The association between perioperative opioids and breast cancer recurrence: a narrative review of the literature.

BACKGROUND AND OBJECTIVE: Opioid use disorder is an evolving crisis, and 17.2% of postsurgical patients continue to fill an opioid prescription one year after surgery. Preclinical studies suggest perioperative opioid use, defined here as opioids used in the setting of operative pain, may be linked to inferior oncologic outcomes. If this were true, opioid minimization strategies for surgical patients may reduce opioid-related deaths in more than one way. This review aims to describe the association between perioperative opioid use and breast cancer recurrence. METHODS: On November 1, 2021, we

The role of surgery for haematologic neoplasms of bone.

We report on 205 patients with haematologic neoplasms of bone treated from 1985 to 2009. There were 77 patients with primary bone lymphoma, 77 with myeloma and 51 with plasmacytoma. All patients had medical treatments; 43 patients had wide and 162 intralesional surgery. Mean follow-up was 5 years (median, 3.5 years); 11 patients were lost to followup. At the latest examination, 99 patients were alive without disease, 20 were alive with disease and 75 were dead of disease; 13 patients (6.7%) had local recurrence; 12 patients (24%) with plasmacytoma developed myeloma. Survival to death was signi

Safety and efficacy of transcervical resection of parapharyngeal space neoplasms.

There are several surgical approaches for resection of parapharyngeal space (PPS) neoplasms. The purpose of this study was to evaluate local disease control, facial nerve injury, and need for mandibulotomy associated with resection of PPS neoplasms via the transcervical approach with submandibular gland excision. A retrospective chart review of 33 patients who underwent resection of a PPS neoplasm between October 1991 and July 2000 was performed. Of the 33 patients, 3 patients developed local recurrence after a median follow-up of 24 months. None of the patients experienced facial nerve paresi

MCPyV-Negative Merkel Cell Carcinoma In Situ Associated With Immunosuppression.

Merkel cell carcinoma (MCC) is a rare and aggressive cutaneous neoplasm that is almost always intradermal. Immunosuppression increases the risk of MCC, which is believed to be due to increased susceptibility to Merkel cell polyomavirus (MCPyV). Intraepidermal MCC, or MCC in situ (MCCis), is extremely rare and usually associated with other cutaneous lesions. Here, we describe a case of MCPyV-negative MCCis arising in an immunocompromised patient. This case adds to only 9 previously reported cases of MCCis without a coexisting neoplasm and suggests that immunosuppression can lead to MCCis by mec

EWSR1-SMAD3-Positive Fibroblastic Tumor.

EWSR1-SMAD3-positive fibroblastic tumor is a recently characterized neoplasm with distinct clinicopathologic features and recurrent EWSR1-SMAD3 gene fusion. ESFT typically presents as a small, painless tumor in extremity subcutaneous tissues. Their behavior is benign, although they are prone to local recurrence. They typically comprise two components: intersecting fascicles of overlapping, uniform plump spindle cells, and less cellular hyalinized areas containing stippled calcifications. Immunohistochemically, the cells consistently show diffuse ERG nuclear expression, while other markers are

Primary cutaneous adenoid cystic carcinoma.

A patient had primary cutaneous adenoid cystic carcinoma. The tumor closely resembled adenoid cystic carcinoma of salivary gland origin and was composed of cribriform and tubular masses of basaloid cells. Lumina formed by neoplastic cells contained mucin and hyalin , and similar constituents were located between neoplastic cell masses and adjacent dermis. Perineural invasion was observed. A review of seven other primary cutaneous adenoid cystic carcinomas indicates that this neoplasm tends to affect middle-aged and older individuals, is often present for several years prior to diagnosis, has a

Perineural invasion as a predictor of local recurrence in cats with squamous cell carcinoma treated with electrochemotherapy.

INTRODUCTION: Squamous cell carcinoma (SCC) is a malignant neoplasm that accounts for approximately 15-25% and 70-80% of all feline cutaneous and oral tumors, respectively. Similar to that in humans, feline SCC can be highly invasive locally; however, its metastasis rate is low. Thus, effective local treatment may be curative for most patients, and includes surgery, electrochemotherapy (ECT), cryosurgery, or a combination of these. However, this neoplasia can manifest more aggressively in some patients, leading to higher recurrence rates. In humans, perineural invasion (PNI) has been described

Primary orbital apocrine adenocarcinoma: case report and literature review.

Primary apocrine adenocarcinoma (PAA) originating from the orbit is a rare malignant neoplasm. We present the case of a 61-year-old-male with PAA of the orbit. The patient underwent a right orbital exenteration, neck dissection, and adjuvant radiotherapy. Orbital exenteration is commonly performed as the primary intervention for PAA of the orbit. The role of adjuvant radiotherapy to prevent local recurrence is unclear and may be determined on a case-by-case basis.

Papillary cystic neoplasm of the pancreas.

