Musculoskeletal manifestations and autoimmune diseases related to new biologic agents.

PURPOSE OF REVIEW: The anti-tumor necrosis factor agents are now widely used in the management of patients with rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis and juvenile inflammatory arthritis. One of the most common observations made during their use is the development of autoantibodies. The purpose of this paper is to review this phenomenon and its clinical implications. RECENT FINDINGS: While the development of different autoantibodies is a common encounter, rare cases of lupus-like syndromes have been reported. On the other hand, a variety of immune-mediated clinical

Infectious diseases affecting the musculoskeletal system in children and adolescents.

Many infectious organisms can lead to musculoskeletal symptoms in children. This article reviews recent studies on rheumatic fever, invasive group A streptococcal infections, septic infections of bones and soft tissues, reactive arthritides caused by viruses, fungi or bacteria, and Lyme disease. The search for etiologic organisms in chronic arthritis continues. Although Koch's postulates are not fulfilled in most cases of infectious arthropathies, progress has been made in diagnosing and treating these diseases.

Role of the Osteocyte in Musculoskeletal Disease.

PURPOSE OF THE REVIEW: The purpose of this review is to summarize the role of the osteocyte in muscle atrophy in cancer patients, sarcopenia, spinal cord injury, Duchenne's muscular dystrophy, and other conditions associated with muscle deterioration. RECENT FINDINGS: One type of bone cell, the osteocyte, appears to play a major role in muscle and bone crosstalk, whether physiological or pathological. Osteocytes are cells living within the bone-mineralized matrix. These cells are connected to each other by means of dendrites to create an intricately connected network. The osteocyte network ha

Musculoskeletal Findings in Behcet's Disease.

Behcet's disease is a multisystem disease characterized by recurrent oral and genital ulcers, relapsing uveitis, mucocutaneous, articular, gastrointestinal, neurologic, and vascular manifestations. Rheumatologic manifestations may also occur in Behcet's disease, and arthritis and arthralgia are the most common musculoskeletal findings followed by enthesopathy, avascular necrosis, myalgia, and myositis. Although the main pathology of Behcet's disease has been known to be the underlying vasculitis, the etiology and exact pathogenesis of the disease are still unclear. Musculoskeletal findings of

Musculoskeletal manifestations of Lyme disease.

Musculoskeletal symptoms in Lyme disease are very common at all stages of the disease. Lyme arthritis, whether intermittent or chronic, is a hallmark of late Lyme disease. This may cause severe joint pain and swelling especially confined to one or a few joints, most notably the knee. Antibiotic therapy is very effective in treating Lyme arthritis in the majority of cases. However, a small proportion of individuals will develop persistent chronic arthritis which is likely mediated through immunologic mechanisms. In these patients treatment strategies should include anti-inflammatory medications

Therapeutics of musculoskeletal disease in the horse.

Therapeutic medications play a crucial role in the successful therapy of many musculoskeletal diseases that occur in horses. For example, appropriate antibiotic therapy is extremely important in the treatment of diseases caused by infections with microorganisms such as botulism, tetanus, osteomyelitis, and muscle abscesses. In addition, numerous prescription medications and nutritional supplements are available for the treatment of osteoarthritis in horses. Many of these agents currently on the market fall into a new class of drugs called SADMO agents. Unfortunately, the efficacy and mechanism

Musculoskeletal manifestations of Lyme disease.

Musculoskeletal involvement, particularly arthritis, is a common feature of Lyme disease. Early in the illness, patients may experience migratory musculoskeletal pain in joints, bursae, tendons, muscle, or bone in one or a few locations at a time, frequently lasting only hours or days in a given location. Weeks to months later, after the development of a marked cellular and humoral immune response to the spirochete, untreated patients often have intermittent or chronic monoarticular or oligoarticular arthritis-primarily in large joints, especially the knee-during a period of several years. The

Lyme disease: musculoskeletal manifestations.

A previously unrecognized musculoskeletal syndrome led to the recognition of this "new" infectious disease. Several distinct patterns of musculoskeletal involvement can be seen throughout the course of untreated Lyme disease. Diffuse, nonspecific muscle achiness and stiffness can be seen early to be followed by characteristically brief, recurrent episodes of LIMP and transient arthritis that help to differentiate LD from other arthropathies. Chronic oligoarticular arthritis is seen in a few. Lyme arthropathy most closely resembles the reactive arthropathies. In a few cases, LD has mimicked oth

Musculoskeletal consequences of coeliac disease.

