Variations in umbilical cord clamping practices in the United States: a national survey of neonatologists.

OBJECTIVE: Since the first publication of the American College of Obstetricians and Gynecologists committee opinion in 2012, and following the update in 2017, multiple institutions in the United States (US) adopted the practice of delayed cord clamping (DCC) and/or umbilical cord milking (UCM) in preterm and term infants. However, there have been variations reported in practices with regard to method of placental transfusion, timing of cord clamping and gestational age thresholds. Furthermore, the optimal cord clamping practice in situations of depressed infants needing resuscitation or in hig

Application of Neonatologist Performed Echocardiography in the assessment and management of persistent pulmonary hypertension of the newborn.

Pulmonary hypertension contributes to morbidity and mortality in both the term newborn infant, referred to as persistent pulmonary hypertension of the newborn (PPHN), and the premature infant, in the setting of abnormal pulmonary vasculature development and arrested growth. In the term infant, PPHN is characterized by the failure of the physiological postnatal decrease in pulmonary vascular resistance that results in impaired oxygenation, right ventricular failure, and pulmonary-to-systemic shunting. The pulmonary vasculature is either maladapted, maldeveloped, or underdeveloped. In the premat

Neonatologist-performed point-of-care functional echocardiography in the neonatal intensive care unit.

Functional echocardiography (fECHO) refers to a bedside, limited assessment of the ductus arteriosus, myocardial performance and pulmonary or systemic haemodynamics that is brief in nature and addresses a specific clinical question or management dilemma. This point-of-care ultrasonography is increasingly used internationally and locally among neonatal units to assist with management of neonatal haemodynamic conditions. This article intends to explain the modality, its indications, interpretation and implications for management, and how it impacts long-term outcomes, particularly in chronic lun

A definition of gentle ventilation in congenital diaphragmatic hernia: a survey of neonatologists and pediatric surgeons.

Ventilation practices have changed significantly since the initial reports in the mid 1980 of successful use of permissive hypercapnia and spontaneous ventilation [often called gentle ventilation (GV)] in infants with congenital diaphragmatic hernia (CDH). However, there has been little standardization of these practices or of the physiologic limits that define GV. We sought to ascertain among Diaphragmatic Hernia Research and Exploration; Advancing Molecular Science (DHREAMS) centers' GV practices in the neonatal management of CDH. Pediatric surgeons and neonatologists from DHREAMS centers co

A study of congenital cardiac disease in a neonatal population--the validity of echocardiography undertaken by a neonatologist.

OBJECTIVES: To estimate the incidence of, and profile the spread of, congenital cardiac defects, and to assess the accuracy of the echocardiographic diagnosis as performed by a neonatologist. DESIGN: Hospital-based study. METHODS: All neonates meeting our criterions, specifically those with a persistent murmur 48 hours after birth, underwent an echocardiographic examination to exclude an underlying congenital cardiac defect. All scans performed by the neonatologist were videotaped and reviewed by a paediatric cardiologist. We assessed concordance according to inter-rater agreement. RESULTS:

"The weight of words": a phenomenological study of Chinese neonatologists' lived experiences of delivering bad news.

BACKGROUND: Delivering bad news in the Neonatal Intensive Care Unit (NICU) constitutes a profound ethical and emotional challenge. In China, this complexity is intensified by a defensive medical culture and strained physician-family dynamics. This study explores the lived experiences of Chinese neonatologists navigating these high-stakes interactions to uncover the phenomenon's essential structure. METHODS: A descriptive phenomenological study was conducted. Using a purposive sampling strategy, 19 neonatologists from a tertiary NICU in Western China were recruited. Data were collected through

Differing attitudes toward phenobarbital use in the neonatal period among neonatologists and child neurologists in Sweden.

OBJECTIVE: In spite of its known negative effects on brain development, phenobarbital is the most commonly used drug for neonatal seizures. We conducted a national survey among neonatologists and child neurologists to determine the variation in practice and attitudes. METHODS: Surveys were sent to all board-certified child neurologists and neonatologists in Sweden as well as to residents in subspecialty training. In addition to questions concerning respondent demographic characteristics, the survey contained questions related to overall attitudes and practices and specific patient scenarios.

Accuracy of cardiac auscultation in asymptomatic neonates with heart murmurs: comparison between pediatric trainees and neonatologists.

The aim of this study was to assess whether cardiac auscultation performed by pediatric trainees and neonatologists can reliably distinguish innocent from pathologic heart murmurs in asymptomatic neonates. From January 2008 to April 2009 the pediatric trainees of our institution were requested to refer all asymptomatic neonates with a murmur and classify the murmurs as "innocent," "pathologic," or "possibly pathologic." Prior to echocardiography, each neonate was evaluated by two experienced neonatologists. The echocardiographic studies of 169 neonates were analyzed. Abnormal cardiac anatomy w

Neonatologist-Performed Echocardiography versus Pediatric Cardiologist for the Patent Ductus Arteriosus in Preterm Newborns.

Objective: Premature babies have a fragile circulatory system balance; therefore, rapid inter- vention may be needed in patients with patent ductus arteriosus (PDA). Echocardiography performed by a neonatologist can help to choose the best treatment option for newborns in a timely manner. The aim is to evaluate the performance of neonatologists in ductus assessment. Materials and Methods: Among the neonates born at the hospital, those diagnosed with PDA (n=40) under 32 weeks of gestational age were evaluated with echocardiography by a neo- natologist and a pediatric cardiologist. Role of neonatologist-performed echocardiography in the assessment and management of patent ductus arteriosus physiology in the newborn.

Neonatologist-performed echocardiography (NPE) is an indispensable tool in the haemodynamic management of critically ill newborn infants. NPE is used to facilitate timely diagnosis of a patent ductus arteriosus (PDA) in preterm infants and to assess its haemodynamic significance. Before treatment is considered, it is obligatory to confirm structural cardiac normality. Importantly, NPE offers the ability to guide therapeutic interventions, allowing an individualised haemodynamic management approach to the PDA. After discussing PDA pathophysiology, an overview is provided on the role of NPE in t

Early Detection of Isolated Transient Right Ventricular Hypertrophy in a Term Neonate: the Neonatologist's Role.

Right ventricular hypertrophy (RVH) is a rare finding in the neonatal age. Multiple pathological conditions are associated with the development of RVH, such as metabolic diseases, hereditary disorders and premature closure of the ductus arteriosus during the intrauterine life. Transient isolated RVH may be due to perinatal factors such as perinatal distress and corticosteroid treatment of preterm neonates with bronchopulmonary dysplasia. We report a case of a term neonate, appropriate for gestational age presenting transient hypertrophy of the right ventricle, who was diagnosed with echocardio
