BACKGROUND: Early treatment with nasal continuous positive airway pressure (CPAP) in newborns with respiratory distress syndrome is useful, by recruiting alveoli and restoring the functional residual capacity.
POPULATION AND METHODS: Nasal CPAP was supplied by the Infant Flow Driver (Electro Medical Equipment). From 15 June 1994 to 15 December 1994, 42 neonates received nasal CPAP. Their mean birthweight and gestational age were 1511 +/- 411 g and 30.9 +/- 2.5 weeks, respectively. Fifteen infants had been ventilated for hyaline membrane disease and nasal CPAP was applied immediately after ext

Late preterm infants are those infants born between 34 0/7 weeks through 36 6/7 week of gestation. These are physiologically less mature and have limited compensatory responses to the extrauterine environment compared with term infants. Despite their increased risk for morbidity and mortality, late preterm newborns are often cared in the well-baby nurseries of hospital after birth and are discharged from the hospital by 2-3 days of postnatal age. They are usually treated like developmentally mature term infants because many of them are of same birth weight and same size as term infants. There

PURPOSE: To determine the effects of surfactant replacement therapy (SRT), high-frequency oscillatory ventilation (HFOV), and general improvements in quality of care on the incidence of severe retinopathy of prematurity (ROP).
METHODS: Retrospective comparison of the incidence and severity of ROP in two groups of preterm infants admitted to our neonatal intensive care unit (NICU) in two consecutive 5-year periods (1986-1995) and screened for ROP. During the second study period, natural surfactant was introduced in the treatment of respiratory distress syndrome (RDS) and HFOV was used for trea

The care of infants has a somewhat sordid history, not without its side-show elements, political intrigues, and amazing success. Abandonment of infants and outright infanticide was not uncommon in ancient times. The Roman Church, however, was instrumental in beginning to rescue abandoned infants. Over time, greater recognition was afforded newborns and young children with the advent of dispensaries and hospitals created for the purpose of administering to this population. France was a focal point for the evolution of the modern care of newborns, with development of incubators at the end of the

Each mammal produces milk specific to its newborn that meets all nutritional needs. Breast milk is not only a secretory product but is also a complex liquid containing several components that provide enteral nutrition. The stage of lactation, the fullness of the breast, the feeding of the baby, and the health of the mother during the breastfeeding period cause differences in the composition of breast milk. Although the positive effects of breast milk on the physical and intellectual development of a child in the short and long term have been known for centuries, its mechanism has not been eluc

OBJECTIVE: In-hospital transmission of methicillin-susceptible Staphylococcus aureus (MSSA) among neonates remains enigmatic. We describe the epidemiology of MSSA colonization and infection in a 30-bed neonatal ward.
DESIGN: Multimodal outbreak investigation
SETTING: A public 800-bed tertiary care university hospital in Switzerland
METHODS: Investigations in 2012-2013, triggered by a MSSA infection cluster, included prospective MSSA infection surveillance, microbiologic screening of neonates and environment, onsite observations, and a prospective cohort study. MSSA isolates were characteriz

Healthcare-professionals who work in neonatal units believe that a very important part of their work is the care of sick newborns, and their families if the neonate has an incurable disease or will die. The effort is focused on preventing disproportionate and unnecessary treatments that result in pain and discomfort, and also separate the child from his family. These situations usually occur when the infant has a terminal illness, extreme immaturity with complications, or severe birth defects. In this paper, the Ethics Working Group of the Spanish Society of Neonatology reflects on decision ma

BACKGROUND: Bronchopulmonary sequestration is a lung malformation characterized by nonfunctioning lung tissue without primary communication with the tracheobronchial tree. Intrauterine complications such as mediastinal shift, pleural effusion or fetal hydrothorax can be present. We present the case of a newborn with bilateral intralobar pulmonary sequestration.
METHODS: Prenatal ultrasonography in a primigravida at 20 weeks of gestation revealed echogenic masses in the right fetal hemithorax with mediastinal shift towards the left side. Serial ultrasound confirmed persistence of the lesion wi

Singapore has a maternity hospital since 1924, but for many decades the newborns could only receive basic care. Neonatal and perinatal mortality rates were high. Marked improvement in neonatal care began from the 1980s when many neonatal departments were set up to provide intensive care. Improved socioeconomic status, better healthcare facilities, effective infection control, immunisation programmes and availability of potent antibiotics contributed to the decline of perinatal and neonatal mortality. Following the implementation of the glucose-6-phosphate dehydrogenase (G6PD) deficiency screen

Congenital malformations of the breast and nipple-areolar complex are described from the aspect of neonatal dysmorphology. Their classification is based on the accepted nomenclature in the medical literature and on a special method of clinical examination. The embryology and the possible genes involved in breast development are also discussed to clarify the mechanisms underlying the occurrence of breast malformations. Except for amastia, polymastia and polythelia, which have received much attention, the data for many other breast malformations are sparse. Case control studies or even case seri

