Trajectories of alcohol use in the UK military and associations with mental health.

INTRODUCTION: There are higher levels of alcohol misuse in the military compared to the general population. Yet there is a dearth of research in military populations on the longitudinal patterns of alcohol use. This study aims to identify group trajectories of alcohol consumption in the UK military and to identify associations with childhood adversity, deployment history and mental disorder. METHODS: Data on weekly alcohol consumption across an eight year period and three phases of a UK military cohort study (n=667) were examined using growth mixture modelling. RESULTS: Five alcohol trajecto

The long-term burden of military deployment on the health care system.

Health care providers need to be aware that stress complaints that result from deployment can emerge even after many years. This has important implications for health care policies. The main aim of this study is to investigate the relation between the development of posttraumatic stress and other mental health complaints and the burden on (mental) health care after a deployment. For this study we used data from a large prospective cohort study on stress-factors related to deployment in 1007 Dutch soldiers, who were deployed to Afghanistan. Participants were assessed at six follow up times up u

The consequences of modern military deployment on calcium status and bone health.

This article highlights the potential negative effect of the current combat environment on bone health of young military men and women who may be at risk for stress fractures and future bone disease because of alterations primarily in diet and physical activity level during deployment. A combination of physiologic biomarkers, including bone turnover and bone mineral density, and nutrition and exercise surveys can provide meaningful data on potential health risks related to deployment. Soldiers participating in an investigation into bone health before and after deployment did not have decreased

Military Veterans' Psychological Health and Physical Activity After Separation From Service.

INTRODUCTION: The transition from military to civilian life can bring about substantive challenges for U.S. veterans. The purpose of this study was to examine veterans' trajectories of psychological health prior to and after separation, and to examine whether veterans who engaged in more physical activity would report better psychological health over time. METHODS: Longitudinal data between 2001 and 2016 from the Millennium Cohort Study were analyzed, which consisted of U.S. military personnel who separated from service, followed up every 3-5 years. Veterans (N=37,464, Mage=36.3 years, SD=10.

Travel Health Needs of Children in U.S. Military Families Stationed Abroad.

INTRODUCTION: While the U.S. DoD mandates preventive health measures for service members overseas, the health needs of their accompanying children are poorly described. This study aims to quantify and characterize the recommended travel health preventive services, with attention to malaria, dengue, yellow fever, Japanese encephalitis, tick-borne encephalitis (TBE), typhoid, and rabies, in military-connected expatriate children. MATERIALS AND METHODS: The registered country of residence of active duty service members and their families in December 2022 as reported by the Defense Manpower Data

Avoidable Hospitalizations in the Military Health System, Fiscal Years 2018-2019.

INTRODUCTION: Ongoing health reforms in the Military Health System (MHS) are expected to shift locations of ambulatory care for up to 1.9 million beneficiaries. We sought to model the impact of this policy by determining potentially avoidable hospitalizations in the MHS based on different primary care settings. MATERIALS AND METHODS: We used the MHS Data Repository to conduct a retrospective cross-sectional study of TRICARE Prime and Prime Plus beneficiaries aged 18 to 64 years during fiscal years 2018-2019. Crude and adjusted risk ratios for each Agency for Healthcare Research and Quality pr

Oversight May Reduce Military Health System Leakage: Preventing Inappropriate Interhospital Transfers.

INTRODUCTION: The Military Health System (MHS) offers an example of a socialized healthcare model, operating within a larger "purchased care" civilian healthcare market. This arrangement has facilitated a trend wherein MHS clinicians often transfer moderate-to-complex patients to surrounding civilian hospitals, despite having the capability to care for such patients in-house. In an effort to stem this behavior, two initiatives were introduced at Carl R Darnall Army Medical Center (CRDAMC): A Transfer Policy Statement and Transfer Rounds. The Transfer Policy Statement emphasized that patients o

Psychoeducation as Precision Health in Military-Related Mild Traumatic Brain Injury.

A significant proportion of Service Members and Veterans (SMVs) experience at least 1 mild traumatic brain injury during military activities (mil-mTBI), which can result in enduring cognitive symptoms. Although multiple cognitive rehabilitation (CR) interventions have been developed for this population, patient psychoeducation focusing on biopsychosocial relationships and health behaviors is often cited as the first line of defense for mil-mTBI sequelae. However, theoretical and conceptual foundations of these psychoeducational techniques are not well articulated. This raises questions about t

Hormonal contraceptive use, neuromuscular performance, and musculoskeletal health during military training.

Despite data showing half of active-duty servicewomen use hormonal contraception, its impact on physical performance during military training remains unclear. This study examined differences in general and military-specific physical performance and bone health changes based on hormonal contraceptive use. One hundred eighty-five female Marine Officer Candidates (MOCs) were grouped by self-reported menstrual/hormonal contraceptive status. Physical activity, physical fitness and cardiometabolic health among Finnish military workers.

INTRODUCTION: The Western lifestyle challenges national defence. Inactivity, obesity, high BP and elevated lipid and glucose levels as well as tobacco use all increase cardiometabolic risk. The present study was thus aimed at investigating the health and physical activity of employees in a military environment, concentrating on comparisons between soldiers and civilians. METHODS AND DESIGN: A total of 260 employees from 6 brigades were included in the present study. Health status was evaluated with body composition, cardiometabolic risk markers from laboratory samples and a questionnaire conc

Fertility Preservation Knowledge, Perceptions, and Expectations Among Military Health Care Students.

INTRODUCTION: The rising demand for assisted reproductive technology and the expansion of health care coverage by Congress underscores the need to equip future military clinicians with knowledge about reproductive care and fertility preservation. Medical and graduate nursing students at the Uniformed Services University of the Health Sciences (USUHS) are uniquely positioned to both personally engage with and professionally educate patients about these services. This study's objective was to evaluate health care students' perceptions of age-related fertility decline, knowledge of fertility pres

Assessing Military Mental Health during the Pandemic: A Five Country Collaboration.

PURPOSE OF REVIEW: Members of a technical panel representing Australia, Canada, New Zealand, the UK, and the US collaborated to develop surveys designed to provide military leaders with information to guide decisions early in the COVID-19 pandemic. The goal of this paper is to provide an overview of this collaboration and a review of findings from the resulting body of work. RECENT FINDINGS: While surveys pointed to relatively favorable mental health and perceptions of leadership among military personnel early in the pandemic, these observations did not reflect the experiences of personnel de

Perinatal mental health and active-duty military spouses: a scoping review.

INTRODUCTION: Mental health conditions (i.e. depression or anxiety) are the most common complication of pregnancy and childbirth in the United States (U.S.) and are associated with increased maternal and infant morbidity and mortality. Research has demonstrated a relationship between stress and mental health diagnoses in pregnancy; therefore, it is concerning that military families face unique challenges which contribute to additional stressors among spouses of active-duty (AD) military personnel during the perinatal period. The objective of this scoping review was to understand the current st

U.S. military enlisted accession mental health screening: history and current practice.

Through the stimulus of war and concerns about neuropsychiatric disability, the U.S. military developed methods to rapidly screen the mental health of World War I and II draftees. Intelligence testing and brief psychiatric screening expanded the accession physical examination and underwent revision to identify only gross mental health disability. Supplemental psychiatric evaluations and written psychological screening tools were abandoned after postwar assessments; they demonstrated poor predictive power in evaluating recruit service capacity for combat environments. Currently, only three ment