BACKGROUND: Papillary cystic neoplasm of the pancreas is a rare disorder that occurs most commonly in young women. It has a low potential for malignancy, and the prognosis following resection is favourable. CASE OUTLINE: An 18-year-old white girl presented with a palpable mass in the right hypochondrium on physical examination associated with epigastric pain, nausea and vomiting, but no fever. Upper gastro-intestinal endoscopy revealed extrinsic compression of the posterior wall of the antrum and duodenal bulb with no mucosal lesion. Computed tomography (CT) scan and then laparotomy revealed

Single-fraction low-energy superficial x-ray radiation for conjunctiva Kaposi sarcoma: A case report.

Kaposi's sarcoma (KS) is a malignant vascular neoplasm associated with human herpesvirus type 8 (HHV-8) infection. Conjunctival involvement is very rare and is considered part of classic KS. We present the case of a 57-year-old female patient with skin lesions diagnosed as classic KS not associated with HIV or immunosuppression, who later developed a conjunctival lesion. This lesion was treated with superficial radiotherapy in a single fraction using a low-energy X-ray device, resulting in good clinical outcomes and no signs of conjunctival recurrence after 5 years of follow-up. Single-fractio

Myxofibrosarcoma. Clinicopathologic analysis of 75 cases with emphasis on the low-grade variant.

Myxofibrosarcoma is one of the most common sarcomas in the extremities of elderly patients. We analysed the clinicopathologic features in a series of 75 patients. All patients were adults (range, 22-91 years; median, 66 years) with an approximately equal incidence in men and women. Thirty-five tumors arose in the lower and 25 in the upper extremities, nine on the trunk, two each in the retroperitoneum and the head and neck region, and one each in the pelvis and penis. Forty-eight cases (69.5%) were located in dermal or subcutaneous tissues. Distinctive histologic features included the followin

Treatments for microcystic adnexal carcinoma--A review.

INTRODUCTION: Microcystic adnexal carcinoma (MAC) is a rare malignant cutaneous neoplasm presenting as a slow-growing, indurated nodule, papule or plaque. Clinically, the lesion can blend into the surrounding skin, obscuring borders and consequently delaying diagnosis histologically. Surgical and histologic techniques that emphasize examination of all margins may optimize management through early diagnosis and prevention of recurrences. OBJECTIVE: This review aims to assess the current surgical and histology techniques that result in lower rates of tumor recurrence and, consequently, better c

Ewing's sarcoma. Radiographic pattern of healing and bony complications in patients with long-term survival.

The radiographic appearance of Ewing's sarcoma was studied retrospectively in 22 patients who survived 5 years or longer after diagnosis and treatment. Expected changes from treatment, including regression of the extraosseous soft tissue mass, periostitis, and reconstitution of the cortex, occurred in all patients. Local recurrence occurred in one patient 10 years after complete remission whereas secondary osteosarcoma occurred more than 5 years after complete remission in two other cases. Both recurrent and secondary tumors presented as new lytic foci at the site of the original primary lesio

[Combined treatment of thymoma. State of the art and our experience].

Thymoma is the most common neoplasm of the anterior mediastinum and is frequently associated with paraneoplastic syndromes. Surgery is the first therapeutic option, but in advanced disease a multidisciplinary approach is feasible, because of chemosensitivity and radiosensitivity of the disease. The natural history of thymoma after surgery points to local recurrence. Adjuvant radiotherapy seems to improve local control and survival. Chemotherapy based on cisplatin plus anthracyclines could be performed in advanced and metastatic disease. The optimal sequence of chemotherapy, radiation therapy a

Solitary extramedullary plasmacytoma of the upper respiratory and digestive tracts.

The diagnosis, treatment and follow-up data for patients with malignant solitary extramedullary plasmacytoma of the upper respiratory and digestive tracts for the period from 1949 to 1975 are presented. The neoplasm developed in the nasal fossae in 3 patients; in the maxillary sinuses in 2; in the supraglottic region in 1; and in the tonsillar fossa in 1. Curative radiation therapy is recommended after the diagnosis is established by biopsy examination only. In the present series, this practice resulted in complete regression of the tumors in all of the patients, with restoration of functions

Recurrent Desmoid Tumor of the Neck: A Case Report of a Benign Disease with Aggressive Behavior.

We present a case of a desmoid tumor recurrence in a patient with a history of a resected desmoid tumor of the right neck area with free surgical margins six months earlier. The neoplasm was found to invade the parapharyngeal space, and wide excision was performed including most of the sternocleidomastoid muscle (SCM), the thrombosed internal jugular vein (IJV), and the infiltrated spinal accessory nerve (SAN). The histopathologic findings displayed free microscopic margins, with close margins at the site of the parapharyngeal space extension. After 3 months, there was no sign of tumor recurre

[Therapeutic options for aggressive fibromatosis].

INTRODUCTION: Aggressive fibromatosis, e.g., desmoid tumour, is a rare neoplasm of the connective tissue with local infiltrative growth. Because of the high recurrence rates and destruction of the surrounding tissue, these tumours are classified as semi-malignant. However, desmoid tumors tend not to metastasise. Arising from deep musculoaponeurotic structures, a monoclonal proliferation of fibroblasts occurs. Radical surgical treatment with tumour excision accompanied by radiotherapy is the current standard therapy that can be supplemented by pharmacological treatment in a few cases. PATIENTS
