NMJ Analyser: a novel method to quantify neuromuscular junction morphology in zebrafish.

MOTIVATION: Neuromuscular junction (NMJ) structural integrity is crucial for transducing motor neuron signals that initiate skeletal muscle contraction. Zebrafish has emerged as a simple and efficient model to study NMJ structural morphology and function in the context of developmental neurobiology and neuromuscular diseases. However, methods to quantify NMJ morphology from voluminous data of NMJ confocal images accurately, rapidly, and reproducibly are lacking. RESULTS: We developed an ImageJ macro called "NMJ Analyser" to automatically and unbiasedly analyse NMJ morphology in zebrafish. Fro

Autoantibody testing in neuromuscular medicine: assay technologies, interpretation, and clinical utility.

Neuromuscular autoantibody testing is an essential component in the diagnosis and management of autoimmune neuromuscular disorders. These immune-mediated diseases target antigens found in nerves, muscles, and neuromuscular junctions, and may occasionally involve the central nervous system, resulting in complex clinical presentations. Developments in antibody identification and validation have provided clinically relevant biomarkers for diagnostic, therapeutic, and prognostic purposes. Among antibody-associated neuromuscular disorders, paraneoplastic onconeural autoantibodies are of particular

Congenital Myasthenic Syndromes.

Congenital myasthenic syndromes comprise a rare heterogeneous group of diseases that impair neuromuscular transmission and are characterized by muscle fatigability and transient or permanent weakness. Symptoms are often present from birth or early childhood. These syndromes have a wide range of phenotypes and severity. Caused by genetic mutations in any of the numerous genes encoding for components of the neuromuscular junction. They are classified by where in the neuromuscular junction the mutated component is located: presynaptic, synaptic, or postsynaptic. Mutations in about 30 genes have b

Beyond Jitter: Spike Count Analysis for Differentiating Botulinum Toxin and Myasthenic Effects on Neuromuscular Function.

PURPOSE: Single-fiber electromyography is the most sensitive tool for diagnosing neuromuscular diseases but is limited in differentiating between presynaptic and postsynaptic neuromuscular junction involvement with increased jitter. With rising botulinum toxin (BoNT) use for therapeutic and cosmetic applications, referrals for electromyography because of myasthenia-like symptoms have increased, complicating differential diagnosis. This study examines whether spike count measurements from single-use concentric needle electrodes can distinguish BoNT effects from neuromuscular junction diseases s

Vaccination in patients with disorders of the muscle and neuromuscular junction.

Neuromuscular diseases cover a broad spectrum of conditions that have important differences. Infections are frequently associated with severe complications, mainly because the neurological involvement can cause the aspiration of respiratory secretions and be significantly worsened by progressive respiratory and/or cardiovascular disease and immunosuppressive therapy. Preventing infectious diseases in patients with disorders of the muscle and neuromuscular junction is clinically important and vaccines play an important role in this regard. The majority of experts recommend that patients with th

Pitfalls and errors in measuring jitter.

The safety factor of neuromuscular transmission can be assessed by measuring the neuromuscular jitter, which reflects the time variability of processes in the motor end-plate. Jitter is increased in any condition with disturbed end-plate function, such as myasthenic conditions and ongoing reinnervation. Jitter is increasingly being measured with concentric needle (CN) electrodes, which are more prone to artefacts than single fiber EMG recordings. The objective of this review is to identify and demonstrate pitfalls that can be seen with CN jitter measurements, made with both voluntary activatio

Neuromuscular junction disorders.

Disorders of the neuromuscular junction, such as myasthenia gravis and Lambert-Eaton myasthenic syndrome, constitute an important and treatable class of diseases. Both disorders are typically caused by an immunologically mediated attack on discrete components of the neuromuscular junction, compromise the efficacy of neurotransmitter transmission, and produce clinically distinct syndromes of fatigable muscle weakness. Although the history, clinical examination, and routine antibody testing can be diagnostic in many cases, specialized neurophysiological tests, such as repetitive nerve stimulatio

Unraveling the causes of sarcopenia: Roles of neuromuscular junction impairment and mitochondrial dysfunction.

Sarcopenia is a systemic skeletal muscle disease characterized by a decline in skeletal muscle mass and function. Originally defined as an age-associated condition, sarcopenia presently also encompasses muscular atrophy due to various pathological factors, such as intensive care unit-acquired weakness, inactivity, and malnutrition. The exact pathogenesis of sarcopenia is still unknown; herein, we review the pathological roles of the neuromuscular junction and mitochondria in this condition. Sarcopenia is caused by complex and interdependent pathophysiological mechanisms, including aging, neuro

Post-stroke sarcopenia as a neuromuscular disorder: a mini review of neural mechanisms and clinical implications.

BACKGROUND: Post-stroke sarcopenia represents a distinct neuromuscular disorder characterized by progressive muscle loss and functional decline. Unlike age-related sarcopenia, stroke-induced muscle wasting involves specific neural mechanisms originating from central nervous system injury. OBJECTIVE: To systematically review the neural mechanisms underlying post-stroke sarcopenia and synthesize evidence on its clinical implications, diagnostic approaches, and therapeutic interventions. METHODS: A mini review of the literature was conducted, synthesizing evidence from key studies addressing ne

Quantification of human neuromuscular function through optogenetics.

