Intracholecystic tubular non-mucinous neoplasm (ICTN) of the gallbladder: a clinicopathologically distinct, invasion-resistant entity.

Preinvasive tumor-forming gallbladder neoplasms that are composed of small, non-mucinous tubules with complex architecture remain a poorly characterized group. Here, we evaluated the clinicopathological characteristics of this entity. Twenty-eight examples were analyzed. Tumors were invariably pedunculated polyps with thin stalks, often presented as loosely attached intraluminal nodules, with cauliflower architecture (akin to cholesterol polyps) comprised of compact, back-to-back acinar-like, small tubular units with minimal/no cytoplasm showing variable complexity, creating a picture distinct

Case of Invasive Carcinoma Derived from Intraductal Papillary Mucinous Neoplasm Negative for GNAS Mutation.

A 70-year-old woman with loss of appetite was referred to our hospital for further examinations. Computed tomography revealed a low density tumor in the body of the pancreas measuring 4 cm in diameter. The main pancreatic duct was dilated on both the head and caudal side of the tumor. Magnetic resonance imaging showed the mass as a low intensity area in T1-weighted and high intensity area in T2-weighted images. Endoscopic retrograde cholangiopancreatography findings indicated that the main pancreatic duct was continuous with the lumen of the tumor. A cytological examination of pancreatic juice

Risk stratification in locally invasive pancreatic serous cystic neoplasms: systematic review and two cases.

AIMS: Current diagnostic criteria for pancreatic serous cystadenocarcinoma (SCC) create a grey zone for tumours that are locally invasive but not yet metastatic. We sought to resolve this ambiguity by defining the clinicopathological features and searching for the malignant potential of this aggressive subset of serous neoplasms. METHODS: A combined cohort of 25 patients was compiled from a systematic literature review and augmented with two institutional cases. RESULTS: Infiltrative growth was a near-universal finding (23/25), often in tumours with a prominent solid architecture. This growt

Hemorrhagic brain abscess mimicking an intracranial neoplasm in invasive Klebsiella pneumoniae syndrome: illustrative case.

BACKGROUND: Brain abscess rarely presents with hemorrhagic transformation. Invasive Klebsiella pneumoniae syndrome (IKPS) is characterized by hematogenous dissemination from a primary hepatic focus, causing metastatic infections in various organs. The authors report a rare case of hemorrhagic brain abscess mimicking an intracranial neoplasm in the setting of IKPS managed with stereotactic drainage. OBSERVATIONS: A 56-year-old male with a history of intra-abdominal surgery presented with progressive right-sided hemiparesis. CT performed at the referring hospital revealed a ring-enhancing lesio

Interobserver agreement of pathologic classification and grading of tumoral intraductal pre-invasive neoplasms of the bile duct.

Current WHO terminology and recent publications have classified tumoral (grossly visible) intraductal pre-invasive neoplasms of bile duct (TIDN) into three categories: intraductal papillary neoplasm of bile duct (IPNB), intraductal papillary oncocytic neoplasm (IOPN), and intraductal tubulopapillary neoplasm (ITPN). A total of 227 cases of TIDN and related lesions ≥3 mm in height were examined by 10 biliary pathologists referring to these 3 categories and two pathologic gradings: two-tiered system (low- and high-grade dysplasia) and modified types 1 and 2 subclassification. Among them, IPNB wa

MRI-Based Pancreatic Atrophy Is Associated With Malignancy or Invasive Carcinoma in Intraductal Papillary Mucinous Neoplasm.

BACKGROUND: Abrupt change in the caliber of the main pancreatic duct (MPD) with distal pancreatic atrophy (PA) was considered as one of worrisome features in the International Association of Pancreatology guideline and American College of Gastroenterology guideline for the management of intraductal papillary mucinous neoplasms (IPMNs). However, this feature was not included in other guidelines. Moreover, the association between PA alone and malignancy in IPMNs has not been fully evaluated. In the present study, we investigated the role of image-based PA in identifying malignant IPMNs or invasi

Implementation of transanal minimally invasive surgery (TAMIS) for rectal neoplasms: results from a single centre.

BACKGROUND: Local excisions are important in a tailored approach to treatment of rectal neoplasms. In cases of low risk T1 local excision facilitates rectal-preserving treatment. Transanal minimally invasive surgery (TAMIS) is the most recent alternative developed for local excision. In this study we evaluate the results after implementing TAMIS as the routine procedure for local excision of rectal neoplasms. METHODS: All patients who underwent TAMIS from January 2016 to January 2020 at St. Olav's University Hospital were included, and clinical, pathological and oncological data were prospect

Endoscopic Ultrasound Elastography as a Novel Diagnostic Method for the Assessment of Hardness and Depth of Invasion in Colorectal Neoplasms.

INTRODUCTION: We aimed to compare the efficacy of endoscopic ultrasound elastography (EUS-EG) with that of magnifying chromoendoscopy (MCE) and endoscopic ultrasonography (EUS) for the diagnosis of the depth of invasion in colorectal neoplasms. This is an important clinical issue as the depth of invasion is associated with the risk of metastasis. METHODS: Consecutive patients with suspected superficial colorectal neoplasms, evaluated by MCE, EUS, and EUS-EG, for whom endoscopic submucosal dissection was considered, were enrolled in 2018 (derivation study) and in 2019-2020 (validation study).
