Effects of 1% and 2% enflurane on intracranial pressure in man.

We have studied the effects of enflurane on intracranial pressure (ICP) in 9 neurosurgical patients, whose ICP was continuously monitored for therapeutic or diagnostic purposes. The control ICP was under 15 Torr in 5 cases, under 20 Torr in 3 cases and of 25 Torr in one case. In order to achieve stable ventilatory conditions, the patients were kept under controlled ventilation, breathing a mixture of 60 to 70% nitrous oxide and oxygen. The PaCO2 was kept constant, at the pre-study level. Mean arterial pressure (MAP) was maintained stable with the administration of fluids and/or a phenylephrine

[General anesthesia with remifentanil in two cases of emergency cesarean section].

Remifentanil is a synthetic opiate with evident advantages for various anesthetic techniques, enhancing quality of anesthesia. Indications are increasingly well-defined. Remifentanil may be used in obstetric analgesia-anesthesia thanks to advantages demonstrated in patients with heart disease (cardiac and non-cardiac anesthesia) and in those requiring neuroanesthesia. Remifentanil is known to cross the placenta rapidly and to be rapidly metabolized and redistributed to both mother and fetus. Based on this, and on pharmacokinetic and pharmacodynamic studies in children, we judged remifentanil t

Frequency of hyperlactatemia in craniotomy and its association with anesthesia: A prospective observational study.

Lactate plays a crucial role in neuroanesthesia, and dysregulated lactate metabolism is hypothesized to contribute to carcinogenesis. Serum lactate has been suggested as a potential biomarker for tumor malignancy and a prognostic factor in patients with brain tumors. However, the clinical significance of intraoperative hyperlactatemia in brain tumor surgery and its relationship with anesthesia technique remain unclear. The aim of this study was to investigate the incidence of hyperlactatemia in brain tumor surgeries and its association with the type of maintenance anesthesia, as well as to exa

Transcranial Motor Evoked Potential Monitoring Using Propofol-Fentanyl Versus Desflurane-Dexmedetomidine Anesthesia During Spinal Cord Tumor Resection: A Randomized Controlled Trial.

BACKGROUND: Patients undergoing resection of spinal cord tumours require intraoperative neuromonitoring. Transcranial electrical stimulation is used to record myogenic responses during surgery. This study aimed to compare the effect of 2 anaesthetic regimens, propofol/fentanyl versus desflurane/dexmedetomidine, on the ability to record MEPs with an amplitude of 50 µV or greater. Our secondary outcome compared intraoperative haemodynamics, recovery profile, and postoperative analgesia between the groups. METHODS: We conducted a prospective, double-blinded, open-label, single-centre, randomized

Comparison of Sevoflurane and Desflurane with Dexmedetomidine for Recovery in Neurosurgical Patients Undergoing Supratentorial Tumor Surgery.

BACKGROUND: Anesthetic management in neurosurgical procedures requires rapid recovery and stable hemodynamics to enable early neurological evaluation. Sevoflurane and desflurane, with low blood-gas solubility, are commonly used inhalational agents. Dexmedetomidine, an α2-adrenergic agonist, provides sedation and hemodynamic stability. Limited studies have compared sevoflurane and desflurane in combination with dexmedetomidine for neurosurgical recovery. AIMS AND OBJECTIVES: To compare the recovery profile, cognitive function, and intraoperative brain relaxation in patients undergoing supraten

Adenosine-induced Flow Arrest to Facilitate Control of a Basilar Artery Injury during Craniotomy for Petroclival Meningioma: A Case Report.

Intraoperative hemorrhage is a life-threatening complication during neurosurgery, especially in posterior fossa surgery, where critical vasculature and brain structures are present. Adenosine has been used in neurovascular surgery, particularly in the management of intraoperative aneurysm rupture and hemorrhage for its ability to produce transient flow arrest. This case report describes a novel application for adenosine, in which adenosine-induced flow arrest was used to facilitate control of a vascular injury sustained during meningioma surgery. A 46-year-old female diagnosed with a petrocliv

Anesthesia for carotid endarterectomy: a survey.

Indications for carotid endarterectomy (CEA) have been expanded recently, and a consensus statement has been made regarding these changes. However, the debate regarding the "ideal" anesthetic for CEA remains on-going. This study was designed to evaluate the actual anesthetic techniques used by anesthesiologists for CEA. A total of 426 1-page questionnaires were mailed to all current (1995) members of the Society of Neurosurgical Anesthesia and Critical Care (SNACC). Of these, 216 (50.7%) were completed and returned. The majority of these respondents (84.7%) administered general anesthesia (GA)

Hemispherotomy Revised: A complication overview and a systematic review meta-analysis.

INTRODUCTION: Hemispherectomy/hemispherotomy has been employed in the management of catastrophic epilepsy. However, initial reports on the associated mortality and morbidity raised several concerns regarding the technique's safety. Their actual, current incidence needs to be systematically examined to redefine hemispherotomy's exact role. RESEARCH QUESTION: Our current study examined their incidence and evaluated the association of the various hemispherotomy surgical techniques with the reported complications. MATERIAL & METHODS: A PRISMA-compliant systematic review and meta-analysis was per

Postoperative Stridor and Acute Respiratory Failure After Parkinson Disease Deep Brain Stimulator Placement: Case Report and Review of Literature.

