Vascular biomedicine in an era of chronic disease and multimorbidity.

It is increasingly common that patients present with more than one disease and that diseases are chronic in nature. Cardiovascular conditions such as hypertension, heart failure and stroke, renal diseases and cardiometabolic conditions such as diabetes are prime examples of chronic diseases which pose major challenges in contemporary healthcare provision. The complex features of multimorbidity call for precision medicine approaches that take comorbidity and chronicity into account. The research basis of chronic disease and multimorbidity, however, is currently in its infancy. This applies to a

[Multimorbidity and frailty as geriatric syndromes - Similarities and differences].

In the context of demographic trends, two geriatric syndromes that partially overlap but are conceptually distinct are becoming increasingly prevalent: multimorbidity and frailty. These two terms are often insufficiently distinguished from one another. However, they differ fundamentally in terms of their underlying concepts, pathophysiology, and clinical implications. A deeper understanding of these two entities is essential both for the individualized medical care of geriatric patients and for the prevention of functional decline in old age.

The Prognostic and Functional Impact of Multimorbidity in Systemic Sclerosis.

OBJECTIVE: Our objective was to define the frequency and impact of multimorbidity in systemic sclerosis (SSc). METHOD: Australian Scleroderma Cohort Study participants meeting American College of Rheumatology/EULAR criteria were included. Charlson Comorbidity Index scores were calculated at each visit, with multimorbidity defined as scores ≥4. Generalized estimating equations were used to model longitudinal data in multivariable models including age, sex, subclass, interstitial lung disease, and pulmonary arterial hypertension status. Survival was analyzed using Cox hazard modeling. RESULTS:

[A woman in her 50s with obesity, multimorbidity and seizures].

BACKGROUND: Obesity is a growing concern, leading to widespread use of weight-loss medications. Awareness of potential side effects and drug interactions is crucial when patients present with unexplained symptoms. CASE PRESENTATION: We report the case of a woman in her fifties who was admitted to hospital following a generalised tonic-clonic seizure that progressed to status epilepticus. She had been taking a bupropion/naltrexone combination for weight loss over the preceding three months, along with escitalopram for anxiety. Upon admission, she also exhibited fever, unstable blood pressure a

Oral health and multimorbidity: is diet the chicken or the egg?

Oral health is a critical component of overall health and well-being, not just the absence of disease. The objective of this review paper is to describe relationships among diet, nutrition and oral and systemic diseases that contribute to multimorbidity. Diet- and nutrient-related risk factors for oral diseases include high intakes of free sugars, low intakes of fruits and vegetables and nutrient-poor diets which are similar to diet- and nutrient-related risk factors for systemic diseases. Oral diseases are chronic diseases. Once the disease process is initiated, it persists throughout the lif

Interactive exploration of adverse events and multimorbidity in CKD.

BACKGROUND: Persons with chronic kidney disease (CKD) are at increased risk of adverse events, early mortality and multimorbidity. A detailed overview of adverse event types and rates from a large CKD cohort under regular nephrological care is missing. We generated an interactive tool to enable exploration of adverse events and their combinations in the prospective, observational German CKD (GCKD) study. METHODS: The GCKD study enrolled 5217 participants under regular nephrological care with an estimated glomerular filtration rate of 30-60 or >60 mL/min/1.73 m2 and an overt proteinuria. Cardi

Multimorbidity in people living with HIV and cancer in Mexico.

BACKGROUND: The proportion of older people living with HIV (PLWH) has increased. Non-communicable diseases occur earlier in PLWH than in the general population. OBJECTIVE: The goal of this study was to estimate the prevalence of comorbidities in PLWH and cancer in a tertiary referral center in Mexico City. MATERIAL AND METHODS: In this retrospective study, we included PLWH > 40 years with a history of cancer, coming to Instituto Nacional de Cancerologia from 2010 through 2019. All patients needed to be on antiretrovirals for at least six months. Data collected included cancer type, comorbidi

Multimorbidity in patients with monoclonal gammopathy of undetermined significance.

Monoclonal gammopathy of undetermined significance (MGUS), a precursor to multiple myeloma, is present in over 5% of adults aged 70 and older, a population with a high prevalence of multimorbidity. MGUS is often diagnosed incidentally when patients seek care for unrelated conditions. Our study sought to examine patterns of multimorbidity among MGUS patients, as overall health may impact patient care and the prioritization of MGUS surveillance. We examined patterns of comorbidities in 429 patients diagnosed with MGUS (2007-2015) and 1287 matched controls. Twenty-seven conditions were defined at

Addressing Multimorbidity and Polypharmacy in Individuals With Atrial Fibrillation.

