V to VII Nerve Transfer for Smile Reanimation.

Using the wording "facial reanimation," surgeons mean restoring movements to the paralyzed face. According to the condition of mimic muscle, facial palsy can be classified as recent (mimic muscle still alive) and chronic (atrophy of mimic muscle) palsy. The treatment is quite different because in the former group the mimic muscles can be still used so long as a new motor source would be connected to the damaged facial nerve. In the latter group, muscular transplantation is needed to substitute the atrophied mimic muscles of the middle part of the face. In both cases, the neural impulse that ma

Experimental nerve transfer model in the neonatal rat.

Clinically, peripheral nerve reconstructions in neonates are most frequently applied in brachial plexus birth injuries. Most surgical concepts, however, have investigated nerve reconstructions in adult animal models. The immature neuromuscular system reacts differently to the effects of nerve lesion and surgery and is poorly investigated due to the lack of reliable experimental models. Here, we describe an experimental forelimb model in the neonatal rat, to study these effects on both the peripheral and central nervous systems. Within 24 hours after birth, three groups were prepared: In the ne

Nerve Transfer Surgery for Penetrating Upper Extremity Injuries.

CONTEXT: Nerve transfer surgery is an option for repair of penetrating injuries of the upper extremity. In the right setting, it has advantages over tendon transfers and nerve grafting. OBJECTIVE: To review our experience since 2006 of nerve transfer surgery in the upper extremities. DESIGN: We included cases performed to repair penetrating trauma within three months of injury with at least three years' follow-up. MAIN OUTCOME MEASURES: Preoperative and postoperative muscle strength of the affected extremity. RESULTS: All 16 patients were males aged 16 to 43 years. Six patients underwent n

Evaluating motor unit properties after nerve transfer surgery.

Nerve transfer surgery (NTS) shows promise in restoring movement to muscles paralyzed by spinal cord (SCI) and peripheral nerve injury (PNI). Yet, motor outcomes vary, and the neurophysiological factors influencing responders and non-responders remain unclear. As the fundamental goal of NTS is to reinnervate paralyzed muscles by creating new motor units (MUs), we examined MU properties after NTS for individuals with SCI and PNI. Nine participants (3 SCI, 6 PNI, 50.3 ± 13.9 years) >18 months post-NTS were evaluated and compared to either age-matched controls (SCI) or the contralateral limb (PNI

Hypoglossal nerve transfer in obstetric brachial plexus palsy.

A cost-benefit analysis was performed of hypoglossal nerve transfer in six patients with obstetric brachial palsy taking into account the factors donor site morbidity and extent of recovery. Hypoglossal nerve transfer was employed in four children for elbow flexion only; in two patients for elbow flexion as well as for elbow extension. The transfer was part of an extended brachial plexus reconstruction for treatment of obstetric brachial plexus palsy. After a mean post-operative interval of 52 months (SD+/-8.1), two professional speech therapists investigated late donor site morbidity by analy

Teflon versus thyroplasty versus nerve transfer: a comparison.

Surgical rehabilitation of the paralyzed larynx is currently performed by Teflon injection, thyroplasty, and reinnervation techniques. Proponents of the two newer techniques maintain that they are preferred to Teflon injection because superior phonatory quality is achievable. This paper was written in an attempt to dissect the issues regarding this question. Teflon remains the quickest and least expensive procedure, but further experience with stroboscopic and other voice analyses reveals that the other procedures demonstrate some superiority in phonatory quality over Teflon. Phrenic nerve transfer for brachial plexus motor neurotization.

We report a series of 164 patients who underwent phrenic neurotization to elements of the brachial plexus with root avulsion injuries. Recipient nerves included musculocutaneous nerve in 125 patients (78 direct neurotizations and 48 with intervening autograft), median nerve in 10 patients, and a variety of other nerves in 28 patients. Sixty-five patients presented a follow-up period of 2 or more years. Of this group, 55 patients (84.6%) achieved a recovery of M-3 or better. We observed no long-term deleterious effects on respiratory function.

Restoration of shoulder abduction by nerve transfer in avulsed brachial plexus injury: evaluation of 99 patients with various nerve transfers.

During the 5-year period between 1987 and 1991, 99 patients with total root or upper root (C5, C6, or C7) injuries were treated by nerve transfer to obtain shoulder abduction. More than eight different combinations of coaptation between donor nerves (intercostal nerves, phrenic nerve, spinal accessory nerve, ipsilateral C7, or cervical motor nerves) and recipient nerves (suprascapular nerve, axillary nerve, and upper trunk or C5) were applied. All patients had at least 2 years of follow-up. Different results were obtained in each category. Simultaneous neurotization of the suprascapular and ax

Distal nerve transfers for ulnar nerve reinnervation and hand function restoration.

INTRODUCTION: Ulnar nerve injuries lead to severe intrinsic hand muscle dysfunction and major impairment of grip, pinch, and quality of life. Distal nerve transfers have been introduced to overcome the limitations of conventional repair and grafting by shortening the regeneration distance and improving reinnervation of intrinsic muscles. RESEARCH QUESTION: Can distal anterior interosseous nerve to deep motor branch of the ulnar nerve (AIN-to-UN-DMB) transfer restore intrinsic hand function and improve patient-reported quality of life in proximal ulnar nerve injuries? Distal nerve transfers for median nerve reinnervation and hand function restoration.