Coeliac disease is no longer confined to the gastrointestinal tract. Increasing evidence has positioned it as a systemic condition with profound implications for bone, muscle, and joint health. Yet, the musculoskeletal consequences of coeliac disease remain largely under-recognised, underdiagnosed, and undertreated. This narrative review critically explores the multifaceted pathophysiology linking gluten sensitivity to skeletal fragility, sarcopenia, and autoimmune arthropathies, integrating findings from paediatric to elderly populations. Nutritional deficiencies, chronic inflammation, immune

Musculoskeletal hydatid disease is rare and can be located anywhere but most commonly the bone and muscles of the spine, pelvis, then the lower limbs. Imaging is essential for its diagnosis, performing the pre-therapeutic assessment, guiding possible percutaneous treatments, and providing post-therapeutic follow-up. Musculoskeletal hydatidosis can take several forms that may suggest other infections and tumors or pseudotumors. MRI and CT are superior for its diagnosis but ultrasound and radiography remain the most accessible examinations in developing countries where this parasitosis is endemi

Phenylbutazone, musculoskeletal disease, and leukemia.

Four hundred and nine patients age 30 and over with leukemia or lymphoma and a subset of 127 patients with myelocytic leukemia were compared with equal numbers of matched hospitalized control subjects and matched subscribers to the Kaiser-Permanente Medical Care Program (member controls). Prior use of phenylbutazone was somewhat more frequent in cases than in member controls only, but the difference was not statistically significant and the association appeared to be explained by an underlying relation of musculoskeletal disease to these lymphohematopoietic cancers. The association of musculos

Current-future burden concerning musculoskeletal diseases and high body mass index.

This study aims to systematically assess the global burden of musculoskeletal (MSK) diseases caused by high body mass index (BMI) and to analyze its temporal trends and cross-national inequalities. Data were extracted from the Global Burden of Disease 2021. We quantified the temporal trends by calculating the estimated annual percentage change of the age-standardized DALYs rate (ASDR). Joinpoint regression analysis was used to identify the significant turning points in the ASDR. The cross-national inequality situation was evaluated using the sociodemographic index (SDI). The Bayesian Age-Perio

cPLA2 in musculoskeletal and autoimmune diseases: Molecular mechanisms and therapeutic insights.

Cytosolic phospholipase A2 (cPLA2) is a central regulator of lipid signaling that links cytokine and growth factor signaling to arachidonic acid metabolism and downstream eicosanoid production. Dysregulated cPLA2 activity is increasingly recognized as a shared pathogenic mechanism across musculoskeletal and autoimmune diseases, including osteoarthritis, rheumatoid arthritis, intervertebral disc degeneration, osteoporosis, inflammatory myopathies, sarcopenia and Duchenne muscular dystrophy, as well as inflammatory bowel disease and multiple sclerosis. This review provides a focused and mechanis

The role of TRIM proteins in chronic inflammation-associated musculoskeletal diseases.

Musculoskeletal disorders (MSDs), including osteoarthritis (OA), rheumatoid arthritis (RA), osteoporosis (OP), and intervertebral disc degenerative disease (IVDD), are among the most common and serious health problems worldwide. However, despite extensive research, drug therapy for these diseases remains a major challenge. This is likely due to a poor understanding of the underlying mechanisms. Recent evidence suggests that the development of these MSDs is associated with protein dysregulation and ongoing chronic inflammation. Protein turnover is controlled by the ubiquitin-proteasome system (

Vagus nerve stimulation as a novel therapeutic approach for musculoskeletal diseases.

Vagus nerve stimulation (VNS) has gained significant attention as a therapy for various medical conditions due to its ability to modulate chronic diseases, pain, and inflammation. VNS delivered by an implanted device is FDA approved for severe epilepsy and refractory depression. VNS delivered with implantable devices or transcutaneous methods are now being studied in several musculoskeletal diseases including osteoarthritis, rheumatoid arthritis, systemic lupus erythematosus, and fibromyalgia. VNS activates the neuroimmune axis including the cholinergic anti-inflammatory pathway (CAP), suppres

Vaccine hesitancy and bone health: Musculoskeletal sequelae of vaccine-preventable diseases.

Vaccine hesitancy threatens global health by diminishing herd immunity and permitting the return of vaccine-preventable diseases (VPDs) including poliomyelitis, measles and coronavirus disease 2019 (COVID-19). These infections carry short-term and long-term musculoskeletal consequences, such as paralysis, limb deformities, septic or reactive arthritis, osteomyelitis and chronic pain, that place an ever increasing burden on orthopaedic services. This narrative review synthesizes clinical and epidemiological evidence linking sub-optimal immunization to orthopaedic morbidity caused by major VPDs.

Preventative and therapeutic potential of tocotrienols on musculoskeletal diseases in ageing.

Musculoskeletal health is paramount in an ageing population susceptible to conditions such as osteoporosis, arthritis and fractures. Age-related changes in bone, muscle, and joint function result in declining musculoskeletal health, reduced mobility, increased risk of falls, and persistent discomfort. Preserving musculoskeletal wellbeing is essential for maintaining independence and enhancing the overall quality of life for the elderly. The global burden of musculoskeletal disorders is significant, impacting 1.71 billion individuals worldwide, with age-related muscle atrophy being a well-estab
