Fifty-one extremely preterm infants were studied to ascertain whether there is an association between sonographic abnormalities and neurological examination at term, controlling for factors such as low birthweight and chronic lung-disease. Their mean birthweight was 956g and mean gestational age was 27.9 weeks. Sonography was performed at least once during the first week, twice within the first month, and once within a month of term-corrected age. Neurological assessment was used to classify the infants as normal, suspect or abnormal at term. Infants were divided into three groups, based on so

Understanding the efficacy of digital biomarkers in vision-based human motion analysis is essential, not only for interpreting the computer-aided exam results but also for advancing the next generation of digital health tool solutions. This study extensively analyzes digitized neurological examination (DNE) biomarkers for detecting and documenting exam features of Parkinson's disease (PD) and other neurological disorders (OD). Collected over 113 participants, DNE-113, a multi-test DNE database of finger tapping, finger to finger, forearm roll, stand-up and walk, and facial activation tests, co

AIM: Little is known of minor neurological dysfunction (MND) in infancy. This study aimed to evaluate the inter-assessor reliability of the assessment of MND with the Touwen Infant Neurological Examination (TINE) and the construct and predictive validity of MND in infancy.
METHOD: Inter-assessor agreement was determined in a sample of 40 infants (24 males, 16 females) aged 3 to 12 months (25 born at term: gestational age 37-41wks, median 39; and 15 born preterm, gestational age 24-35wks, median 32). Thirty typically developing term infants (18 males, 12 females; gestational age 37-42wks, medi

INTRODUCTION: The aim of the study was to compare the utility of instrument-based assessment of peripheral nerve function with the neurologic examination in ulnar neuropathy at the elbow (UNE).
METHODS: We prospectively recruited consecutive patients with suspected UNE, performed a neurologic examination, and performed instrument-based measurements (muscle cross-sectional area by ultrasonography, muscle strength by dynamometry, and sensation using monofilaments).
RESULTS: We found good correlations between clinical estimates and corresponding instrument-based measurements, with similar abili

BACKGROUND AND PURPOSE: Accurate long-term outcome prognostication in basilar artery occlusion strokes may guide clinical management in the subacute stage. We determine the prognostic value of the follow-up neurological examination using the National Institutes of Health stroke scale (NIHSS) and identify 24- to 48-hour NIHSS risk categories in basilar artery occlusion patients.
METHODS: Participants of an observational registry of radiologically confirmed acute basilar artery occlusion (BASICS [Basilar Artery International Cooperation Study]) with prospectively collected 24- to 48-hour NIHSS

This study examined whether a neurologic examination at 6 months of age is predictive of neurodevelopmental outcome at 12 months in very-low-birth-weight (VLBW) infants. A neurologic examination and the Bayley Scales of Infant Development were performed at 6 and 12 months with VLBW infants and full-term (FT) controls. VLBW infants were categorized based on early medical complications. High-risk (HR) infants had diagnoses of bronchopulmonary dysplasia, pulmonary immaturity, grade III or IV intraventricular hemorrhage, and/or periventricular leukomalacia. VLBW infants with other diagnoses were p

OBJECTIVE: To describe the findings of short Hammersmith Neonatal Neurologic examination (HNNE) in preterm small for gestational age (SGA) and appropriate for gestational age (AGA) infants at term equivalent age (TEA) and to correlate it with the global score of Hammersmith Infant Neurologic Examination (HINE) performed at 4-6 months of corrected age.
METHODS: This prospective cohort study was conducted at the high risk follow-up clinic of our center. 52 preterm infants born <35 weeks gestation were examined using HNNE at TEA and followed- up till 4-6 months of corrected age to estimate HINE.

OBJECTIVE: To determine the sensitivity and specificity of clinical tests for detecting focal lesions in a prospective blinded study.
METHODS: 46 patients with a focal cerebral hemisphere lesion without obvious focal signs and 19 controls with normal imaging were examined using a battery of clinical tests. Examiners were blinded to the diagnosis. The sensitivity, specificity, and positive and negative predictive values of each test were measured.
RESULTS: The upper limb tests with the greatest sensitivities for detecting a focal lesion were finger rolling (sensitivity 0.33 (95% confidence in

