Cervical cancer: Renal complications and survival after percutaneous nephrostomy.

INTRODUCTION: Obstructive nephropathy is a frequent complication in the course of advanced cervical cancer (CC), and ultrasonography-guided percutaneous nephrostomy (PCN) is a well established technique for fast ureteral desobstruction. OBJECTIVE: To identify possible factors related to the survival and quality of life of patients with advanced CC presenting acute urinary obstructive complications that after desobstruction by PCN recovered urinary flux and renal function. METHOD: This is an analytical, descriptive, cross-sectional study that included 45 patients with CC who underwent PCN and

Ultrasound-guided percutaneous nephrostomy in non-dilated pelvicaliceal system.

Percutaneous placement of nephrostomy catheters in patients with non-dilated pelvicaliceal systems remains problematic. We successfully performed ultrasound-guided percutaneous nephrostomies in 5 such patients after intravenous administration of a diuretic agent. Diuresis resulted in transient dilatation of the calyces, facilitating the procedures, which were performed under real-time sonographic guidance using the Seldinger technique. No complications were encountered.

Renal artery laceration secondary to percutaneous nephrostomy catheter placement.

Renal artery hemorrhage secondary to placement of percutaneous nephrostomy catheters can lead to major operations and even nephrectomy. We recently treated a woman who suffered a renal artery laceration during percutaneous nephrostomy catheter placement using interventional radiographic techniques. The perforation site was identified using angiography and treated using selective embolization. The patient was able to avoid a surgical procedure with its inherent risks.

A new percutaneous nephrostomy technique in the treatment of obstructive uropathy.

PURPOSE: To describe the percutaneous nephrostomy technique used at our centre in the treatment of supravesical urinary tract obstruction and to analyse the results obtained. MATERIALS AND METHODS: Three-hundred and seventy-three patients underwent 412 percutaneous nephrostomies, most of which (78%) in an emergency setting, performed with a newly devised "mixed" technique. The procedure combines the positive elements of the two techniques employed to date in the management of upper urinary tract obstruction: the Seldinger angiographic technique and the Goodwin technique. Once the site for the

The role of percutaneous nephrostomy in malignant urinary tract obstruction.

Percutaneous nephrostomy is a well established technique for rapid relief of ureteric obstruction and improvement of renal function. However, its role in the management of renal failure resulting from advanced abdominopelvic malignancy is controversial and there are no clear guidelines to predict which patients benefit from such intervention both in terms of survival time and quality of life. To establish a protocol for selection of patients with abdominopelvic malignancy most likely to benefit from nephrostomy for renal obstruction, the medical records of 50 consecutive patients undergoing th

Percutaneous nephrostomy for ureteral obstruction in patients with gynecologic malignancy.

Reversible ureteral obstruction is not an infrequent complication of gynecologic malignancy or its treatment. In the past, open nephrostomy has been the standard method of management in such patients. This paper discusses the nonoperative technique of percutaneous nephrostomy which allows placement of either an external nephrostomy tube or an internal ureteral stent under local anesthesia. Six illustrative cases are presented and the indications and technical aspects of the procedure are discussed.

Percutaneous nephrostomy for assessment of functional recovery of obstructed kidneys.

The use of percutaneous nephrostomy to evaluate the recoverability of function of the obstructed kidney is described and examined as a practical clinical method. Long-term drainage was used to assess the functional status of four kidneys and found to be of great value. The use of percutaneous nephrostomy to evaluate function has been mentioned previously but has not become established. In view of inaccuracy of other methods of assessment, we consider percutaneous nephrostomy to be the most accurate predictive test available today and suggest criteria for its use.

Percutaneous nephrostomy in patients with advanced or recurrent cervical cancer.

Thirteen percutaneous nephrostomies were performed in 10 patients with advanced (no = 4) (group A) or recurrent (no = 6) (group B) cervical cancer. Urinary diversion was unilateral in 7 patients and bilateral in 3 patients. The catheter was kept in place for 4 months in mean (range: 1-7 months). A normalization of renal function was achieved in 4 out of 4 patients of group A, and in 3 out of 6 patients of group B. Of the 4 patients of group A, who afterwards underwent antineoplastic integrated treatments, 2 patients are currently alive with no evidence of disease after 48 and 20 months from th

[Percutaneous drainage nephrostomy in patients over 70 years of age. Apropos of 98 nephrostomies in 74 patients].

From 1985 to 1992, 98 ultrasound-guided percutaneous drainage nephrostomies were performed in 74 patients with a mean age of 77 years (range: 70-88 years). The diversion was indicated because of upper urinary tract obstruction (87% of cases), urinary fistula (4%) or secondary displacement of the first PCN (9%). The initial disease was benign in 29 patients (42.5%, including 48% of renal and ureteric stones), malignant in 39 cases (53%, including 79% of pelvic cancers) and not specified in 6 cases (4.5%). PCN was performed successfully in 93% of patients and allowed improvement in renal failure

Cystoscope assisted trans-conduit removal of fragmented percutaneous nephrostomy catheter.

