Comparison of onset of neuromuscular blockade with electromyographic and acceleromyographic monitoring: a prospective clinical trial.

BACKGROUND: Reliable devices that quantitatively monitor the level of neuromuscular blockade after neuromuscular blocking agents' administration are crucial. Electromyography and acceleromyography are two monitoring modalities commonly used in clinical practice. The primary outcome of this study is to compare the onset of neuromuscular blockade, defined as a Train-Of-Four Count (TOFC) equal to 0, as measured by an electromyography-based device (TetraGraph) and an acceleromyography-based device (TOFscan). The secondary outcome was to compare intubating conditions when one of these two devices r

Vigilance: the behavioral impact of quantitative monitoring on administration and antagonism of neuromuscular blocking agents.

PURPOSE: The aim of this study was to determine whether use of acceleromyography (1) changes dosing of neuromuscular blocking agents (NMBAs), and (2) increases the time between neostigmine administration and extubation, when compared with subjective assessment of neuromuscular blockade. METHODS: For this retrospective study, data were collected from patient electronic medical records. Patients were included if they had received an NMBA as part of a general anesthetic in 2013 and 2014. Data were analyzed by category of monitoring device: quantitative monitor (acceleromyograph [AMG]) or subject

A comparison of accelerometric monitoring by TOF Watch® SX and electromyographic monitoring by Tetragraph® for recovery from neuromuscular blockade.

BACKGROUND: Neuromuscular monitoring has become a standard of care for management of anesthesia. While acceleromyography (AMG) is the most common technology used in clinical practice, guidelines suggest that electromyographic (EMG) devices are ideal for quantitative neuromuscular monitoring. The Tetragraph® is an EMG monitor that has recently been marketed. STUDY OBJECTIVE: The aim of this study is to assess the agreement during recovery from neuromuscular blockade of this new monitor with the TOF Watch® SX, and to compare intraobserver variability for the two devices. DESIGN: Single-center,

Neuromuscular ultrasound with superb microvascular imaging in muscular sarcoidosis: morphological features and treatment monitoring.

Muscular sarcoidosis is a rare disease characterised by non-caseating granulomas in muscle tissue. We report a woman in her late 70s with progressive lower-limb weakness, in whom ultrasonography revealed distinctive hypoechoic tumours in muscles. Using superb microvascular imaging (SMI), we detected intricate vascular networks within these lesions, which had not been previously reported in muscular sarcoidosis. Based on the clinical presentation and overall sonographic findings, muscular sarcoidosis was suspected, and a subsequent muscle biopsy provided findings consistent with this diagnosis.

Pro-Con Debate: Do We Need Quantitative Neuromuscular Monitoring in the Era of Sugammadex?

In this Pro-Con article, we debate the merits of using quantitative neuromuscular blockade monitoring. Consensus guidelines recommend their use to guide the administration of nondepolarizing neuromuscular blockade and reversal agents. A major impediment to this guideline is that until recently, reliable quantitative neuromuscular blockade monitors have not been widely available. Without them, anesthesia providers have been trained with and are adept at using a variety of qualitative neuromuscular blockade monitors otherwise known as peripheral nerve stimulators. Although perhaps less accurate,

An Intelligent Wearable EMG Sensing Framework for Athlete Neuromuscular Monitoring and Performance Progression Assessment.

Electromyography (EMG)-based sensing is an important tool for assessing neuromuscular activity and monitoring athlete development; its reliability depends on electrode placement, signal quality, and accurate identification of muscle activation periods. This study proposes an intelligent EMG sensing framework integrating preliminary electrode placement assessment, muscle activity detection, feature extraction, and regression-based progression prediction. A placement assessment indicated that positioning the electrode adjacent to the innervation zone produced the highest RMS under the tested con

Effects of partial neuromuscular blockade on lateral spread response monitoring during microvascular decompression surgery.

OBJECTIVE: We evaluated the effect of partial neuromuscular blockade (NMB) and no NMB on successful intraoperative monitoring of the lateral spread response (LSR) during microvascular decompression (MVD) surgery. METHODS: Patients were randomly allocated into one of three groups: the TOF group, the NMB was targeted to maintain two counts of train-of-four (TOF); the T1 group, maintain the T1/Tc (T1: amplitude of first twitch, Tc: amplitude of baseline twitch) ratio at 50%; and the N group, no relaxants after tracheal intubation. Successful LSR monitoring was defined as effective baseline estab

Awareness during emergence from anaesthesia: significance of neuromuscular monitoring in patients with butyrylcholinesterase deficiency.