INTRODUCTION: Proximal and extensive median nerve median nerve injuries cause profound disability, impairing daily activities and quality of life. Outcomes after autologous nerve grafting are often suboptimal, due to long regeneration distances and complex branching. Distal nerve transfers offer a more direct reconstructive approach. RESEARCH QUESTION: May combined distal nerve transfers (ECRB-AIN, SUP-FDS, ADM-TE, and DDN-PDN) restore hand function and quality of life in patients with proximal or extensive median nerve injuries? MATERIAL AND METHODS: A retrospective series of 13 patients (9

The Anterior Approach for Transfer of Radial Nerve Triceps Fascicles to the Axillary Nerve.

Nerve transfers are an increasingly popular method for surgical treatment of nerve injuries. One of the most popular of these is the transfer of radial nerve triceps fascicles to the axillary nerve. The most common approach for this transfer is the posterior approach, which gives excellent access to both nerves but is not easy to combine with other nerve transfers. We describe here an alternative, the anterior approach, that offers safe access to both radial and axillary nerves and has the added advantage of compatibility with approaches for other common nerve transfers.

Transfer of a Radial Nerve Branch to the Brachialis Nerve for Restoration of Elbow Flexion.

Nerve transfers for brachial plexus reconstruction and the treatment of peripheral nerve injury have demonstrated excellent clinical outcomes and may be superior to nerve grafting. Previously described nerve transfers for restoration of elbow flexion include the Oberlin (ulnar to musculocutaneous) and double fasicular (median to biceps and ulnar to brachialis) transfers. However, these transfers cannot be performed in patients with loss of elbow flexion and concomitant high median and ulnar nerve injury. Other transfers utilizing the thoracodorsal or intercostal nerves have been described; how

Transfer of pectoral nerves to suprascapular and axillary nerves: an anatomic feasibility study.

PURPOSE: We conducted an anatomic study to provide detailed information on the pectoral nerves and anatomic data on the transfer of the pectoral nerves to the axillary nerve. Moreover, we experimentally determined the feasibility of transferring the pectoral nerves to the suprascapular nerve in upper brachial plexus injury. METHODS: We dissected 26 brachial plexus from 15 fresh cadavers. The origin, location, course, and branching of the pectoral nerves were recorded. The length and the diameter of the pectoral nerves were measured. The diameter of the suprascapular and axillary nerves was re

Intercostal Nerve Transfers to the Musculocutaneous-A Reliable Nerve Transfer for Restoration of Elbow Flexion in Birth-Related Brachial Plexus Injuries.

Introduction  There is consensus on the need for early microsurgical reconstruction in birth palsies involving three or more roots, that is, extensive partial palsies and total palsies. The fundamental principles of these operations are complete exploration and judicious use of the ruptured stumps by nerve grafting to suitable distal targets. The frequent observation of root avulsions in such cases makes it imperative to look for extraplexual nerve donors for some functions. Intercostal nerves are readily available in such patients. Materials and Methods  This is a study of 50 patients of exte

Distal nerve transfers for radial nerve reinnervation and hand function restoration.

INTRODUCTION: Radial nerve palsy, with wrist drop and loss of finger and thumb extension, severely impairs grasp and release. In proximal or extensive defects, direct repair or grafting may fail, making distal nerve transfer a preferred alternative. RESEARCH QUESTION: Can distal median-to-radial nerve transfers restore hand extension and improve quality of life in patients with radial nerve injury? MATERIAL AND METHODS: This prospective study included nine patients with radial nerve injuries treated using distal nerve transfer. Surgical reconstruction consisted of transfer of the median nerv

Nerve transfers for femoral nerve palsy: an updated approach and surgical technique.

OBJECTIVE: Femoral nerve palsy results in significant impairment of lower extremity function due to the loss of quadriceps muscle function. The authors have previously described their techniques utilizing the anterior obturator and sartorius nerves for transfer in cases of femoral nerve palsy presenting within 1 year of injury. In the current study, the authors discuss their updated techniques, results, and approach to partial and complete femoral nerve palsies using femoral nerve decompression and nerve transfers. METHODS: They conducted a retrospective review of patients with femoral nerve

Transfer of obturator nerve for femoral nerve injury: an experiment study in rats.

BACKGROUND: Quadriceps palsy is mainly caused by proximal lesions in the femoral nerve. The obturator nerve has been previously used to repair the femoral nerve, although only a few reports have described the procedure, and the outcomes have varied. In the present study, we aimed to confirm the feasibility and effectiveness of this treatment in a rodent model using the randomized control method. METHODS: Sixty Sprague-Dawley rats were randomized into two groups: the experimental group, wherein rats underwent femoral neurectomy and obturator nerve transfer to the femoral nerve motor branch; an

TRANSFER OF NERVE BRANCHES OF THE FLEXOR CARPI RADIALIS TO THE POSTERIOR INTEROSSEOUS NERVE.

OBJECTIVE: The objective of this paper was to study the anatomical variations of the flexor carpi radialis muscle (FCR) and determine in cadaver limbs whether the FCR nervous branch can be connected to the posterior interosseous nerve (PIN) without tension and how close to the target muscles the transfer can be performed. METHOD: Thirty cadaveric upper limbs were dissected. RESULTS: The FCR received exclusive innervation of the median nerve, distally to the intercondylar line of the humerus.
