving personnel (n=72 629). Analyses

Risk of functional gastrointestinal disorders in U.S. military following self-reported diarrhea and vomiting during deployment.

INTRODUCTION: Military personnel are frequently deployed to regions of the world with high travelers' diarrhea (TD) rates. Pathogens associated with TD have been linked to several post-infectious sequelae, including functional gastrointestinal disorders (FGD), such as irritable bowel syndrome (IBS) and functional dyspepsia. Furthermore, stress associated with deployment may potentiate the increased FGD risk. AIM: We sought to assess whether self-reported diarrhea, vomiting, and stressors during deployment were associated with increased FGD risk. METHODS: Using active duty military medical en

Prevalence of Tinnitus and Association with Self-Rated Health among Military Personnel Injured on Combat Deployment.

INTRODUCTION: Tinnitus is an auditory problem frequently reported by military personnel and is currently responsible for 1 billion dollars annually in disability compensation. Recent military conflicts in Iraq and Afghanistan saw high levels of combat exposure coupled with a surge in blast weaponry, both of which can adversely affect hearing. The present study explored the prevalence of tinnitus and the association with self-rated health among military personnel injured during combat deployment. MATERIALS AND METHODS: A total of 1,026 U.S. military personnel who sustained an injury during ope

Operational impact of tongue piercing on French military pilots during overseas deployments: a case report.

Piercing in general and particularly tongue piercing is gaining popularity in the Western world. The French army and so the French air force are concerned by this phenomenon. A case report of a pilot suffering from dental pain with operational consequences due to a dental fracture during Operation Trident (Kosovo) is presented. The dental examination highlighted the fact that the dental fracture was directly due to the presence of a tongue piercing. Tongue piercing and wearing tongue jewelry have numerous adverse effects, especially frequent dental complications. If these complications do not

Environmental Presence of Burkholderia pseudomallei at Military Sites in an Endemic Region: Implications for Future Deployments and Studies.

The footprint of melioidosis is expanding globally, but its historical roots are in Southeast Asia and northern Australia. Melioidosis has long been described in military personnel deployed to melioidosis-endemic regions; however, the magnitude of the risk has not been quantified, and the nature of infecting events remains speculative. As with infections in the endemic population, the greatest concern is for inhalational melioidosis. Soil and air at four military locations in the highly melioidosis-prevalent environment of Darwin, Northern Territory, Australia, were sampled. Burkholderia pseud

The relative impact of injury and deployment on mental and physical quality of life among military service members.

US service members injured in the recent conflicts in Iraq and Afghanistan were more likely to survive than those in previous conflicts because of advances in medicine and protective gear. However, there is limited research examining the long-term impact of injuries while deployed on physical and mental quality of life (QOL) among service members. We used data from two time-points with an average follow-up period of 4.27 years (SD = 2.13; n = 118,054) to prospectively examine the association between deployment and injury status with QOL. Data were derived from the Millennium Cohort Study surve

Emotional distress and positive and negative memories from military deployment: the influence of PTSD symptoms and time.

During military deployment, soldiers are confronted with both negative and positive events. What is remembered and how it affects an individual is influenced by not only the perceived emotion of the event, but also the emotional state of the individual. Here we examined the most negative and most positive deployment memories from a company of 337 soldiers who were deployed together to Afghanistan. We examined how the level of emotional distress of the soldiers and the valence of the memory were related to the emotional intensity, experience of reliving, rehearsal and coherence of the memories,
