Neuronavigation Based on Track Density Image Extracted from Deterministic High-Definition Fiber Tractography.

BACKGROUND: Diffusion tensor imaging-based navigation is unable to resolve crossing fibers or to determine with accuracy the fanning, origin, and termination of fibers. It is important to improve the accuracy of localizing white matter fibers for improved surgical approaches. We propose a solution to this problem using navigation based on track density imaging extracted from high-definition fiber tractography (HDFT). CASE DESCRIPTION: A 28-year-old asymptomatic female patient with a left-lateral ventricle meningioma was enrolled in the present study. Language and visual tests, magnetic resona

Neuronavigation-assisted endoscopic unilateral cyst fenestration for treatment of symptomatic septum pellucidum cysts.

BACKGROUND: Traditional surgical treatments for this rare disease include open surgical procedures and ventriculoperitoneal shunting. In 1995, endoscopic fenestration was first applied to treatment of cysts of the septum pellucidum (CSP). However, cyst fenestration generally takes a bilateral approach by making two burr holes leading to two fenestrations in the lateral walls of the cyst. Some disadvantages are related to bilateral fenestration. So far, there is no consensus on the surgical indications, the endoscopic approaches, and techniques for CSPs. Based on our experience with 14 cases of

Transcranial acoustoelectric imaging (tABI) of seizure activity in human head model with neuronavigation.

Objective. The human brain consists of multiple interacting neuronal networks that interleave at fine spatial and temporal scales. This complexity presents a challenge for scalp EEG and other noninvasive mapping techniques to accurately identify seizures and abnormal current patterns from background activity. To address this unmet need, this study investigates transcranial acoustoelectric brain imaging (tABI) with neuronavigation as a new method for mapping EEG-derived currents through the skull.Approach.In tABI, ultrasound (US) is focused and steered in the brain as surface electrodes record a

Neuronavigation-guided Judet screw technique for C2 pedicle fractures: how I do it.

BACKGROUND: Atypical Hangman's fractures may involve bilateral C2 pedicle fractures. Surgical fixation is often required to prevent instability and neurological impairment. The Judet technique, involving transpedicular screw fixation, offers a targeted approach to stabilize C2 pedicle fractures while preserving cervical motion. METHOD: This article presents a neuronavigation-guided modification of the Judet technique for C2 pedicle screw placement. Advanced intraoperative computed tomography (CT) imaging, virtual trajectory planning and intraoperative navigation guidance provide surgical prec

The endonasal patient reference tracker: a novel solution for accurate noninvasive electromagnetic neuronavigation.

OBJECTIVE: Electromagnetic (EM) navigation provides the advantages of continuous guidance and tip-tracking of instruments. The current solutions for patient reference trackers are suboptimal, as they are either invasively screwed to the bone or less accurate if attached to the skin. The authors present a novel EM reference method with the tracker rigidly but not invasively positioned inside the nasal cavity. METHODS: The nasal tracker (NT) consists of the EM coil array of the AxiEM tracker plugged into a nasal tamponade, which is then inserted into the inferior nasal meatus. Initially, a proo

Magnetically guided neuronavigation of flexible instruments in shunt placement, transsphenoidal procedures, and craniotomies.

OBJECTIVE: Magnetically guided neuronavigation of flexible instruments is a new tool that can be used in the frameless navigation of deep-seated lesions or shunt placements. Disadvantages of optical systems such as the line-of-sight problem, the necessity of rigid pin fixation of the head, and missing tracking of the tip of flexible instruments should be solved by the new tracking system. Until now, the accuracy of magnetically guided systems was mostly estimated in laboratory setups. METHODS: In this study, intraoperative accuracy of the system was tested in 60 patients with either hydroceph

Resection of subependymal giant cell astrocytoma guided by intraoperative magnetic resonance imaging and neuronavigation.

PURPOSE: Subependymal giant cell astrocytoma (SEGA) is a rare, benign tumor that occurs mainly in children; complete resection can achieve cure. Guidance of surgery by combined intraoperative magnetic resonance imaging (iMRI) and functional neuronavigation is reported to achieve more radical resection and reduced complications. However, reports about the resection of SEGA with such guidance are rare. We report here our preliminary experience of the resection of SEGA guided by iMRI and neuronavigation, focusing on the feasibility, benefits, and pitfalls of this combination of techniques. METHO

Successful resection of a left insular cavernous angioma using neuronavigation and intraoperative language mapping.

Despite recent literature advocating the surgical removal of symptomatic Cavernous Angiomas (CA), even in critical brain areas, very few observations of insular CA surgery have been described, particularly in the left hemisphere. We report the case of a successful resection of a CA located in the dominant insula, using both neuronavigation and intra-operative functional mapping. This 33-year-old right-handed man harbored a left insular CA, revealed by generalized seizures following a bleed confirmed on MRI. The preoperative examination was normal. A stereotactic-guided surgery was performed un

Microscope neuronavigation-guided microsurgical fenestration of quadrigeminal cistern arachnoid cysts: how I do it.

BACKGROUND: Management options for treatment of quadrigeminal arachnoid cysts (QAC) include microsurgical/endoscopic fenestration or shunt. There is an open debate about which method is the best. Microsurgical fenestration is well suited for treatment of QAC with predominant infratentorial component and without hydrocephalus making endoscopic procedures more challenging. METHOD: We describe the microsurgical technique and related anatomy to fenestrate infratentorial QAC through supracerebellar infratentorial approach. We also discuss our experiences with this approach, some of the drawbacks a

Percutaneous radiofrequency thermocoagulation for trigeminal neuralgia using neuronavigation-guided puncture from a mandibular angle.

Percutaneous radiofrequency thermocoagulation (RFT) of the Gasserian ganglion is an effective treatment for primary trigeminal neuralgia (pTN). Currently Hartel anterior approach is the most commonly used method to access the Gasserian ganglion. However, this approach is associated with high recurrence rate and technical difficulties in certain patients with foramen ovale (FO) anatomical variations. In the present study, we assessed the feasibility of accessing the Gasserian ganglion through the FO from a mandibular angle under computed tomography (CT) and neuronavigation guidance.A total of 1
