CASE: A 46-year-old man was involved in a high-energy collision resulting in a left traumatic hemipelvectomy with contralateral pelvic fractures and regional soft-tissue injuries. He had a pulseless left lower extremity and multiple life-threatening injuries. Emergent hemipelvectomy was performed, with intraoperative findings confirming external iliac artery transection. Remaining orthopaedic injuries were repaired sequentially, including percutaneous fixation of contralateral acetabular fracture.
CONCLUSION: Traumatic hemipelvectomy requires rapid intervention. Early amputation is necessary

Diabetic foot ulcer is a costly and serious complication of diabetes mellitus and is the major cause of non-traumatic limb amputations worldwide. Its development is primarily the result of diabetic neuropathy and/or peripheral arterial disease with accompanied bone abnormalities and is complicated by invasive infection. The management of this clinical condition focuses on identification of the "at-risk" foot, treatment of the ulcerated foot, and prevention of further complications. As diabetic foot ulcer represents the sum of multiple etiologies, its treatment requires a multidisciplinary team

A case of a 28-year-old man surviving traumatic hemipelvectomy is presented. This patient is the first reported survivor in Australia of traumatic hemipelvectomy and one of few survivors reported in the world literature. Accurate and rapid early management including good resuscitation, rapid transport to the operating room and an aggressive team surgical approach all contributed to survival. Acute complications can be kept to a minimum by adhering to the approach of rapid resuscitation and early intervention by a team of surgeons. Associated dislocation of the contralateral hip with sciatic ne

The authors describe the case of a 29-year-old man with multiple trauma who suffered compartment syndromes necessitating bilateral lower limb amputations as a result of the prolonged (9.5 hours) application of a pneumatic antishock garment (PASG). There was no evidence of lower limb trauma before the garment was put on. Despite the apparent benefits of the PASG in traumatized hypovolemic patients, the lowest possible inflation pressures should be used and removal attempted as soon as hemodynamic stability can be assured.

BACKGROUND: Stubby prostheses are nonarticulated prostheses used by persons with bilateral transfemoral/through-knee amputations (BTF) either exclusively or alongside full-length articulated prostheses. The implications of stubby prosthesis use on musculoskeletal function and movement patterns are unclear. This study aims to provide insights into the biomechanics of walking with stubbies.
METHODS: Gait data were collected using a 10-camera motion capture system from 4 male traumatic BTF who exclusively use stubbies and 4 male traumatic BTF who use multiple prostheses, including stubbies and f

PURPOSE: Physical traumatic injuries are likely to impact on a person’s Health-Related Quality of Life (HRQOL). We present data on HRQOL, from which EQ-5D-5L index scores are derived, and overall perceived health in UK Armed Forces personnel who sustained serious physical combat injuries and compare them to demographically similar personnel without such injuries (uninjured group). To ensure that perception of HRQOL is similar between groups, we also assess for measurement bias through Differential Item Functioning (DIF). METHODS: 577 personnel in an injured group (including subgroups of limb a

Four cases are described wherein massive centrifugal trauma caused a whirl-whiplash type injury to the spinal cord resulting in paralysis. Two patients became paraplegics and two quadriplegics. In three cases, there was a loss of one or more limbs associated with other concomitant injuries. Attention is focused on the problems facing patients with a combination of plegias and amputation.

Frida Kahlo's medical history shows sequelae of polio, a severe traumatic event that caused multiple fractures and a penetrating pelvic injury, as well as a history of countless surgeries. In her biographical accounts and her works, chronic disabling pain always appears for long periods. Besides, a chronic foot ulcer, gangrene that required amputation of the right leg, a history of abortions, and a positive Wasserman reaction suggest that the artist could have suffered from antiphospholipid antibody syndrome (APS).

Macroglossia is a potentially life-threatening problem. The multiple aetiologies responsible for this condition include cysts, tumours, amyloidosis, angioedema, and infections. Although injuries to the oral cavity are common, self-inflicted traumatic macroglossia is not common. We report a case of a comatose patient with limb contractures and severe trismus who developed entrapment of the tongue with progressive macroglossia and near auto-amputation of the tongue. The importance of early recognition and treatment of tongue entrapment in comatose patients is emphasised since this is a potential

OBJECTIVE: Traumatic popliteal artery injury is associated with an increased propensity for limb loss, morbidity, and mortality above an already elevated baseline risk to life and limb. Previous studies of outcomes in this patient group have been limited by selection bias. This study analyzed outcomes after blunt popliteal artery injury using propensity matching to reduce confounding variables associated with multiple mechanisms of traumatic vascular injury and to identify factors associated with amputation.
METHODS: A retrospective review was conducted of prospectively collected data from th

PURPOSE: Patients who sustain traumatic amputation of multiple fingers suffer both a functional and psychologic loss. Previous studies of prosthesis use for finger amputees have focused primarily on the psychologic benefits. Clinically our group noticed a functional improvement on hand function tests when patients with multiple digit amputations used a prosthesis. Given the expense of multiple finger prostheses we sought to determine if they led to a consistent functional improvement in these patients.
METHODS: Ten consecutive patients performed a battery of hand function tests and rated thei

BACKGROUND: KwaZulu-Natal bears a significant trauma burden, with polytrauma patients often experiencing traumatic limb amputations. This study investigates traumatic limb amputations in the subgroup of severely injured polytrauma patients admitted to the trauma ICU in KwaZulu-Natal. This study aims to describe the management and outcomes of traumatic limb amputations in polytrauma patients at the trauma ICU.
METHODS: This retrospective observational study utilised data from the trauma ICU registry (BCA207/09). Descriptive methods were employed to analyse demographics, injury mechanisms, timi

A case of a traumatic forearm amputation and associated complete avulsions of the ulnar and median nerves from the brachial plexus due to a crush-traction injury of the distal part of the right forearm is reported. The patient also suffered a traumatic head injury. The injury of the upper limb and the general condition of the patient were so serious that an amputation at the 1/3 middle part of the forearm had to be performed.

CASE: A traumatic hemipelvectomy (THP) is a rare life-threatening injury, with limited reports in the civilian population. We present the case of a patient who sustained a massive pelvic injury with an incomplete unilateral THP, a contralateral unstable pelvis, and a fracture of the acetabulum.
CONCLUSION: With advancements in resuscitative techniques, carefully timed and planned surgical interventions, and a coordinated multidisciplinary approach, a greater number of patients may be able to survive a massive pelvic injury.

The treatment of acute peripheral finger injuries is part of the daily routine of surgeons in emergency departments. This article presents the most common forms of peripheral finger injuries and the specific diagnostic and therapeutic aspects. The injuries include incision and tear injuries, injuries to the nailbed, distal extensor tendon injuries, severed flexor tendons of the distal joint, bite injuries, high-pressure injection injuries and amputation injuries of the distal phalanx of fingers. For the latter, the form, level and height of the amputation are decisive for therapy. Soft tissue
