Air dissection of the spaces of the head and neck: A new teaching and dissection method.

Our goal was to evaluate a new air dissection technique for the spaces of the head and neck using fresh-frozen cadavers. Eight sides from four fresh-frozen Caucasian cadavers were used in this study. Compressed air was initially placed into the pterygomandibular space while simultaneously observing the spaces of the head and neck. Subsequently, the pterygomandibular space on the contralateral side of the specimen was insufflated and observations made. For these methods, simultaneous observation of mediastina and lungs was made using intrathoracic endoscopy. On all sides, the buccal, submental,

Impact of neck dissection in early tongue and buccal cancer without neck extension.

PROBLEM: The role of elective neck dissection in early stage tongue and buccal squamous cell carcinoma with negative neck lymph nodes is still controversial. METHODS: We retrospectively reviewed patients with T1-2N0M0 buccal and tongue cancer who underwent primary tumour excision with or without elective neck dissection between January 1997 and December 2006. RESULTS: Elective neck dissection specifically improved disease-free survival of T2N0M0 buccal cancer and overall survival of T2N0M0 tongue cancer. CONCLUSION: Elective neck dissection seems to improve the disease-free survival rate of

Incidental parathyroidectomy in thyroidectomy and central neck dissection.

BACKGROUND: Although higher thyroidectomy volume has been linked with lower complication rates, its association with incidental parathyroidectomy remains less studied. The volume relationship is even less clear for central neck dissection, where individual parathyroid glands are at greater risk. METHODS: Patients undergoing thyroidectomy with or without central neck dissection were evaluated for incidental parathyroidectomy, hypoparathyroidism, and hypocalcemia. Univariate and multivariable analyses were performed using binary logistic regression. RESULTS: Overall, 1,114 thyroidectomies and

Vertebral artery dissection after a chiropractor neck manipulation.

The differential diagnosis for ischemic central nervous system infarcts in young patients includes paradoxic emboli through cardiac shunts, vasculitis, and vascular trauma. We report a young woman who developed headache, vomiting, diplopia, dizziness, and ataxia following neck manipulation by her chiropractor. A computed tomography scan of the head revealed an infarct in the inferior half of the left cerebellar hemisphere and compression of the fourth ventricle causing moderate acute obstructive hydrocephalus. Magnetic resonance angiography revealed severe narrowing and low flow in the intracr

Neck pseudo-bruising secondary to acute aortic dissection.

We report a case of lethal acute aortic dissection revealed by a large ecchymotic area at the root of the neck; the subject was an 86-year-old man who died suddenly in his hospital room. His medical history included ischemic heart disease with pacemaker implantation and an aortic surgical prosthesis. Forensic autopsy showed a major subcutaneous hemorrhage in the neck and large left pleural and pericardial adhesions secondary to thoracic surgery. The cause of death was an acute aortic dissection. Large adhesions led to an extensive mediastinal hemorrhage, spreading to the subcutaneous tissues o

Elective neck dissection in papillary thyroid carcinoma patients.

BACKGROUND: This study was designed to review the diagnostic performance of cervical lymph node metastases for assessment of elective neck dissection in papillary thyroid carcinoma patients and to determine the technique (using Methylene blue dye) that demonstrated the highest success rate with regard to the detection rate and sensitivity. METHODS: Preoperative 24 h, 1 mL of 1.0% methylene blue was injected into the parenchyma surrounding the primary tumor with ultrasound guidance by a 27-gauge needle. Intra-operative lymph nodes that were stained blue and/or having afferent lymphatic channel

[Salvage central neck dissection for papillary thyroid cancer].

OBJECTIVE: To evaluate the salvage central neck dissection (CND) for papillary thyroid cancer in the patients with suspicious residual positive lymph nodes in the central compartment. METHODS: A total of 85 cases undergoing salvage CND between January 2006 to January 2009 was reviewed. RESULTS: Fifty-nine (69.4%) of the 85 patients showed residual positive lymph nodes in the central compartments. The number of dissected lymph nodes for each dissection ranged 2 - 13 nodes, with an average of 5.7 nodes. A total of 159 positive lymph nodes were found in the 59 cases, with an average of 2.7 posi

Efficacy of neck dissection: are surgical volumes important?