BACKGROUND: Butyrylcholinesterase deficiency can result in prolonged paralysis after administration of succinylcholine or mivacurium. We conducted an interview study to assess whether patients with butyrylcholinesterase deficiency were more likely to have experienced awareness during emergence from anaesthesia if neuromuscular monitoring had not been applied. METHODS: Patients referred during 2004-2012 were included. Data on the use of neuromuscular monitoring were available from a previous study. Interviews, conducted by telephone, included questions about awareness and screening for post-tr

Our experience with facial nerve monitoring in vestibular schwannoma surgery under partial neuromuscular blockade.

INTRODUCTION AND OBJECTIVES: Facial nerve monitoring is fundamental in the preservation of the facial nerve in vestibular schwannoma surgery. Our objective was to analyse the usefulness of facial nerve monitoring under partial neuromuscular blockade. METHODS: This was a retrospective analysis of 69 patients operated in a tertiary hospital. RESULTS: We monitored 100% of the cases. In 75% of the cases, we could measure an electromyographic response after tumour resection. In 17 cases, there was an absence of electromyographic response. Fifteen of them had an anatomic lesion with loss of contin

Delayed diagnosis of butyrylcholinesterase deficiency with insufficient neuromuscular monitoring and a confounding effect of SedLine® brain function monitoring: a case report.

Intraoperative monitoring has always been a vital part of the care of an anaesthetised patient. Neuromuscular monitoring is important to use when patients have received neuromuscular blocking agents. Quantitative neuromuscular monitors are preferred over qualitative monitors and clinical judgement alone in reducing residual neuromuscular block and the associated respiratory complications. Additionally, brain function monitors can be utilised to assess the level of consciousness in anaesthetised patients. These monitors can be useful during surgical procedures and at the conclusion of a procedu

Feasibility, validation and application of digital tools for remote monitoring in neuromuscular diseases: the DT4RD protocol.

INTRODUCTION: Rare diseases (RDs) affect millions of people worldwide and present major challenges for research due to small patient populations, limited longitudinal data and the difficulty of capturing real-world disease manifestations. Digital technologies provide new opportunities to collect multimodal data directly from patients and their environments. In particular, video recordings, wearable-based physical activity monitoring and digital questionnaires can generate detailed, longitudinal information on symptoms, functional abilities and daily-life impact. The digital tools for rare diso

Comparing home polysomnography with transcutaneous CO2 monitoring to laboratory polysomnography in children with neuromuscular disorders.

STUDY OBJECTIVES: Clinical utility of home polysomnography in children with neuromuscular disorders is limited by lack of evidence that sleep-disordered breathing can be reliably identified and inability to diagnose hypoventilation because carbon dioxide is not measured. METHODS: This study aimed to determine the feasibility, accuracy, and parent satisfaction for home polysomnography performed with a type 2 portable monitoring device and a transcutaneous CO2 monitor. Results of laboratory and home polysomnography were compared with Mann-Whitney U tests and random intercept regression models.

A Successful Use of Electromyography-Based Neuromuscular Monitoring in a Patient With Cerebral Palsy: A Case Report.

Neuromuscular monitoring in patients with cerebral palsy (CP) is technically challenging due to limb contractures and muscle atrophy. We report a case of a 28-year-old male with CP who underwent surgery under general anesthesia. Intraoperative recovery from neuromuscular block was successfully monitored using an electromyography-based neuromuscular monitor. Due to finger deformities, the abductor digiti minimi muscle was selected for monitoring. The onset of rocuronium 0.65 mg/kg was delayed and its duration markedly prolonged. Adequate recovery of neuromuscular function was confirmed with a t

An Experimental Study to Optimize Neuromuscular Blockade Protocols in Cynomolgus Macaques: Monitoring, Doses, and Antagonism.

BACKGROUND: Neuromuscular blocking agents (NMBAs) are a crucial component of anesthesia and intensive care through the relaxation of skeletal muscles. They can lead to adverse reactions such as postoperative residual neuromuscular block. Only one agent is capable of an instant block reversal in deep block situations, but is restricted to aminosteroid agents. Among animal models, non-human primates are an essential model for a great diversity of human disease models. The main objective of this study was to establish a model for NMBA monitoring with current available drugs before testing new rev

Management of Muscle Relaxation With Rocuronium and Reversal With Neostigmine or Sugammadex Guided by Quantitative Neuromuscular Monitoring.

BACKGROUND: The optimal pharmacological reversal strategy for neuromuscular blockade remains undefined even in the setting of strong recommendations for quantitative neuromuscular monitoring by several national and international anesthesiology societies. We evaluated a protocol for managing rocuronium blockade and reversal, using quantitative monitoring to guide choice of reversal agent and to confirm full reversal before extubation. METHODS: We conducted a prospective cohort study and enrolled 200 patients scheduled for elective surgery involving the intraoperative use of rocuronium. Provide
