Head Kinematics Associated with Off-Field Head Injury Assessment (HIA1) Events in a Season of English Elite-Level Club Men's and Women's Rugby Union Matches.

OBJECTIVES: The purpose of this study was to investigate head kinematic variables in elite men's and women's rugby union and their ability to predict player removal for an off-field (HIA1) head injury assessment. METHODS: Instrumented mouthguard (iMG) data were collected for 250 men and 132 women from 1865 and 807 player-matches, respectively, and synchronised to video-coded match footage. Head peak resultant linear acceleration (PLA), peak resultant angular acceleration (PAA) and peak change in angular velocity (dPAV) were extracted from each head acceleration event (HAE). HAEs were linked t

Intermittent daytime mouthpiece ventilation successfully augments nocturnal non-invasive ventilation, controlling ventilatory failure and maintaining patient independence.

A 53-year-old woman with spinal muscular atrophy and a 7-year history of nocturnal non-invasive ventilation (NIV) use via nasal mask and chinstrap was admitted electively. Outpatient review suggested symptomatic hypercapnia and hypoxaemia. Use of her usual NIV resulted in early morning respiratory acidosis due to excess mouth leak, and continuous face mask NIV was instigated while in hospital. Once stabilised, she elected to return to nasal ventilation. At outpatient review, respiratory acidosis reoccurred despite diurnal use of NIV. Using the patient's routine ventilator and a novel mouthpiec

Fusion of maxillary and mandibular alveolar process together with a median mandibular cleft: a rare congenital anomaly.

Congenital fusion of the maxillary alveolar process and mandible is a very rare entity. A one day old male newborn was referred to our clinic with a diagnosis of a 'absence of oral cavity'. Physical examination revealed that there was no mouth opening bilaterally in the posterior region due to maxillary and mandibular bone fusion; and in the front there was only 1 mm of mouth opening. Examination of the intraoral space could not be achieved. No other congenital anomaly was detected on general examination. The infant, who was fed via a nasogastric tube for 1 month, was operated on. An intra-ora

[The management of chemically burned oral tissue--a case report].

Oral and perioral burns often result in post-healing microstoma, a shallow vestibule, tongue ankylosis, esthetic problems, and masticatory and speech impairments. The main cause being persistent extensive scar tissue contractions during the healing process. These disastrous sequelae can be minimized by surgical reconstruction. Use of special prosthesis may be required to provide scar tissue with the physical ability to resist contraction in post-injury and post-surgery stages. A twenty-eight-year old female was admitted to Makay Memorial Hospital in October 1986, with chemicals burn over the l

Physiological effects of a mouth-borne heat exchanger during heavy exercise in a cold environment.

A mouth-borne heat and moisture exchanger (HME) was tested. Nine healthy subjects performed an incremental-load cycle ergometry test to exhaustion, breathing once through the HME and once through a similar device without heat-exchange function (control). HME substantially increased inspired gas temperatures and decreased expired gas temperatures measured at the mouth; at 260 W (pulmonary ventilation (VE) approximately 1001 min-1) these changes amounted to + 15 degrees C and -5 degrees C, respectively. The breathing resistance was increased by HME but remained well within tolerable levels even

Head Acceleration Event Magnitude and Incidence Rate in Academy Rugby Union: A Comparison Across Club and International Competition.

BACKGROUND: Head acceleration events (HAEs) are an increasing concern in rugby union due to their association with brain injury and long-term neurological health. OBJECTIVES: This original research study aimed to establish essential baseline data on HAEs in under-21 professional rugby using instrumented mouthguards (iMGs) and to determine whether a higher competition level (professional club versus international) is associated with elevated magnitude and incidence rate of HAEs. METHODS: A prospective observational cohort design was used with 32 under-21 club academy players and 38 under-20 i

Contact-events and associated head acceleration events in semi-elite women's rugby union: A competition-wide instrumented mouthguard study.

This study aimed to quantify contact-events and associated head acceleration event (HAE) probabilities in semi-elite women's rugby union. Instrumented mouthguards (iMGs) were worn by players competing in the 2023 Farah Palmer Cup season (13 teams, 217 players) during 441 player-matches. Maximum peak linear acceleration (PLA) and peak angular acceleration (PAA) per-event were used as estimates of in vivo HAE (HAEmax), linked to video analysis-derived contact-events and analysed using mixed-effects regression. Back-rows had the highest number of contact-events per full-match (44.1 [41.2 to 47.1]

Self-inflicted injury in a case of Hallervorden-Spatz disease.

Hallervorden-Spatz disease (HSD) is a rare neurodegenerative disorder characterized by abnormally high deposits of iron in the brain. This report describes a child with HSD who presented with self-inflicted ulceration of the lip and tongue, which was initiated during periods of intense oro-facial spasms. Other findings included dental caries and trauma to the primary incisors. Comprehensive dental care was carried out under general anaesthesia. The self-mutilation of the oro-facial mucosa was eliminated by placement of upper and lower soft resin bite guards.

Capturing Head Impacts in Boxing: A Video-Based Comparison of Three Wearable Sensors.

Wearable sensors are used to quantify head impacts in athletes, but recent work has shown that the number of events recorded may not be accurate. This study aimed to compare the number of head acceleration events recorded by three wearable sensors during boxing and assess how impact type and location affect the triggering of acceleration events. Seven boxers were equipped with an instrumented mouthguard, a skin patch, and a headgear patch. Contacts to participants' heads were identified via three video cameras over 115 sparring rounds. The resulting 5168 video-identified events were used as re

Physics-Informed Machine Learning Improves Detection of Head Impacts.

In this work we present a new physics-informed machine learning model that can be used to analyze kinematic data from an instrumented mouthguard and detect impacts to the head. Monitoring player impacts is vitally important to understanding and protecting from injuries like concussion. Typically, to analyze this data, a combination of video analysis and sensor data is used to ascertain the recorded events are true impacts and not false positives. In fact, due to the nature of using wearable devices in sports, false positives vastly outnumber the true positives. Yet, manual video analysis is ti

On-Field Performance of an Instrumented Mouthguard for Detecting Head Impacts in American Football.

Wearable sensors that accurately record head impacts experienced by athletes during play can enable a wide range of potential applications including equipment improvements, player education, and rule changes. One challenge for wearable systems is their ability to discriminate head impacts from recorded spurious signals. This study describes the development and evaluation of a head impact detection system consisting of a mouthguard sensor and machine learning model for distinguishing head impacts from spurious events in football games. Twenty-one collegiate football athletes participating in 11

Development, Validation and Pilot Field Deployment of a Custom Mouthpiece for Head Impact Measurement.

The objective of this study was to develop a mouthpiece sensor with improved head kinematic measurement for use in non-helmeted and helmeted sports through laboratory validation and pilot field deployment in female youth soccer. For laboratory validation, data from the mouthpiece sensor was compared to standard sensors mounted in a headform at the center of gravity as the headform was struck with a swinging pendulum. Linear regression between peak kinematics measured from the mouthpiece and headform showed strong correlation, with r2 values of 0.95 (slope = 1.02) for linear acceleration, 1.00

Laboratory and On-field Data Collected by a Head Impact Monitoring Mouthguard.

Although concussion continues to be a major source of acute and chronic injury in automotive, athletic and military arenas, concussion injury mechanisms and risk functions are ill-defined. This lack of definition has hindered efforts to develop standardized concussion monitoring, safety testing and protective countermeasures. Recent research has provided evidence of the role of repetitive head impact exposure as a predisposing factor for the onset of concussion using developed instrumented helmets and mouthguards. To overcome this knowledge gap, we have developed, tested and deployed a head imp
