Munchausen syndrome (MS), a complex form of factitious disorder (FD), presents significant diagnostic and management challenges in emergency and hospital settings. Patients deliberately fabricate or induce symptoms to gain medical attention, often leading to unnecessary interventions, resource misallocation, and iatrogenic harm. This study highlights the diagnostic complexity and the need for multidisciplinary management of Munchausen syndrome through a detailed case report and literature review. A 30-year-old woman presented with neurological symptoms mimicking Guillain-Barré syndrome (GBS),

In recent years factitious disorders have gained increasing significance in all medical specialties although in child psychiatry publications on this topic are rare. In this article these and other somatoform disorders are discussed with reference to a case report on a 14-year-old girl. Psychodynamic aspects of this syndrome are presented and differentiated from simulation. Conversion syndromes and hysterical neurosis and comparisons are made with other publications on this topic. Treatment of these patients, who repeatedly succeed in subjecting themselves to invasive and sophisticated diagnos

BACKGROUND: Child abuse is a major public health crisis and is on the rise. Dermatologists are frequently involved in its evaluation in differentiating abusive injuries from accidental injuries and in excluding pathological conditions that may mimic abuse. Battered child syndrome or physical abuse is not only a common form of child abuse but can also result from a rarer form of child abuse known as Munchausen syndrome by proxy. In this form of abuse, mother, who is the usual perpetrator, induces an illness or abuses the child for her own self-serving psychological needs.

Factitious Disorder Imposed on Another (FDIA), formerly known as Munchausen Syndrome by Proxy, is a rare form of abuse where parents induce or fabricate symptoms in their child, exposing them to unnecessary and potentially harmful medical treatments. In over 90% of cases, the perpetrators are mothers. A father was put on trial for the murder of his three-month-old daughter and was eventually convicted of repeatedly inducing respiratory crises, ultimately causing her death. Investigations began after multiple visits to the Emergency Department for respiratory episodes reported by the father but

Recurrent episodes of hypoglycemia are uncommon in non-diabetic patients. The workup investigation must confirm hypoglycemia and distinguish between endogenous versus exogenous hyperinsulinism. Simultaneous measurements of plasma glucose, insulin, C-peptide, and a screen for oral hypoglycemic agents should be performed. According to the results, further imaging studies may be necessary. A 43-year-old woman suffering from recurrent hypoglycemia presented to the emergency room (ER) with a hypoglycemic coma. She has had multiple episodes of documented hypoglycemia for the last 13 years. The case

Munchausen syndrome by proxy is a kind of child abuse in which affected children are often hospitalized for long periods and endure repetitive, painful and expensive diagnostic attempts. We present herein two toxicologically confirmed cases of Munchausen syndrome by proxy. Case 1 is a 16-month-old male who had fever, peripheral cyanosis, tremor, and reported cardiac arrest. Symptoms recurred in the hospital when the mother administered fluids. Toxicology detected 3.5 ng/ml mercury (Hg) in the fluid and 9.4 microg Hg/g creatinine in the urine. Case 2 is a 14-year-old female who had irregular bl

OBJECT: We report a case of intractable recurrent corneal erosion that was unresponsive to any medication, but was cured by psychiatric care after diagnosis of Munchausen syndrome.
CASE: The patient, a 35-year-old female, who developed pain in her left eye in July, 20 XX, was diagnosed with epidemic keratoconjunctivitis at the hospital where she worked as a nurse. Despite medication, the eye condition gradually worsened. After undergoing examination at many medical institutions, she came to our hospital on October 6, 20 XX. Her left eye manifested superficial punctate keratitis, then exhibite

Fifty four children were studied 1-14 (mean 5.6) years after fabrications of illness had been identified. Thirty of the 54 children were living in families with their biological mothers and 24 were with other family members or in substitute families. Further fabrications were identified for 10 children who had been living with their mothers and there were 'other concerns' for a further eight children. Thirteen children residing with mother and 14 not residing with mother at follow up had a range of disorders including conduct and emotional disorders, and problems related to school, including d

Renal and urologic problems in pediatric condition falsification (PCF) or Munchausen by proxy (MSP) can result in serious diagnostic dilemma. Symptoms of hematuria, pyuria and recurrent urinary tract infections have occasionally been described. However, MSP presenting as azotemia has not been previously reported. We describe the case of an unfortunate boy who had to undergo unnecessary hemodialysis for persistent hyperkalemia and azotemia before a final diagnosis of the falsification of investigations by the parents was made.