Extracorporeal life support and extracorporeal membrane oxygenation characterize the use of mechanical devices for temporary support of heart and lung function. The mechanical circuit consists of a blood pump (heart), membrane oxygenator (lung: which accomplishes both carbon dioxide removal and oxygen delivery), heat exchanger and a servo-control module. Venous blood is drained from the right atrium through the right internal jugular vein, and returned oxygenated through either the right common carotid artery (venoarterial bypass), or into a large vein (venovenous bypass). All patients treated

Neonatal mortality remains unacceptably high throughout the world. Survival of sick infants in their first month of life has improved over the past six decades. However, many comorbidities persist, with lifelong implications for health. The current ecosystem for research and development of drugs and medical devices to treat neonatal disorders is hindering further improvements to neonatal outcomes, especially infants born preterm or needing critical care. Innovation is lagging, and this is a public health problem characterised by multifactorial challenges in leadership, collaboration, regulatio

The clinician-performed point-of-care ultrasound (POCUS) is a useful tool, and its scope includes bedside assessment of pulmonary (e.g., pneumothorax, pleural effusion), cardiac (e.g., pulmonary hypertension, ductus arteriosus), gastrointestinal (e.g., necrotising enterocolitis), and intracranial (e.g., intraventricular hemorrhage, cerebral blood flow velocities) pathologies, procedural guidance and rapid assessment of etiologies of acute clinical deterioration (e.g., pneumothorax, poor cardiac contractility, intraventricular hem-orrhage). Newborns have a reduced immunological resistance. That fact together with the risk factors, associated with hospitalization, determine the high level of nosocomial infections'illnesses in neonatological wards. The NI incidence of every 100 treated newborns at these wards in Bulgaria is 1.99% for the 2000-2009 period. At the neonatological wards in Bulgaria the nosocomial sepsis has a frequency of 1 of 1000 released patients. The leading role is that of coagulase-negative staphylococcus (CNS) with 28.27%. The leading etiological cause of nosocomial pneumonia in neonatal age in Bulgaria for the

OBJECTIVE: Neonatal nosocomial infection is a very important cause of morbidity and death in the hospital setting. The aim of our work is to study the incidence and risk factors associated with nosocomial infection in a neonatal unit.
METHOD: We followed 321 patients in a 6-month-long prospective, observational study. As units of measurement we used Incidence Density and Accumulative Incidence. For the statistical analysis we used chi-square and the logistic regression.
RESULTS: Nosocomial infection incidence density for the whole neonatal unit was 39.63% infections per 100 patients per mont

A patient was admitted to the hospital in the 31st week of gestation because of reduced sensation of fetal movements and a pathologic fetal heart rate tracing (Fig. 1). At first no underlying pathology could be found. After a week she developed a polyhydramnios. After repeated ultrasonographic examinations an atresia of the fetal small bowel was suspected (Fig. 2). The diagnosis could be confirmed after birth and on the same day the child was operated on. An atresia of the ileum was found. After liberal resection of the atretic part, recovery was complete. The malformation has an incidence of

Epidemiology, clinical features and laboratory results of a nosocomial outbreak of Echovirus 7 infections are described. After the admission of the index case with presumably perinatal infection to the neonatology unit, 3 additional babies from the total of 16 patients on the ward were infected. A 5th baby in another unit was also infected; this baby shared in part the nursing staff with the other group. Clinical symptoms of the infection were fever (5 children), gastrointestinal (5) and neurological symptoms (4), apnea and bradycardia (1), and a fine macular rash (1). Five of the 24 persons o

Extracellular vesicles (EVs) represent nanoparticles released by numerous cell types and exerting distinct biological roles. Extracellular vesicles are enclosed in a lipid layer and contain proteins, lipids, nucleic acids, carbohydrates or even mitochondria. The fundamental utility of EVs is intercellular communication, while their implication in various biological processes, such as inflammation and hemostasis, is well-documented. Growing evidence indicates the significant usefulness of EVs as biomarkers in certain prematurity-related diseases, like bronchopulmonary dysplasia (BPD) and necrot

An essential link in the cystic fibrosis (CF) newborn screening (NBS) process is communication of results. While this is described between NBS programs and primary care providers, data of this occurrence is limited with neonatologists. Neonatology providers represent a group caring for critically ill infants with conditions that can impact their ability to complete diagnostic testing after an abnormal NBS. Delays in testing can prolong time to diagnosis. We fielded a survey to assess neonatology provider knowledge and awareness of the Pennsylvania state CF NBS algorithm after an update occurre