UNLABELLED: OBJECTIVES/HYPHOTHESIS: Surgical volumes have been shown in many fields to reflect expertise and to be associated with improved clinical outcomes. We sought to test similar hypotheses that neck dissection is a volume-sensitive procedure wherein the number of lymph nodes harvested and clinical outcomes are correlated with the number of neck dissections performed. METHODS: We conducted a retrospective analysis of 375 neck dissections. The number of nodes harvested was examined by multiple linear regression before and after mathematical correction for pathologists' variation over tim

Vertebral artery dissection presenting with isolated neck pain.

Unilateral pain in the cervical region and limitation of neck movements are nonspecific symptoms frequently encountered in daily medical practice. Vertebral artery dissection is rarely considered as a diagnostic possibility unless brainstem or cerebellar ischemia follows the acute pain. Three cases of vertebral artery dissection (VAD) having the sole complaint of pain of acute onset in the posterior neck region are presented. None of the patients had ever reported a similar pain, and the neurological examination was unremarkable in all of them. Doppler ultrasonography suggested VAD in 2 cases,

Neck and mediastinal node dissection in pharyngolaryngoesophageal tumors.

BACKGROUND: Specific reports about neck node metastasis in cervical esophageal tumors and mediastinal node metastasis in patients with pharyngolaryngoesophageal tumors are lacking. This study was undertaken to evaluate the need for neck and mediastinal lymph node dissection when dealing with carcinomas of this region. METHODS: A retrospective review of the records of 34 patients who underwent total pharyngolaryngoesophagectomy and gastric transposition (TPLEGT) for squamous cell carcinoma of the pharyngoesophageal junction was done. Sixteen patients had esophageal carcinomas, 14 had hypophary

Modified neck dissection for metastatic nonseminomatous testicular carcinoma.

OBJECTIVE: To examine the role of neck dissection in the treatment of metastatic stage 3 nonseminomatous germ-cell tumors (NSGCTs) of testicular origin. METHOD: A retrospective review was made of 45 patients with metastatic NSGCT who underwent 48 unilateral and 3 bilateral neck dissections. Only level III-VI nodes were dissected, often with concomitant or staged mediastinal dissection, thoracotomy, and/or retroperitoneal node dissection. Occasionally, resection of the clavicle, jugular vein, or subclavian artery, or a combination of these, was required to eradicate the disease. RESULTS: Ther

Vertebral artery dissection due to minor neck trauma.

Vertebral artery dissection has been previously reported following minor head and neck trauma. Such activities as rapid head turning, tennis, yoga, and vigorous exercise have been implicated. We report a case of vertebral artery dissection following minor neck trauma suffered in a volleyball game. The anatomy of the vertebral artery, pathophysiology of dissection, and emergency department recognition and treatment of this disorder are discussed.

Adult respiratory distress syndrome after radical neck dissection.

The clinical management of an unusual case of postoperative ARDS is reported. A few hours following neck surgery and septic insult, the patient developed unexpected ARDS. Aetiologic and supportive treatment were successfully instituted and after 72 hours of intensive therapy, the patient's clinical status improved. The very short time lapse between the septic insult and appearance of ARDS is emphasized. A brief literature review on aetiology, diagnosis and therapy of sepsis, as well as some pertinent aspects concerning the pathogenesis of ARDS and its linkage to sepsis are presented.

Investigations into shoulder function after radical neck dissection.

In order to determine shoulder function after radical neck dissection, and to evaluate the outcome of postoperative physical treatment, 43 patients were investigated 10 days up to 1 month after this procedure. Shoulder function was judged by means of (a) clinical investigation of the shoulder girdle and by (b) electromyographic testing of the trapezius muscle. Our results demonstrated a correlation between the extent of atrophy and clinical parameters such as abduction and lateral displacement of the scapula. Electromyography revealed damage present mainly in the descending part of the trapezi

Neck Dissection and Survival Among Head and Neck Cancer Patients Undergoing Adjuvant Immunotherapy.

BACKGROUND: Some studies suggest that neck dissection (ND) should be avoided in candidates for immunotherapy because lymph nodes are primary sites for immunotherapy activation. Our study investigates ND utilization and associated differences in overall survival (OS) among patients with head and neck cancer (HNC) undergoing adjuvant immunotherapy. METHODS: The 2013-2018 National Cancer Database was retrospectively reviewed for patients with HNC undergoing surgery with curative intent, and adjuvant immunotherapy.
