[The usefulness and limits of the ambulatory monitoring of arterial pressure].

The interpretation of blood pressure values, measured by the physician in the medical environment, has some limitations. Blood pressure fluctuates considerably due to circadian rhythm and various environmental stimuli and thus casual readings may not be representative of the true blood pressure status of the patient. Non-invasive ambulatory blood pressure monitoring has greatly increased our knowledge regarding the diagnosis, treatment efficacy, natural history and even prognosis of hypertension. This technique allows the evaluation of repeated blood pressure measurements at given times (dayti

Access and reimbursement of ambulatory cardiac monitoring across Europe.

Ambulatory cardiac monitoring (ACM) allows long-term electrocardiogram (ECG) monitoring to detect arrhythmias with different modalities, ranging from short-term Holter monitoring (up to 48 h) to long-term continuous patch ECG monitors (up to 14 days), external event recorders (up to 30 days), and implantable loop recorders (ILRs). Access and reimbursement for ACM across Europe are not well understood. We performed a systematic review and analysis to understand ACM reimbursement across Europe, including a review of the reimbursement systems in each country and a detailed inspection of clinical

Optimizing ambulatory reflux monitoring: current findings and future directions.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is the most common diagnosis seen in outpatient gastroenterology clinics. The diagnosis is made by a variable combination of symptoms, response to acid suppressive therapy, endoscopic evaluation, and pH testing. In this review, we evaluate how to utilize various reflux testing in clinical practice based on current evidence. AREAS COVERED: Ambulatory reflux monitoring is a recognized diagnostic tool for clinical decision making in patients with/without established GERD, persistent reflux symptoms, and lack of response to proton pump inhibito

Ambulatory Blood Pressure Monitoring to Diagnose and Manage Hypertension.

This review portrays how ambulatory blood pressure (BP) monitoring was established and recommended as the method of choice for the assessment of BP and for the rational use of antihypertensive drugs. To establish much-needed diagnostic ambulatory BP thresholds, initial statistical approaches evolved into longitudinal studies of patients and populations, which demonstrated that cardiovascular complications are more closely associated with 24-hour and nighttime BP than with office BP. Studies cross-classifying individuals based on ambulatory and office BP thresholds identified white-coat hyperte

Number and timing of ambulatory blood pressure monitoring measurements.

Ambulatory blood pressure (BP) monitoring (ABPM) may cause sleep disturbances. Some home BP monitoring (HBPM) devices obtain a limited number of BP readings during sleep and may be preferred to ABPM. It is unclear how closely a few BP readings approximate a full night of ABPM. We used data from the Jackson Heart (N = 621) and Coronary Artery Risk Development in Young Adults (N = 458) studies to evaluate 74 sampling approaches to estimate BP during sleep. We sampled two to four BP measurements at specific times from a full night of ABPM and computed chance-corrected agreement (i.e., kappa) of n

Prognostic significance of ambulatory ECG monitoring for ventricular arrhythmias.

Ventricular arrhythmia can be detected in ambulatory ECG monitoring in individuals with or without cardiac disease, and its prognostic value varies, depending on the underlying condition. The use of continuous or intermittent ambulatory ECG monitoring can be helpful for diagnosis when there is a high pre-test probability of identifying a transient arrhythmia. In addition, Holter monitoring can be used for risk stratification of patients, in the context of the prognostic value of non-sustained ventricular arrhythmias in various clinical settings, as discussed in detail.

Applications of 24-hour noninvasive ambulatory blood pressure monitoring.

Casual blood pressure (CBP) measurements using a standard sphygmomanometer have traditionally constituted the principal modality for the assessment and management of hypertension. However, CBP measurement has shortcomings. Ambulatory blood pressure monitoring (ABPM) provides abundant information on blood pressure (BP), including heart rate, all BP readings for test periods, BP average, BP variability, BP load, load index, distribution pattern of BP, reduction percentage of BP, trough/peak ratio, and summary statistics for overall 24-hour, daytime and nighttime periods. Over the last three deca

Accelerometer-based body-position sensing for ambulatory electrocardiographic monitoring.

Our objective is to validate the ability of 3 appropriately placed accelerometers to determine body position during ambulatory electrocardiographic (ECG) monitoring and to demonstrate the clinical applicability of this method. During ambulatory (Holter) monitoring, the ability to know a patient's position (lying down, sitting, standing, or changing from one position to another) is important in the evaluation of common symptoms such as dizziness, palpitations, and syncope. Changes in body position are also known to alter the electrical axis of the heart, resulting in artifactual changes in QRS

Ambulatory 24-hour electrocardiographic monitoring following total cavopulmonary connection.

A total of 72 patients (53 males, 19 females) in the age range 1-22 years (mean 6.4 years) who underwent univentricular repair between April 1990 and May 1997 at our institute were followed up from six months to seven years (mean 3.6 years). Twenty-four hours ambulatory electrocardiographic Holter monitoring was done in all the patients. Thirty-four out of 40 patients (85%) who underwent fenestrated total cavopulmonary connection and 25 out of 32 patients (78%) who underwent non-fenestrated total cavopulmonary connection had normal Holter recording; thus 59 out of 72 (82%) patients had normal

Canadian hypertension society guidelines for ambulatory blood pressure monitoring.

The Canadian Hypertension Society has developed guidelines for the use of ambulatory blood pressure (BP) monitoring in clinical practice. Published articles with the best available levels of evidence were used to support the following recommendations: Physicians should only use ambulatory BP monitoring devices that have been validated independently using established protocols. A decision to withhold drug therapy based upon the ambulatory BP should take into account normal values for 24-h and awake ambulatory BP. Based upon studies on prognosis and a clinical trial based upon BP control as an o

Role of ambulatory urodynamic monitoring in clinical urological practice.

A retrospective review was conducted of 125 ambulatory monitoring (AM) studies performed on 111 women and 11 men between 1992 and 1996. All patients had had conventional cystometry prior to AM. All tests were interpreted with the aid of event markers and urinary diaries. Ambulatory traces were reported using the definitions derived from Coolsaet's work. To assess the role of AM in routine clinical urological practice, ambulatory diagnoses were compared with those of conventional cystometry. A survey of patient management was carried out by postal questionnaire or review of hospital notes to de

Ambulatory blood pressure monitoring: coming of age in nephrology.

The number of patients undergoing ambulatory blood pressure monitoring (ABPM) and the number of publications using this technique to evaluate the risks and effects of high blood pressure on target organs has been increasing, and dramatically so, in the last 5 years. Much of this growth has centered on the role of the blood pressure load (the percentage of systolic or diastolic readings above a preset value during a specific time period) and the changes in blood pressures levels that occur, with sleep. Although many studies are focused on the interaction between blood pressure (as assessed by A

Second-generation calcium antagonists and ambulatory blood pressure monitoring.

Ambulatory blood pressure monitoring (ABPM) is a particularly useful method for evaluating the effects of antihypertensive drugs. ABPM allows the therapeutic effect of an agent to be assessed continually by a large number of measurements, and the greater number of readings contributes to the higher degree of reproducibility associated with ABPM compared to other methods for measuring blood pressure. ABPM also enable measurements to be taken in "real-life" situations and removes the problem of observer bias. The number of patients required for clinical studies can be significantly reduced by us
