[Comparative efficacy of island latissimus dorsi myocutaneous flap versus lower trapezius myocutaneous flap for repair of posterior neck skin and soft tissue defects].

OBJECTIVES: Posterior neck skin and soft tissue defects often result from electrical burns, post-tumor resection, infections, or trauma. The region has complex anatomy, high mobility, and proximity to the cervical spine, major vessels, and critical nerves; thus, reconstruction must balance functional restoration and aesthetic outcomes. The island latissimus dorsi myocutaneous flap, with its reliable blood supply, large harvestable area, and long rotation arc, is widely used for trunk and limb reconstruction and shows particular advantage for large defects in the thoracic, dorsal, and posterior

Head and neck reconstruction using infrahyoid myocutaneous flaps.

CONTEXT AND OBJECTIVE: The use of pedicled myocutaneous flaps in head and neck reconstruction is widely accepted. Here we describe our experience with infrahyoid flaps (IHFs) employed to cover surgical defects in the oral cavity and oropharynx in patients with benign and malignant tumors. The aim was to evaluate the success rate for infrahyoid myocutaneous flap procedures performed at a single institution. DESIGN AND SETTING: Retrospective study, at the Head and Neck Surgery Service, Unicamp. METHODS: Fourteen IHFs were used to reconstruct surgical defects in eleven men (78.5%) and three wom

[The myocutaneous sternocleidomastoid flap for reconstruction of the mouth cavity and pharynx].

BACKGROUND: Among the myocutaneous island flaps applied for reconstruction of large soft-tissue defects after ablative surgery for malignomas, the pectoralis major flap is the one most frequently used. In comparison, the sternocleidomastoid myocutaneous island flap is not as popular. METHODS: We report on our experiences with the superiorly based sternocleidomastoid myocutaneous (SCM) island flap (cutaneous island of 6-8 cm in diameter) in seven consecutive cases after resection of malignomas of the oral cavity and the pharynx. The vascularization from the occipital artery is additionally sup

Short gracilis myocutaneous flaps for vulvoperineal and inguinal reconstruction.

From October of 1990 to April of 1993, 16 short gracilis myocutaneous flaps and 1 short gracilis muscular flap were used to reconstruct vulvoperineal, inguinal, perineal, and ischial soft-tissue defects. Five of the 6 bilateral myocutaneous flaps were used for vulvoperineal reconstruction after radical vulvectomy combined with partial vaginectomy and one radical vulvectomy. Four unilateral myocutaneous flaps and one muscular flap were used for inguinal, suprapubic, ischial, and perineal reconstruction after release of contracted scar or excision of an ischial pressure sore. The immediate compl

[Free myocutaneous flap of the latissimus dorsi in cervicofacial surgery].

Free myocutaneous latissimus dorsi transplants are exceptionally used in cervicofacial surgery. The authors have performed an anatomical study of 23 non-embalmed subjects, using injections of neoprene latex and barium sulfate into the axillary artery and dye injections. Dissections, arteriographs and corrosion show that the lower scapular pedicle is constant, with an average length of 9.5 cm and a caliber that is sufficient for vasuclar microsurgery. Eight patients with large T4 cervicocephalic neoplasms were operated, including two with tongue tumors, one with a tumor of the oropharynx, two o

Repair of urinary tract fistulas with bulbocavernosus myocutaneous flaps.

Urinary tract fistulas resulting from severe trauma or pelvic irradiation are often associated with extensive tissue loss, scar formation, and fibrosis. Two cases, one with a urethro-vaginal fistula secondary to trauma and one with a vesico-vaginal fistula secondary to irradiation, are presented. In neither case could the bladder, urethra, or vagina be repaired primarily. Using a bulbocavernosus myocutaneous "island" flap, the fistulas were successfully repaired. The anatomy of the graft and the operative procedure are described. This new procedure should be considered in urinary tract fistula

Delayed failure of myocutaneous flaps in head and neck reconstruction.

Since the concept of myocutaneous flap reconstruction of the head and neck region was introduced it has opened a new approach to surgical management in this area. This flap not only reduced the number of operations as compared to "staged procedure", but also reduced the costs of medical care. We are, however, experiencing significant failures as the applications of this flap are extended. This paper reviews our experience with myocutaneous flaps in 15 partial and total failures. An evaluation of these failures reveals that most occurred 1 1/2 to 3 weeks after reconstruction. The factors predis

The lower trapezius myocutaneous flap for head and neck reconstruction.

The lower trapezius myocutaneous flap has proved to be another useful adjunct in the reconstructive armamentarium of the head and neck surgeon. The flap is based on the descending branch of the transverse cervical artery and vein. Providing these structures are protected during neck dissection, this flap may well become the first consideration of flaps for reconstruction of extensive defects of the head and neck. The flap is safe, reliable, and easy to raise, with constant vascular anatomy. The donor site functional deficit is minimal and the donor site may be closed by direct closure without

Reconstruction of large pharyngeal defects with microvascular free flaps and myocutaneous pedicled flaps.

