Intractable pruritus in neuromyelitis optica.

Neuromyelitis optica (NMO) is an immune-mediated disease of the central nervous system. Pruritus in patients with NMO has rarely been reported. We aimed to explore the characteristics and underlying mechanisms of pruritus in NMO patients. We reviewed the case records of 64 NMO patients who visited the Department of Neurology at the Third Hospital of Sun Yat-sen University between 2009 and 2015. Of the 64 aquaporin-4 antibody-positive NMO patients, 18 had pruritus. Spinal magnetic resonance imaging (MRI) showed lesions in all of these patients. A total of 13 patients presented with brain lesion

Multisystem involvement in neuromyelitis optica.

We describe a case of pediatric neuromyelitis optica (NMO) with muscle and lung involvement in addition to central nervous system disease. Our patient initially presented with features of area postrema syndrome, then subsequently with optic neuritis. The patient also had recurrent hyperCKemia that responded to corticosteroids. Finally, axillary and hilar adenopathy with pulmonary consolidation were noted as well and responded to immunomodulation. Our case highlights multisystem involvement in NMO including non-infectious pulmonary findings which have not been described in the pediatric populat

Experimental models of neuromyelitis optica.

For a long time, the most important inflammatory demyelinating diseases of the central nervous system (CNS), for example, multiple sclerosis (MS) and neuromyelitis optica (NMO), were extremely hard to differentiate, often with severe consequences for affected patients. This changed with the discovery of NMO-immunoglobulin G (IgG), a specific autoantibody which was detected in the vast majority of NMO patients, and with the demonstration that this autoantibody targets aquaporin 4 (AQP4), a water channel found on astrocytes in the CNS. These findings paved the way for the generation of experimen

Sarcoidosis associated with neuromyelitis optica.

Neuromyelitis optica (NMO) is an autoimmune disorder diagnosed by an elongated spinal cord lesion associated with unilateral or bilateral optic neuritis and anti-aquaporin 4 (AQP4) antibodies in the serum. It is triggered by or associated with several autoimmune diseases, but not with sarcoidosis. It responds to immunomodulators better than to steroid treatment. Sarcoidosis is an autoimmune disorder which manifests as non-caseating granulomas, usually in the lung parenchyma, but also in other tissues, including the brain. The involvement of the central nervous system in neurosarcoidosis differ

Paroxysmal dystonia and neuromyelitis optica.

Paroxysmal dyskinesias (PD) are thought to be rare movement disorders. The overwhelming majority of reported cases are primary. Secondary PD has seen reported to occur in some conditions, mainly in multiple sclerosis and head trauma. The anatomic origin of the lesion is also rarely seen at the spinal cord. Our objective was to describe four patients with paroxysmal dystonia secondary to spinal lesions during the recovering phase of a neuromyelitis optica (NMO) bout. In the reviewed literature, we do not find any report of PD related to NMO.

Early presentation of neuromyelitis optica.

Neuromyelitis optica is a rare autoimmune demyelinating disease of the central nervous system in childhood. Its relapsing form is usually reported in adults. We report a 3-year-old girl with relapsing, IgG seropositive neuromyelitis optica. Initially she presented with optic neuritis, followed by three relapses with deterioration of optic neuritis and developing transverse myelitis. With each relapse, the treatment was less effective. Four years after the onset of the disease, the patient was blind, had paraplegia associated with urinary and bowel incontinence and short stature.

Neuromyelitis Optica-AQP4: an update.

Devic disease (neuromyelitis optica [NMO]) is an idiopathic inflammatory demyelinating and necrotizing disease characterized by optic neuritis and transverse myelitis, either simultaneously or in isolation. NMO is often idiopathic but may also be associated with systemic autoimmune disease. The prognosis of NMO is severe, especially in those with early and recurrent relapses. MRI studies have revealed that most frequently, there is a long spinal cord lesion that extends through three or more vertebral segments in length. NMO-IgG is the first antibody marker for any inflammatory central nervous

Neuromyelitis optica in Japanese sisters.

We report cases of Japanese sisters with neuromyelitis optica (NMO). The elder sister was 25, when she was diagnosed with right optic neuritis. After 3 months, she developed left optic neuritis and myelitis. At age 27, she had the second relapse, but she has been free from episodes thereafter. The younger sister was 26, when she was diagnosed with optic neuritis. Thus far, she has 9 relapses, comprising both myelitis and optic neuritis. Both sisters had normal brain MRI scans, longitudinally extensive transverse myelitis over 3 vertebral segments, and positive results for anti-aquaporin-4 anti

Neuromyelitis optica: concepts in evolution.

Neuromyelitis optica (NMO) is a rare demyelinating disease, affecting selectively the optic nerve and the spinal cord. It was previously considered to be a severe variant of multiple sclerosis (MS) due to the similar pathological features and its resemblance to optico-spinal, or Japanese, MS, typical of Asian populations. The finding that most NMO patients have auto-antibodies against aquaporin-4, a water channel particularly abundant on the astrocytes of the glia limitans, has allowed early diagnosis and specific treatment of these patients, and has greatly improved our knowledge of its patho

[Neuromyelitis optica: a clinical update].