PURPOSE OF REVIEW: The objectives of this review were to (1) discuss how multimorbidity and polypharmacy contributes to the complexity of management among individuals with AF and (2) identify any interventions to manage polypharmacy in relation to AF. RECENT FINDINGS: Based on the four landmark clinical trials of novel anticoagulants, the most common comorbidities with AF are hypertension, heart failure, diabetes, stroke and myocardial infarction. Polypharmacy was also found prevalent in 76.5% of patients with AF, with a median of six drugs per patient. Despite the consequences of polypharmac

Habitual fasting duration and accelerated multimorbidity in older adults.

BACKGROUND: Prolonged fasting has been linked to metabolic benefits in younger adults, but its effects on chronic diseases in older populations remain unclear. OBJECTIVE: To examine the association between habitual fasting duration and 15-year chronic disease accumulation in older adults, overall, within organ systems, and between age groups. METHODS: Data were drawn from the Swedish National study on Aging and Care in Kungsholmen, a longitudinal population-based cohort of 2981 predominantly community-dwelling adults aged ≥60 years. Habitual fasting duration was the longest interval between

The association between multimorbidity and arteriovenous fistula creation outcomes.

OBJECTIVE: The multimorbidity weighted index (MWI) quantifies both cumulative disease burden and its impact on physical functioning. This study examined the association between MWI and short-term outcomes of arteriovenous fistula (AVF) creation for hemodialysis access. METHODS: We performed a retrospective review of AVFs created between January 2011 and December 2019 of national Vascular Quality Initiative data linked to United States Renal Data Systems data. The primary exposure was multimorbidity as measured by the MWI. The MWI was dichotomized as above or below the median. The primary outc

Multimorbidity after renal transplantation: incidence and implications for patients.

PURPOSE OF REVIEW: Kidney transplant recipients are living longer with functioning grafts while remaining vulnerable to accumulating chronic conditions driven by preexisting CKD-related disease burden, ageing, and long-term immunosuppression. Multimorbidity is increasingly recognized as a determinant of survival, healthcare utilization, treatment burden, and quality of life, yet its trajectory after transplantation remains poorly characterized. RECENT FINDINGS: Recent literature shows that multimorbidity is common at transplantation and likely increases further afterwards, although posttransp

Sex-Specific Multimorbidity-Multibehaviour Patterns in Primary Care Populations.

BACKGROUND: A conceptual shift in healthcare emphasises multimorbidity and multibehaviours as interconnected phenomena, highlighting dose-response associations and sex-specific differences. Data-driven approaches have been suggested for overcoming methodological challenges, of multimorbidity research. By using exploratory factor analysis, this study aimed to identify sex specific lifestyle associative multimorbidity patterns, providing valuable evidence to primary care providers and informing future multimorbidity guidelines. METHODS: A retrospective observational study examined the electroni

Periodontitis and Systemic Inflammatory Multimorbidity: Narrative Review of Literature.

An increasing number of systemic inflammatory diseases, such as diabetes mellitus, rheumatoid arthritis (RA), cardiovascular disease, and respiratory disorders, have been linked to periodontitis, a chronic inflammatory condition of the gums. This review delves into the complex connections between systemic inflammatory multimorbidity and periodontitis, emphasizing how these illnesses are exacerbated by chronic inflammation and sharing similar pathophysiological pathways. Common inflammatory pathways that involve periodontal pathogens, like Porphyromonas gingivalis, and important cytokines, like

[Direct Oral Anticoagulants in elderly with multimorbidity: what precautions?].

Elderly with multimorbidity are at higher risk of accumulating direct oral anticoagulants (DOAC). The presence of renal insufficiency, an extreme age or weight, a limited functional reserve, and a polypharmacy that may interact with DOAC, lead to an increased risk of bleeding. A medication review and an evaluation of DOAC exposure in this population could help identify this risk and better manage it. This evaluation is conducted in collaboration with coagulation experts through calibrated measurement of anti-Xa (rivarobaxan, apixaban and edoxaban), along with D-dimers measurement as a potentia

Cardiometabolic multimorbidity and incident dementia: the Swedish twin registry.

AIMS: Cardiometabolic diseases (CMDs), including diabetes, heart disease, and stroke, are established risk factors for dementia, but their combined impact has been investigated only recently. This study aimed to examine the association between mid- and late-life cardiometabolic multimorbidity and dementia and explore the role of genetic background in this association. METHODS AND RESULTS: Within the Swedish Twin Registry, 17 913 dementia-free individuals aged ≥60 were followed for 18 years. CMDs [including age of onset in mid (60) or late (≥60) life] and dementia were ascertained from medical
