The Argyll Robertson pupil, a miotic pupil that fails to react to direct light, has been described for more than a century. Originally associated with tabes dorsalis, the sign has now been found in a number of conditions with lesions in the area of the nucleus of Edinger-Westphal. Magnetic resonance imaging studies have localized the lesion in patients with sarcoidosis and multiple sclerosis. With the declining incidence of neurosyphilis, the sign is increasingly likely to indicate another cause, although an assiduous search for lues should also be undertaken.

Low-luminance myopia of subjects with one normal eye and one aphakic eye was measured with a laser optometer. Low-luminance myopia was registered in the normal eyes, but no such myopia could be found in the aphakic eyes. The same subjects' aphakic eyes were further tested with miotic and dilated pupils. Miosis or mydriasis did not significantly alter the results as compared with measurements obtained with normal pupil size. These results would appear to be the first direct experimental evidence that accommodation is the primary cause of low-luminance myopia.

The effects of epinephrine on the permeability of the blood-aqueous barrier, stimulated by paracentesis of the anterior chamber, were studied in rabbits following alpha-adrenergic blocking by means of topical or systemic thymoxamine. The alpha-adrenergic block was tested by continuous recording of the systemic arterial blood pressure. The reversal of the vascular effects of epinephrine after administration of thymoxamine was demonstrated at the bloodaqueous barrier. The miotic property of thymoxamine is believed to have had no influence on our results.

Twenty patients with untreated primary open angle glaucoma (POAG) were tested for the presence of cholinergic supersensitivity of the iris using a 0.1% solution of pilocarpine. Nineteen patients had a significant miotic response compared to six of a group of twenty control subjects. We believe that an ocular parasympathetic neuropathy may be an important factor in promoting or perpetuating elevated intra ocular pressure in POAG. The mechanism of this is suggested and several possible areas of further study discussed.

Acute angle-closure glaucoma complicating ciliochoroidal detachment developed in eight eyes of six patients. The clinical presentation was uniform: extremely shallow central anterior chamber depth, flat peripheral anterior chamber, closed angle, and elevated intraocular pressure (IOP). In eyes undergoing vitreoretinal surgery up to now dilatation of a miotic pupil by the pupillary stretching technique could be performed only in aphakic eyes. Now a new needle with a tiny eye at its tip has been developed. With the help of this needle, 4 iris loop sutures can be led around the iris only passing the anterior chamber. This permits pupillary stretching also in phakic eyes. The modified technique was used on 3 phakic and 2 aphakic eyes.

Marfan syndrome is an autosomal dominant connective tissue disorder characterized by skeletal, cardiovascular, and ocular anomalies. Ectopia lentis is the most common ocular manifestation. We report an ocular sign not previously described in Marfan syndrome, iridocorneal adhesions secondary to anterior lens subluxation. Three patients with the Marfan syndrome had iridocorneal adhesions on slit-lamp examination. One patient developed adhesions following treatment with pilocarpine. An 18-year-old woman suffered cardiac arrest and became comatose after ingesting a large amount of proxyphene and salicylates. Six days after admission, her pupils, initially miotic, became fixed and dilated. On the seventh day disc edema developed. This was due to optic neuritis from acute Darvon overdose. Within eight weeks the disc edema resolved into a pale white atrophy with severe loss of vision. This is the first reported case of optic atrophy after propoxyphene overdose.

This 75-year-old woman had phacomorphic angle closure, dense nuclear sclerosis, deep set eye, miotic pupil and tight corneal wound during phacoemulsification. Phacoemulsification wound burn was noted at the end of surgery. Tenon was harvested from the inferior conjunctiva, placed over the gape and anchored by two radial corneoscleral 10-0 nylon. Ten days later, anterior optical coherence tomography showed good wound apposition and sutures were removed with visual recovery to 20/25 (6/7.5) without astigmatism.

Angle recession is a relatively common result of ocular injury, which, if extensive, creates a noteworthy risk of elevated intraocular pressure (IOP) and glaucoma. After the presentation of a newborn with congenital glaucoma, four additional members in two generations of a family were found to be affected with megalocornea. The absence of the disorder in the parents of three affected siblings can be explained by autosomal recessive inheritance, autosomal dominant inheritance or germ-line mosaicism. There is substantial evidence that prostaglandins (PG's) mediate acute elevations of intraocular pressure (IOP) due to laser irradiation of the iris. In the present study, we determined the optimal laser parameters needed to produce the maximal rise in IOP in anesthetized rabbits. Application of argon laser (0.75 watts, 0.5 sec. duration, and 8 spots of 500 micron size) to the iris of pigmented rabbits elicited an acute rise in IOP and miosis accompanied by an increase in aqueous protein concentration.