INTRODUCTION: Neuromyelitis optica (NMO) or Devic's disease is an autoimmune, inflammatory and demyelinating central nervous system disorder that affects mainly to optic nerve and spinal cord. Recent advances have substantially permitted to expand the knowledge about this entity. AIM: To present a clinical update on the current understanding of the nature, progression, diagnosis and treatment of NMO. DEVELOPMENT: Due to its demyelinating nature and its recurrent behavior in most cases, NMO was first considered a form of multiple sclerosis (MS). However, recent findings have led to the conclu

Neuromyelitis optica: effect of gender.

Neuromyelitis optica is a distinct CNS demyelinating disease associated with the autoantibody NMO-IgG, which targets the water channel aquaporin-4. Neuromyelitis optica may be distinguished from typical multiple sclerosis on the basis of key clinical and neuroimaging characteristics in addition to detection of NMO-IgG. The disease has an even stronger female predilection than does multiple sclerosis, with a reasonably consistent female to male gender ratio of >3:1 reported from samples of diverse racial and regional populations worldwide. The gender distribution appears to be similar in both c

[Experimental models of neuromyelitis optica].

Recently a specific auto-antibody response has been found in patients with neuromyelitis optica, which is directed against the astrocytic water channel aquaporin 4. In experimental models these antibodies do not induce disease or lesions in the central nervous system, when present in the circulation of normal rats. However, when T-cell mediated inflammation is induced in such animals, circulating antibodies against aquaporin 4 can reach the brain and induce lesions in the central nervous system, which are closely similar to those, seen in patients with neuromyelitis optica.

Cognitive functions in neuromyelitis optica.

BACKGROUND: Neuromyelitis optica (NMO) is characterized by optic neuritis and longitudinally extensive acute transverse myelitis. The brain is generally considered healthy in NMO, though very recent studies have demonstrated that magnetic resonance imaging abnormalities may be observed in various brain regions of NMO patients. To date, cognitive functions have never been investigated in NMO. OBJECTIVE: To investigate cognitive functions in a cohort of 30 patients with NMO. DESIGN: Observational, prospective study. PATIENTS: We studied 30 patients with NMO and compared them with 30 patients

Rethinking neuromyelitis optica (Devic disease).

Neuromyelitis optica (NMO), or Devic disease, has been distinguished from multiple sclerosis (MS) by the presence of optic neuritis that is usually bilateral, simultaneous, and often severe, myelopathic findings accompanied by longitudinally extensive spinal cord imaging abnormalities, no brain imaging abnormalities typical of MS, and often rapid progression to debility and even death. Researchers at the Mayo Clinic have identified an immunoglobulin marker of NMO (the "NMO antibody") that binds selectively to the aquaphorin-4 water channel and may play a causative role. This marker has been fo

Brain abnormalities in neuromyelitis optica.

BACKGROUND: Neuromyelitis optica (NMO) is a severe demyelinating disease defined principally by its tendency to selectively affect optic nerves and the spinal cord causing recurrent attacks of blindness and paralysis. Contemporary diagnostic criteria require absence of clinical disease outside the optic nerve or spinal cord. We have, however, frequently encountered patients with a well-established diagnosis of NMO in whom either asymptomatic or symptomatic brain lesions develop suggesting that the diagnostic criteria for NMO should be revised. OBJECTIVE: To describe the magnetic resonance ima

Neuromyelitis optica with hypothalamic involvement.

We describe two cases of neuromyelitis optica (NMO) with clinical and radiographically confirmed features of hypothalamic involvement, in the absence of other parenchymal brain lesions. Their course is otherwise typical of Devic's form of NMO. A review of the literature identifies additional cases of NMO in which clinical features attributable to under-recognized dysfunction of the hypothalamic-pituitary axis were present. We propose that the currently accepted criteria for the diagnosis of NMO could be revisited to recognize the possibility of lesions developing within hypothalamic structures

Neuromyelitis optica following infectious mononucleosis.

A case of moderatley severe neuromyelitis optica following infectious mononucleosis is described as the first reported instance of this complication. Total recovery occurred. Rapid improvement followed the commencement of corticosteroids. It is postulated that the pathological process was one of postinfectious demyelination.

[Diagnosis and treatment of neuromyelitis optica].

Neuromyelitis optica (NMO) is an inflammatory demyelinating disease characterized by antibodies against aquaporin-4 in up to 80% of the cases and even less in the NMO spectrum disorders, which may be difficult to distinguish from early multiple sclerosis. While immunosuppressive therapy should be introduced in definite NMO, treatment strategies of NMO spectrum disorders are less clearly defined. Here, we review the current guidelines for treatment of NMO and NMO spectrum disorders in the light of two cases, and suggest a practical approach to the management of these disorders.
