[Renal angioleiomyoma with a myogenic and melanocytic immunophenotype, as a variant of angiomyolipoma].

We present a case of a 67 year old male with a cortical nodular tumour of the left kidney. During a year's follow-up with ultrasound and MRI, the tumour was seen to increase in size by 16-20 mm. The nodule was surgically removed. Microscopically it was well defined and unencapsulated, with a proliferation of typical fusiform cells of smooth muscle cell appearance, clumped around well vascularized areas. Immunohistochemically, the neoplasm was positive for muscle markers (smooth muscle actin, desmin and caldesmon) and melanocyte markers (HMB-45 and Melan A). Our case would appear to be a renal

Primary vulvar extragastrointestinal stromal tumor in a 77-year-old woman.

Extragastrointestinal stromal tumors (EGISTs) carry the same morphological, immunohistochemical and molecular features as gastrointestinal stromal tumors (GISTs) and involve extragastrointestinal tract soft tissue. The majority of reported EGIST cases arise from intraabdominal, retroperitoneal, or pelvic soft tissue. A significant subset of such tumors originates from the gastrointestinal muscle layer, grows in an exophytic manner, then loses attachment to the gastrointestinal tract. Consequently, true EGISTs are exceedingly rare. Herein, we are reporting a case of a vulvar EGIST. A 77-year-ol

Interstitial Cells of Cajal in Deep Esophageal Leiomyoma.

Gastrointestinal stromal tumors (GISTs) are potentially aggressive mesenchymal neoplasms with spindle cell, epithelioid, or mixed morphology. They typically express CD117, DOG1, and CD34 and can be diffusely and strongly positive for h-caldesmon. Leiomyomas are benign smooth muscle neoplasms that can arise in a variety of visceral and soft tissue sites, including the gastrointestinal tract. We illustrate a case of a neoplasm of the gastroesophageal junction that was clinically suspected to be a GIST. Histology showed a tumor composed of ovoid and spindle cells arranged in short intersecting fa

Functional MR imaging of submandibular herniation of sublingual tissues through a gap of the mylohyoid muscle in two cases of submandibular "masses".

Except for neoplasms, in symptomatic patients with submandibular swellings, gaps of the mylohyoid muscle may be the cause of herniations of sublingual tissues, such as fat and/or the sublingual gland. In two patients with a submandibular swelling, MRI with standard sequences including contrast enhancement was performed to exclude a neoplastic lesion. In addition, we performed a trueFISP sequence during modified Valsalva's maneuver. In both patients, a neoplasm was excluded. Instead, the trueFISP sequence during the modified Valsalva's maneuver showed submandibular herniation of sublingual tiss

Superficial (Cutaneous) Tumors Exhibiting Morphological and Phenotypic Differentiation Toward Smooth Muscle and Pericytic Lineages.

Diagnosing low-grade spindle cell lesions of the skin can be challenging. In particular, distinguishing among fibroblastic, myofibroblastic, and smooth muscle or pericytic proliferations may be difficult during a routine histologic examination. This article provides an update on recent developments in superficial mesenchymal tumors exhibiting smooth muscle and pericytic differentiation. We focus on the characterization of clinicopathological features, differential diagnosis, and recurrent molecular alterations. Additionally, we address superficial leiomyosarcomas, which are now classified as a

PEComa: morphology and genetics of a complex tumor family.

Perivascular epithelioid cell tumors, or PEComas, are mesenchymal neoplasms composed of histologically and immunohistochemically distinctive epithelioid or spindle cells, which are immunoreactive for both smooth muscle and melanocytic markers. The cells in PEComas are typically arranged around blood vessels and appear to form the vessel wall, often infiltrating the smooth muscle of small- to medium-sized vessels. Periluminal cells are usually epithelioid and the more peripheral cells are spindle shaped. The cells have small, round to oval nuclei, sometimes with focal nuclear atypia, and clear

Acquired orbital retraction syndrome.

Four patients with infiltrative orbital disease involving an extraocular muscle demonstrated a characteristic retraction-motility pattern. The cardinal feature was retraction of the globe on attempted gaze opposite the field of action of the involved muscle. Variable eye movement limitation was present in the field of action and opposite to the field of action of the involved muscle. When motility was restricted, the force-duction test was positive. Modest proptosis and episcleal vascular congestion over the extraocular muscle insertion were associted physical findings. Dysthyroid eye disease,

Multifocal EBV-associated smooth muscle tumors in a patient with cytomegalovirus infection after liver transplantation: a case report from Shiraz, Iran.

INTRODUCTION: Immunodeficient patients, including the recipients of solid organs, exhibit an increase in the incidence of neoplasms. Post-transplant smooth muscle tumor (PTSMT) is a distinct and infrequent entity of these groups of neoplasms. Epstein-Barr virus (EBV) is considered to be involved in the etiology of this neoplasm. CASE REPORT: A 28-year-old man who underwent liver transplantation presented with abdominal pain and diarrhea for several months. He had a history of resistant systemic cytomegalovirus (CMV) infection after transplantation. Radiologic evaluation and colonoscopy reveal

Carcinogenicity of endrin.

Endrin is carcinogenic for rats, and most likely also for mice and dogs. Endrin caused significant incidences of malignant neoplasms at all sites. In one study, female rats were susceptible to the development of neoplasms of the endocrine organs, particularly carcinomas of the adrenal and pituitary glands as well as neoplasms of the reproductive system. In other studies, female rats tended to have carcinomas of the endocrine system, the mammary gland and reproductive system, and male and female rats lymphomas. Rats developed unusual malignant neoplasms, such as Kupffer cell sarcomas of the liv

Cotyledonoid dissecting leiomyoma: an uncommon form of a common disease.