PURPOSE OF REVIEW: The current review will focus on the therapeutic options for reconstruction of large and complex defects of the oropharynx and hypopharynx, and the cervical esophagus following surgery for squamous cell carcinoma. The advantages and disadvantages of pedicled flaps, including the pectoralis major myocutaneous flap (PMMF) and supraclavicular artery flap (SAF), as well as the fasciocutaneous free flaps, including the radial forearm free flap (RFFF), the anterolateral thigh flap (ALT), and the jejunum free flap, are reviewed with particular emphasis on the literature from the pa

Successful use of muscle flaps or myocutaneous flaps in patients with sickle cell disease.

Sickle cell disease presents an unusual challenge to the reconstructive surgeon. The interaction between the underlying hemoglobinopathy and the circulatory mechanics in pedicled flaps leads to a high incidence of flap necrosis in patients with this disease. We present 3 patients with sickle cell disease in whom the use of axial flaps allowed the repair of difficult reconstructive problems in one stage, without preoperative exchange transfusions. The rationale for this approach is discussed.

Rescue Tracheal Reconstruction via Platysma Myocutaneous Flap Following Necrosis of Sternocleidomastoid Muscle Myoperiosteal Flap.

A case of a 60-year-old male patient with thyroid cancer invading the trachea was presented in the current study. After the initial surgery via a sternocleidomastoid muscle (SCM) clavicular periosteal flap for tracheal reconstruction, the patient experienced flap necrosis. Subsequent debridement and tracheocutaneostomy were performed, followed by a successful repair of the tracheal defect using a platysma myocutaneous flap. The case underscores the critical role of preserving the SCM's integrity to ensure adequate blood supply and reduce the risk of flap necrosis. A staged approach, involving

[The latissimus flap (myocutaneous flap). Experiences after 35 operations for repair of thoracic defects].

We used the latissimus flap (myocutaneous flap) for the first time in July 1974 for closing defects of the thoracic wall. We published the first results at the beginning of 1976 [6]. In the meantime we have used this method in 40 patients with good success. We report here the results in 35 patients with critical assessment. Many other authors have in the meantime reported on the successful use of this flap [1, 4, 5, 7, 8, 10].

Sternoclavicular Joint Infections: Improved Outcomes With Myocutaneous Flaps.

Sternoclavicular joint (SCJ) infection is a rare disease and its management remains controversial. Our institution has adopted an aggressive surgical approach of radical SCJ resection combined with myocutaneous flap (MCF) closure whenever possible. We reviewed our experience with this approach in the management of this condition. From July 2004 to June 2018, 50 consecutive patients were treated surgically for SCJ infections. Patient demographics, imaging studies, microbiology, and operative variables were analyzed. All patients underwent ipsilateral SCJ resection. Wound closure was performed w

Muscular, myocutaneous, and fasciocutaneous flaps in complex urethral reconstruction.

The ability to achieve a long-term, stable, stricture-free, hairless urethral lumen in patients with complex anterior stricture and compromised genital skin is one of the ongoing challenges of reconstructive urologic surgery. The conservative approach by endoscopic urethrotomy or dilatation with a self-catheterization schedule rarely affects a definitive cure except in the short filmy superficial strictures of the bulbous portion of the urethra. Genital fasciocutaneous island flaps are currently the golden standard for definitive, reliable resolution of anterior urethral strictures in patients

Is the pectoralis myocutaneous flap in intraoral and oropharyngeal reconstruction outdated?

METHODS: Our experience with 224 immediate pectoralis major myocutaneous flap reconstructions in patients with carcinomas of the oral and oropharyngeal cavities is presented. RESULTS: Although flap-related complications developed in 53% of the patients, all flaps survived, and we had no major skin paddle loss. The incidence of reoperation due to flap-related complications was 2%. All other complications were minor and did not affect the length of hospitalization. Analysis showed no significant risk factors for the development of complications. Because of fistula formation, infection, or metal

Gynecologic reconstruction with a rectus abdominis myocutaneous flap: an update.

This series reports the outcomes and significant complications associated with the rectus myocutaneous flap when used for pelvic or inguinal reconstruction in patients with gynecologic cancers. Perioperative variables were retrospectively reviewed to identify social and medical risk factors as well as intraoperative and postoperative complications that predisposed to rectus flap failure. Fifteen patients with gynecologic malignancies underwent reconstructive procedures using a vertically oriented rectus abdominis myocutaneous flap for either vaginal (n = 14) or inguinal (n = 1) reconstruction.

Perineal reconstruction using single gracilis myocutaneous flaps.

Bilateral gracilis myocutaneous flaps were originally used as part of a technique for creating a neovagina following total pelvic exenteration. Based upon this experience, we began using single flaps for primary repair and closure of large surgical defects in the perineal area that require alternate tissue sources to replace lost skin, mucosa, or adjacent deep tissues. Eighteen single gracilis flaps were used for major vulvovaginal reconstructions in 17 women during the past 5 years. Women undergoing unilateral flap reconstructions included 6 with anorectal cancers and 11 with vulvovaginal tum