Medical child abuse or its better known name, Munchausen's Syndrome by Proxy, is a form of child maltreatment combining physical and emotional abuse as well as child neglect. As in other forms of child abuse and neglect that pediatricians encounter, the diagnosis is medical and is based on the existence of actual or potential harm to the child. A high index of suspicion is necessary when the medical team is faced with inexplicable findings as well as repeated treatment failure. There is extensive variability in the manifestation of this syndrome as well as a wide range of severity including th

Münchausen's syndrome is a psychiatric disease with behaviour disorders including self-injuries. Self mutilations interest all the body, and among others the abdominal wall, who can mimic complications of surgery. We report the history of a woman consulting for an abdominal wall panniculitis two years after a caesarean. Medical history, histological patterns and the psychological history of the patient excluded the diagnosis of a surgical complication and Münchausen's syndrome was strongly evocated. The treatment was surgical, with excision of the cutaneous necrosis. The healing was obtained b

Reports of comorbid eating disorders and Munchausen's syndrome are rare. As part of a follow-up study of the outcome of anorexia nervosa, medical records and direct structured psychiatric interview data were collected on all women treated for anorexia nervosa in Christchurch, New Zealand between 1981-1984. On interview, one participant received diagnoses of anorexia nervosa, bulimia nervosa, and factitious disorder (Munchausen's syndrome). The subject interwove symptoms of her eating disorders with factitious presentations on several occasions. This diagnosis had not been apparent prior to com

A woman presented to our emergency center after self-injection of human chorionic gonadotropin in an attempt to gain admission to the hospital. Her initial urine pregnancy test (an assay for beta-hCG) was negative. Subsequent blood samples received from the patient the same day revealed markedly elevated beta-hCG levels, suggesting possible laboratory error. Although the patient's sonogram was negative for an intrauterine pregnancy, an ectopic pregnancy could not be ruled out and the patient was taken to surgery, where no evidence of pregnancy was found. Retrospective evaluation of the patient

We report on two cases of factitial dermatosis of the breast associated with benign fibrocystic disease. One of the patients could also be diagnosed as showing Munchausen's syndrome. The lesions were protracted and intractable, painful ulcers that recurred and eventually became bilateral, despite attempts at surgical correction. The management of factitial dermatosis of the breast is complex, and should include psychiatric referral as well as avoidance of further surgical treatment if possible.

Background: Munchausen syndrome by proxy (MSBP) or factitious disorder imposed on another (FDIA) is a bizarre psychiatric entity, consisting of the fabrication of symptoms and alteration of laboratory tests by a caregiver. It is considered a serious form of child abuse. Alarm signs are frequent medical visits and strange symptoms that are never objectified during hospitalization. Methods: This case sets a bright light on how difficult the diagnosis and management of FDIA is and the severe consequences this disease has on a defenseless child. Results (Case report): A boy who is 3 years 8 months

Two children were thought to have an atypical gastroduodenal motility disorder because of the history and clinical course; both had received parenteral alimentation because of claims of inability to tolerate enteral feedings, and both continued to have unusual medical problems during parenteral alimentation. Both children had motility studies that were interpreted by a pediatric gastroenterologist to be "abnormal" and "diagnostic" of a motility disorder, but each was eventually shown to have a behavioral abnormality related to Munchausen syndrome by proxy.

Clinical history and inappropriate insulin secretion during hypoglycemic episodes permit the diagnosis of hyperinsulinism. We report 2 cases of factitious hyperinsulinism leading to partial pancreatectomy. Case 1 was an 8-year-old girl who presented with severe hypoglycemia and elevated insulin and C-peptide levels. Catheterization of pancreatic veins was performed to localize the excess insulin secretion. Insulinoma was suspected, and partial pancreatectomy was performed. Ten days after surgery, severe hypoglycemia recurred with severely elevated plasma insulin levels (x100) but very low C-pe

A four-month-old boy with pseudo-seizures was extensively investigated for fits and gastro-oesophageal reflux and subsequently treated for presumed epilepsy. It was finally established that his mother was inducing the "seizures" by asphyxiation and subsequent interviews established that information concerning the parents had been falsified.

The cluster of psychiatric concepts that includes "personality disorders," "psychopathy" and "moral insanity" has long been controversial and uncertain. This article investigates the concept of "psychopathy" in 1950s England and shows how this ambiguity is not a flaw or failure in the concept but absolutely necessary for the role it carries out: policing broad areas of social life. A case of Munchausen syndrome (a type of "psychopathy") in the late 1950s still functions as a precedent in the welfare system today, denying claimants sickness benefit, "closing a loophole," and exemplifying the us