Leiomyomas are benign uterine smooth muscle neoplasms with varied morphology that are well known to undergo secondary changes. Cotyledonoid dissecting leiomyoma is a rare and distinct form of leiomyoma that poses a diagnostic challenge for clinicians, radiologists, and pathologists and can be confused with malignant uterine neoplasms. Only a few cases have been reported so far in the literature. Here we report a case of a cotyledonoid dissecting leiomyoma in a 60-year-old woman, emphasize its gross and histological features, and provide a review of the literature.

Clinicopathological Study of Soft Tissue Neoplasms.

Background Soft tissue neoplasm is a broad term referring to tumors that can arise from various tissues, including muscle, tendons, fat, nerves, and lymphatic and blood vessels. These neoplasms may be either benign or malignant, with classification based on the tissue of origin and tumor behavior. The aim of this study was to assess the relative frequency of soft tissue neoplasms among all neoplasms, determine the incidence of benign and malignant soft tissue tumors (STTs), and examine variations in age, sex, anatomical location, and histological subtypes. Methods This descriptive study was co

Benign perivascular myoid cell tumor (myopericytoma) of the urinary tract: a report of 2 cases with an emphasis on differential diagnosis.

Myopericytoma is a benign mesenchymal neoplasm thought to comprise part of a spectrum of perivascular myoid cell neoplasms with myofibroma, angioleiomyoma, and glomus tumor. We describe 2 such neoplasms involving the urinary tract: 1 incidentally identified in the kidney of a 59-year-old woman and 1 in the urinary bladder of a 52-year-old woman who presented with urinary frequency and dysuria. Histologically, the bladder tumor was composed of numerous blood vessels surrounded by plump perivascular myoid cells, as in subcutaneous myopericytoma. The renal tumor showed similar morphology centrall

Perivascular Leiomyomatosis: A Unique Case Report.

There are 3 histologically benign smooth muscle neoplasms that have unusual growth patterns. These include intravascular leiomyomatosis, benign metastasizing leiomyoma, and leiomyomatosis peritonealis desseminata. We report a unique case of perivascular leiomyomatosis. The tumor showed multiple nodules of benign smooth muscle with some of the nodules closely associated with the periphery of the medial muscle layer of venous channels. All the neoplastic nodules were located on the outer surface of venous channels, thus precluding a diagnosis of intravascular leiomyomatosis. To the best of our k

Epithelioid leiomyosarcoma of the skin and subcutaneous tissue. Clinicopathologic, immunohistochemical, and ultrastructural study of five cases.

This report describes five cases of an unusual variant of malignant smooth-muscle neoplasm involving the skin and subcutaneous tissue characterized by a proliferation of predominantly round to oval epithelioid cells with abundant eosinophilic cytoplasm. Of the five patients, four were men, one a woman; ages ranged from 69 to 85 years (mean, 76.4). The size of the lesions varied from 1.2 to 2.5 cm; two were located in the scalp, two in the face, and one in the back of the neck. Immunohistochemical evaluation of the tumor cells showed immunoreactivity for actin and vimentin in all cases and nega

[Neoplasms of the base of the tongue. Evaluation with magnetic resonance].

The authors investigated MR capabilities in evaluating the neoplasms of the base of tongue. Thirty patients were examined. The images were obtained--at INI, Grottaferrata, Rome--using a 1.5 T superconductive Gyroscan unit by Philips. On T2-weighted images, the neoplasms were always hyperintense; in 10 patients edema could be distinguished from neoplasm. On T1-weighted scans the tumor exhibited the same signal intensity as the tongue muscle in 4 patients, while in the extant 26 cases it was hypointense. This pattern proved to be aspecific and could not be correlated with a specific histology. I

Clear-cell epithelioid leiomyoma of the round ligament.

A case of clear-cell epithelioid leiomyoma of the round ligament in a 69-year-old woman is described. The neoplasm presented as a firm left inguinal mass. A preoperative computed tomography (CT) scan demonstrated an enhancing lesion extending extra-abdominally from the region of the external inguinal ring. The surgical resection specimen was tan-white, well-circumscribed, and measured 5.8 cm in maximum dimension. Microscopic examination revealed a well-demarcated neoplasm comprised of polygonal cells with abundant clear cytoplasm arranged in clusters and single files with abundant intervening

Epstein-Barr virus-associated smooth muscle tumour in an immunosuppressed patient: a rare clinical scenario.

Epstein-Barr virus-associated smooth muscle tumours (EBV-SMTs) are rare neoplasms typically affecting immunosuppressed individuals such as organ transplant recipients. This case follows a woman in her 30s who presented with multi-organ masses 5 years after a kidney transplant. While initial suspicion focused on common post-transplant infections or malignancy, a liver biopsy revealed a smooth muscle neoplasm positive for EBV by EBV-encoded RNA in-situ hybridisation, consistent with EBV-SMT. Managing EBV-SMT is challenging as no standardised treatment regimens exist. When reducing immunosuppress

A limited immunocytochemical panel for the distinction of subepithelial gastrointestinal mesenchymal neoplasms sampled by endoscopic ultrasound-guided fine-needle aspiration.

We studied the use of immunocytochemical analysis with material procured by endoscopic ultrasound-guided fine-needle aspiration (EUS-guided FNA) for the diagnosis of subepithelial intramural gastrointestinal (GI) mesenchymal neoplasms (SIGIMNs). We identified all EUS-guided FNA specimens of SIGIMNs that had undergone immunocytochemical analysis. Results were compared with follow-up histologic diagnoses. There were 95 aspirates that were diagnosed as GI mesenchymal tumors (GI stromal tumors [GISTs], n = 46), leiomyomas (n = 38), peripheral nerve sheath tumors (n = 5), and other neoplasms by cyt