BACKGROUND: Parkinson disease (PD), a neurodegenerative disorder characterized by loss of dopaminergic neurons in the substantia nigra of the midbrain, is commonly thought of as a motion disorder, but it can have significant effect on the respiratory system. Respiratory failure is the most common cause of death in these patients, but it can also affect laryngeal function causing dysphonia, dysphagia, and dysarthric speech. Acute upper airway obstruction is a rare finding in PD, especially in the perioperative settings. In this article we report a PD patient who developed upper respiratory obst

Desflurane increases intracranial pressure more and sevoflurane less than isoflurane in pigs subjected to intracranial hypertension.

Desflurane and sevoflurane may have advantages over isoflurane in neuroanesthesia, but this is still under debate. A porcine model with experimental intracranial hypertension was used for paired comparison of desflurane, sevoflurane, and isoflurane with respect to the effects on cerebral blood flow (CBF), cerebrovascular resistance (CVR), and intracranial pressure (ICP). The agents, given in sequence to each of six pigs, were compared at 0.5 and 1.0 minimal alveolar concentrations (MAC) and three mean arterial blood pressure (MAP) levels (50, 70, and 90 mm Hg) at normocapnia and one MAP level

Perioperative management of hemorrhagic stroke in morbidly obese patients: A narrative review.

BACKGROUND: Intracerebral hemorrhage (ICH) constitutes approximately 10-15% of all strokes and is a major contributor to long-term neurological morbidity and mortality. Morbid obesity (body mass index ≥40 kg/m2) is a well-recognized risk factor for hypertension and hemorrhagic stroke. Paradoxically, some studies report more favorable outcomes among obese patients with stroke - a phenomenon known as the obesity paradox. Nevertheless, specific perioperative anesthetic guidelines for this high-risk population remain inadequately addressed in current literature. METHODS: This narrative review cri

Effects of cisatracurium on cerebral and cardiovascular hemodynamics in patients with severe brain injury.

BACKGROUND: For neuroanesthesia and neurocritical care the use of drugs that do not increase or preferentially decrease intracranial pressure (ICP) or change cerebral perfusion pressure (CPP) and cerebral blood flow (CBF) are preferred. The current study investigates the effects of a single rapid bolus dose of cisatracurium on cerebral blood flow velocity, ICP, CPP, mean arterial pressure (MAP) and heart rate (HR) in 24 mechanically ventilated patients with intracranial hypertension after severe brain trauma (Glasgow coma scale <6) under continuous sedation with sufentanil and midazolam. Post-craniotomy emergence hypertension: A scoping review.

INTRODUCTION: Post-craniotomy emergence hypertension (PCEH) is a phenomenon of transient increase in blood pressure during and after emergence from anesthesia for a craniotomy. It is frequently encountered in neurosurgical and neuroanesthesia practice. Clinical definitions and reported prevalence of PCEH vary significantly, so this review aims to characterize existing literature on the diagnosis and management of this clinical entity. METHODS: A scoping review of the literature was conducted using six electronic databases: PubMed, Embase, CINAHL, Web of Science, Cochrane, and Scopus. Title an

Defining a Taxonomy of Intracranial Hypertension: Is ICP More Than Just a Number?

Intracranial pressure (ICP) monitoring and control is a cornerstone of neuroanesthesia and neurocritical care. However, because elevated ICP can be due to multiple pathophysiological processes, its interpretation is not straightforward. We propose a formal taxonomy of intracranial hypertension, which defines ICP elevations into 3 major pathophysiological subsets: increased cerebral blood volume, masses and edema, and hydrocephalus. (1) Increased cerebral blood volume increases ICP and arises secondary to arterial or venous hypervolemia. Arterial hypervolemia is produced by autoregulated or dys

Inhalational versus total intravenous anesthesia in noncardiac surgery: a comparative review of clinical outcomes.

Inhalational anesthetics have long been the cornerstone of general anesthesia in noncardiac surgery owing to their reliable pharmacokinetics, ease of administration, and cardiopulmonary benefits such as bronchodilation and myocardial preconditioning. Total intravenous anesthesia (TIVA), achieved using short-acting agents such as propofol and remifentanil, and supported by target-controlled infusion systems and depth-of-anesthesia monitors, has emerged as a widely adopted alternative. TIVA is associated with improved recovery profiles, reduced incidence of postoperative nausea and vomiting, and

Is the medicolegal issue avoidable in neurosurgery? A retrospective survey of a series of 115 medicolegal cases from public hospitals.

OBJECTIVE: Since the mid-1950s, neurosurgery has benefited from the remarkable progress due to tremendous advances in neuroimaging techniques, neuroanesthesia, neurostimulation, and brain-computer interfaces, as well as breakthroughs in operating microscopes and surgical instruments. Yet, this specialty has to do with delicate human structures and is hence considered as highly risky by insurance companies. In France, although neurosurgery's casualty rate (6%) is lower than in other specialties, the number of legal prosecutions has increased since 2002 because of easier access to medicolegal pr