Percutaneous nephrostomy catheter fragmentation is an uncommon complication that is managed through different approaches. In this report we describe an iatrogenicly fragmented nephrostomy catheter in a patient with an ileal conduit, that was removed by using combined cystoscope and fluoroscopy guidance through a retrograde trans conduit approach.

Anterograde irrigation - assisted ureteroscopic lithotripsy in patients with percutaneous nephrostomy.

In complicated urinary tract infection with ureteral calculi, urinary diversion is inevitable. So, stenting or percutaneous drainage can be an option. In hemodynamically unstable patients, percutaneous drainage is superior to ureteral stenting (1). Once acute infection is controlled, definite treatment of the stone is necessary. According to a guideline, semirigid ureteroscopy is recommended for lower and mid - ureter stone and flexible ureteroscopy for upper ureter stone (2). Semi - rigid ureteroscopy can migrate stone to kidney, especially in upper ureter stone, lowering stone free rate (3).

Easy Method for Percutaneous Nephrostomy Placement with Transrectal Ultrasonography Probe.

Aim: Percutaneous nephrostomy (PN), the indispensable interventional procedure in situations such as obstructive uropathy and pyonephrosis, requires serious experience. As a result, many urologists and radiologists avoid this procedure. However, this can leave both doctor and patient in a difficult situation when it is necessary. In this study, we aimed at presenting a nephrostomy insertion technique by using a transrectal ultrasonography probe that we use in our clinic and we believe that this has made the PN procedure easier, along with our results. Patients and Methods: Data from patients w

[Lower massive hematuria deferred by arteriovenous fistula following percutaneous nephrostomy].

OBJECTIVES: To report one case of severe lower hematuria 48 hours after urinary diversion by percutaneous nephrostomy for obstructive uropathy. METHODS: The clinical picture, its severity, and previous procedure probably causing it made the study by arteriography essential to identify the existence of an arteriovenous fistula after stabilization of the acute anaemia; it was confirmed and selective embolization was undertaken with an optimal result. We performed a bibliographic search on Pub-Med (MEDLINE) using the terms "percutaneous nephrostomy", "complications", and "arteriovenous fistula".

Nondilated obstructive uropathy: percutaneous nephrostomy performed to reverse renal failure.

Ureteral obstruction can lead to renal failure without involving detectable dilatation of the calyces, renal pelvis, or ureter proximal to the obstruction. This phenomenon was noted in seven patients who had clinical obstruction that we were not able to diagnose using computed tomography (CT) or ultrasonography (US). These patients underwent percutaneous nephrostomy (PCN), which resulted in brisk diuresis and improved renal function. We obtained an accompanying antegrade urogram in these cases, which demonstrated the level of obstruction and indicated that dilatation of the collecting system w

Life-threatening arteriovenous fistula after percutaneous nephrostomy: A case report.

INTRODUCTION: Arteriovenous fistula (AVF) is a rare but serious complication following percutaneous nephrostomy. While more commonly observed after nephrolithotomy, it can also occur with prolonged nephrostomy placement or complicated procedures. Early diagnosis and management are critical for preventing life-threatening complications. CASE PRESENTATION: A 59-year-old female with recurrent bilateral urolithiasis, right heart failure, poorly controlled type 2 diabetes, and chronic kidney disease presented with right flank pain, fever, and fatigue. She had previously undergone right double-J st

A case report of sepsis associated coagulopathy after percutaneous nephrostomy.

BACKGROUND: Hemorrhage is a common complication of nephrostomy and percutaneous nephrolithotripsy, and it is caused by surgical factors. Here we report a rare case of hemorrhage caused by sepsis-related coagulation dysfunction. CASE PRESENTATION: A 72-years-old male patient with bilateral ureteral calculi accompanied by hydronephrosis and renal insufficiency developed sepsis and hemorrhage on the third day after bilateral nephrostomy. After vascular injury was excluded by DSA, the hemorrhage was considered to be sepsis-associated coagulopathy(SAC/SIC), finally the patient recovered well after

Severe Ketamine-Induced Uropathy Associated With Cholangiopathy Requiring Percutaneous Nephrostomy.

Ketamine-induced uropathy (KIU) is becoming more prevalent as ketamine abuse becomes widespread. Recently, a plethora of reports have highlighted the association between recreational ketamine abuse and its deleterious effects including uropathy and cholangiopathy. However, there are hardly any reports that demonstrate the management of severe KIU in young people. We report a case of an Asian female in her late 20's with a 10-year history of ketamine abuse who presented to the emergency department with sharp, epigastric pain for two weeks. An evaluation revealed acute kidney injury and transami