This article will review age-related changes in the neurological examination. The involutional process is a diffuse one, affecting all levels of the neuraxis simultaneously. Most of the numerous little manifestations distinguishing them so sharply from the youthful are really features of the neurology of the elderly. Some signs are characteristic: attitude of general flexion, general poverty of movement, gait, minor extrapyramidal signs, primitive reflexes. However, these features may also be caused by neurological diseases common in the elderly, such as dementia or cervical myelopathy, which

BACKGROUND: Cerebral palsy can typically be diagnosed from the age of two years. Early detection facilitates timely intervention, which can significantly benefit both infants and their families. The Hammersmith Infant Neurological Examination (HINE) is a widely recognized, reliable, and valid tool for the early identification of cerebral palsy and other neurological abnormalities. It is user-friendly and has been translated into 18 languages; however, a Thai version has not yet been developed. This study aimed to translate the HINE into Thai and evaluate its intra- and inter-rater reliability.

BACKGROUND AND OBJECTIVES: Neurophobia is the fear of clinical neurology. Little research has explored how the neurological exam contributes to this fear. We wanted to better understand the confidence that medical residents have with the neurological exam and determine how confidence affects their examination of neurological patients.
METHODS: This was a single-centre study in which Internal Medicine and Family Medicine residents rated their confidence in conducting different aspects of a neurological evaluation through a self-administered survey. Low confidence was defined as a score of ≤3 o

BACKGROUND AND OBJECTIVES: ChatGPT has shown promise in healthcare. To assess the utility of this novel tool in healthcare education, we evaluated ChatGPT's performance in answering neurology board exam questions.
METHODS: Neurology board-style examination questions were accessed from BoardVitals, a commercial neurology question bank. ChatGPT was provided a full question prompt and multiple answer choices. First attempts and additional attempts up to three tries were given to ChatGPT to select the correct answer. A total of 560 questions (14 blocks of 40 questions) were used, although any ima

BACKGROUND: Few neurological assessments are easily performed during the first 6h after birth.
AIMS: To assess a cohort of low risk term born newborns within the first 6h and at 48h after birth using the Hammersmith Neonatal Neurological Examination.
STUDY DESIGN AND SUBJECTS: A population of low risk term born newborns was examined within 3h from birth or between 3 and 6h. Each infant was re-assessed at 48h, establishing the range and frequency distribution of neonatal neurological scores at each time point.
RESULTS: Of the 124 full-term born newborns, 62 were assessed at 0-3h and 62 at 3-

BACKGROUND: Transcranial color-coded sonography (TCCS) has proved to be a fast and reliable tool for the detection of middle cerebral artery (MCA) occlusions in a hospital setting. In this feasibility study on prehospital sonography, our aim was to investigate the accuracy of TCCS for neurovascular emergency diagnostics when performed in a prehospital setting using mobile ultrasound equipment as part of a neurological examination.
METHODS: Following a '911 stroke code' call, stroke neurologists experienced in TCCS rendezvoused with the paramedic team. In patients with suspected stroke, TCCS e

OBJECTIVES: Physical examinations performed by residents in many specialties are often incomplete and inaccurate. This report assessed the documentation of the neurologic examination performed by emergency medicine (EM) residents when examining patients with potential psychiatric or neurologic chief complaints.
METHODS: A retrospective chart review of neurologic examinations documented by EM residents was performed. An eight-item neurologic examination score was created and analyzed by resident postgraduate year. A linear mixed model was used to determine if differences in neurologic examinat

The concept of death by neurologic criteria (DNC), a medicolegal construct also known as "brain death", is over 50 years old. Ancillary examinations have a well-defined role in this diagnosis, principally when the clinical examination cannot be completely or safely performed. Radionuclide studies, which provide a combination of ease of performance and high accuracy, remain amongst the most recommended ancillary studies in current clinical guidelines. There are in fact 2 discrete categories of radionuclide studies, using hydrophilic or lipophilic radiopharmaceuticals (RPs), which have overlappi

Carotid endarterectomy (CEA) and coronary artery bypass grafting (CABG) as a combined procedure is occurring with increasing frequency. CEA-CABG incurs the morbidity of both procedures, and considerations for this procedure include anesthetic techniques that provide hemodynamic stability and prompt emergence from anesthesia. We present, to our knowledge, the first reported use in a combined CEA-CABG of the novel opioid remifentanil as a component of the anesthetic technique to achieve these goals. Remifentanil allows for early neurologic evaluation without sacrificing the hemodynamic stability