The study of human neuromuscular diseases has traditionally been performed in animal models, due to the difficulty of performing studies in human subjects. Despite the unquestioned value of animal models, inter-species differences hamper the translation of these findings to clinical trials. Tissue-engineered models of the neuromuscular junction (NMJ) allow for the recapitulation of the human physiology in tightly controlled in vitro settings. Methods: Here we report the first human patient-specific tissue-engineered model of the neuromuscular junction (NMJ) that combines stem cell technology w

Neuromuscular Diseases.

Neuromuscular diseases are a broadly defined group of disorders that all involve injury or dysfunction of peripheral nerves or muscle. The site of injury can be in the cell bodies (i.e., amyotrophic lateral sclerosis [ALS] or sensory ganglionopathies), axons (i.e., axonal peripheral neuropathies or brachial plexopathies), Schwann cells (i.e., chronic inflammatory demyelinating polyradiculoneuropathy), neuromuscular junction (i.e., myasthenia gravis or Lambert-Eaton myasthenic syndrome), muscle (i.e., inflammatory myopathy or muscular dystrophy), or any combination of these sites. Some neuromus

Plum modulates Myoglianin and regulates synaptic function in D. melanogaster.

Alterations in the neuromuscular system underlie several neuromuscular diseases and play critical roles in the development of sarcopenia, the age-related loss of muscle mass and function. Mammalian Myostatin (MST) and GDF11, members of the TGF-β superfamily of growth factors, are powerful regulators of muscle size in both model organisms and humans. Myoglianin (MYO), the Drosophila homologue of MST and GDF11, is a strong inhibitor of synaptic function and structure at the neuromuscular junction in flies. Here, we identified Plum, a transmembrane cell surface protein, as a modulator of MYO func

Fluctuating, but Not Forgotten: Recognizing Neuromuscular Junction Disorders.

The neuromuscular junction (NMJ), the connection between nerve and muscle, is an essential anatomic site that warrants careful consideration. Dysfunction at this junction often leads to skeletal muscle weakness with a multitude of clinical manifestations. Disorders of the NMJ primarily encompass myasthenia gravis, Lambert-Eaton syndrome, botulism, and organophosphate poisoning. This article will review diseases and disorders of the NMJ, their clinical presentations, and approach to management.

Collagens at the vertebrate neuromuscular junction, from structure to pathologies.

The extracellular matrix at the neuromuscular junction is built upon components secreted by the motoneuron, the muscle cell and terminal Schwann cells, the cells constituting this specific synapse. This compartment contains glycoproteins, proteoglycans and collagens that form a dense and specialized layer, the synaptic basal lamina. A number of these molecules are known to play a crucial role in anterograde and retrograde signalings that are active in neuromuscular junction formation, maintenance and function. Here, we focus on the isoforms of collagens which are enriched at the synapse. We su

Human in vitro neuromuscular junction model to functionally dissect the pathogenic mechanism of anti-AChR autoantibody-positive myasthenia gravis.

BACKGROUND: Myasthenia gravis is a rare autoimmune disease mediated by autoantibodies directed against acetylcholine receptors (AChRs) at the neuromuscular junction. These autoantibodies cause dysfunction through AChR blockade, AChR degradation due to crosslinking and internalisation, and complement activation. METHODS: A novel in vitro model of the human neuromuscular junction was established on a microfluidic platform to investigate the effect of anti-AChR autoantibodies on complement activation and neuromuscular transmission and the mechanism of action of complement inhibition in myastheni

Myasthenia gravis and amyotrophic lateral sclerosis: A pathogenic overlap.

The aim was to examine potential joint disease mechanisms for myasthenia gravis (MG) and amyotrophic lateral sclerosis (ALS) through the examination of long-term patient cohorts for comorbidity. Recent studies support early involvement of the neuromuscular junction in ALS patients with subsequent degeneration of motor neurons. Medical records at Haukeland University Hospital from 1987 to 2012 were examined for International Classification of Diseases diagnostic codes for MG and ALS. Sera were re-tested for antibodies to acetylcholine receptor, titin, MuSK and GM1. We report one patient with bo

Functional Connectivity under Optogenetic Control Allows Modeling of Human Neuromuscular Disease.

Capturing the full potential of human pluripotent stem cell (PSC)-derived neurons in disease modeling and regenerative medicine requires analysis in complex functional systems. Here we establish optogenetic control in human PSC-derived spinal motorneurons and show that co-culture of these cells with human myoblast-derived skeletal muscle builds a functional all-human neuromuscular junction that can be triggered to twitch upon light stimulation. To model neuromuscular disease we incubated these co-cultures with IgG from myasthenia gravis patients and active complement. Myasthenia gravis is an a

Immunoglobulin Treatment in Neuromuscular Medicine.

Intravenous immunoglobulin (IVIg) is used in the treatment of autoimmune diseases, including immune-mediated central and peripheral nervous system disorders. This article will review the indications, proposed mechanism of actions, and administration of immunoglobulin treatment in various neuropathies, neuromuscular junction disorders, and myopathies. IVIg may have more than one mechanism of action to alter the pathogenesis of underlying neuromuscular disease. IVIg treatment has been used as a first-line treatment in Guillain-Barre syndrome, chronic inflammatory demyelinating polyradiculoneurop
