[Laser: otology, oto-neurosurgery].

The use of lasers in otology is rather recent and their use in otoneurosurgery is even more so, as shown by the limited number of publications dealing with this subject in the literature. After a review of the main bibliographical data on these subjects, the authors present their experience and analyze their results. Two types of lasers were used successively, an Argon laser and then a laser with a double KTP 532 and Nd: YAG 1320 nm beam. The Argon laser and the KTP 532 laser were exclusively used in otology whereas in otoneurosurgery, both KTP 532 and Nd: YAG 1320 nm beams were used depending

Press releases in neurosurgery: is scientific accuracy compromised in public reporting of neurosurgery research?

OBJECTIVE: Press releases describing medical research are commonly used by academic medical centers to present their research to public audiences. Press releases may be written by public relations teams who lack formal scientific training and aim to generate publicity surrounding new research. To the authors' knowledge, press releases on neurosurgery have not been assessed for their presentation and potential misrepresentation of scientific findings. METHODS: Using the search term <Neurosurgery>, the authors identified the 100 most recent press releases published on EurekAlert! (eurekalert.or

[Anaesthesiological Concepts in Paediatric Neurosurgery].

Paediatric neurosurgery presents a unique set for the anaesthesiologist, necessitating a patient tailored approach for a diverse population, ranging from neonates to adolescents and from healthy to chronically ill children. Understanding the intracranial compartment as the sum of brain tissue, blood volume and cerebral spinal fluid is essential. Given that approximately a quarter of a child's cardiac output is dedicated to cerebral perfusion, intracranial bleeding might cumulate in haemorrhagic shock. Anaesthesiological measures to maintain homeostasis and their influences both on cerebral aut

Anaesthesia Considerations on Paediatric Neurosurgery.

Paediatric neurosurgery has seen significant increases and improvements because of advancements in technology and monitoring techniques. This type of surgery presents unique challenges to the anaesthesiology team because of the general characteristics of paediatric patients and the complexity of the procedures. Managing paediatric patients undergoing complex neurosurgery requires profound knowledge of age-related normal physiology and the principles of common paediatric neuroanaesthesia. This review focuses on updated information about various critical topics in paediatric neurophysiology, ble

Clinical Perspectives on Psychiatric Neurosurgery.

INTRODUCTION: Surgical interventions such as stereotactic radiosurgery and magnetic resonance-guided focused ultrasound, and neuromodulatory interventions such as deep brain stimulation (DBS) and vagal nerve stimulation, are under investigation to remediate psychiatric conditions resistant to conventional therapies involving drugs and psychological supports. OBJECTIVE: Given the complicated history of psychiatric neurosurgery and its renaissance today, we sought to examine current perceptions and predictions about the field among practicing functional neurosurgeons. METHODS: We designed a 51

Smartphone-assisted minimally invasive neurosurgery.

OBJECTIVE: Advances in video and fiber optics since the 1990s have led to the development of several commercially available high-definition neuroendoscopes. This technological improvement, however, has been surpassed by the smartphone revolution. With the increasing integration of smartphone technology into medical care, the introduction of these high-quality computerized communication devices with built-in digital cameras offers new possibilities in neuroendoscopy. The aim of this study was to investigate the usefulness of smartphone-endoscope integration in performing different types of mini

Anesthesia for minimally invasive neurosurgery.

PURPOSE OF REVIEW: With an ultimate aim of improving patients overall outcome and satisfaction, minimally invasive surgical approach is becoming more of a norm. The related anesthetic evidence has not expanded at the same rate as surgical and technological advancement. This article reviews the recent evidence on anesthesia and perioperative concerns for patients undergoing minimally invasive neurosurgery. RECENT FINDINGS: Minimally invasive cranial and spinal surgeries have been made possible only by vast technological development. Points of surgical interest can be precisely located with the

Neurosurgery: Skull Base Craniofacial Trauma.

Much of craniofacial trauma involves the frontal sinuses. Because of its response to injury, the frontal sinus mucosa has an innate ability to develop mucoceles, and if infected, mucopyocoeles. This article presents a therapeutic algorithm for all forms of craniofacial trauma with concentration on the most severe injury-the through and through fracture and its surgical remediation.

Remote cerebellar hemorrhage in neurosurgery.

Remote cerebellar hemorrhage (RCH) is a rare complication in neurosurgery that remains enigmatic both in underlying mechanism and clinical behavior. The range of procedures associated with RCH is diverse and includes both supratentorial and spinal procedures that entail significant CSF loss or lesion resection. The risk factors identified in the literature are not sufficient in predicting the patients at risk of developing RCH. It thereby remains an unpredictable hazard that requires better understanding of its pathophysiology. This study is a comprehensive review of the available literature t

[Antiepileptic prophylaxis for elective neurosurgery].

In tumoral surgery, the risk factors for perioperative epilepsy can be roughly grouped into two categories: those related to the preoperative patient's conditions (type and location of the tumors, their impact on the surrounding brain…) and those specifically related to surgery (cerebral edema, parenchymal hematoma, surgical approach, complete or incomplete resection...). The first category is supposed to be responsible for preoperative and late postoperative epilepsy, while the second would be more related to the risk of epilepsy in the first postoperative week (or may be even in the first 48

Patients selection for awake neurosurgery.

BACKGROUND: Based upon the surgical location and indication, including redundant regions, eloquent areas, deep brain stimulation, and epilepsy foci, some patients will benefit from an awake craniotomy, which allows completion of neurocognitive testing during the intra-operative period. This paper suggests patient selection criteria through a new decision algorithm. METHODS: We completed a retrospective chart review at Tampa General Hospital after IRB approval; data were obtained concerning total number of craniotomies, indications, and problems experienced for selection of awake vs. general a

Stereotactic neurosurgery for movement disorders.

Stereotactic neurosurgery for the treatment of movement disorders focuses primarily on the treatment of Parkinson's disease (PD), essential tremor (ET), and dystonia. The surgical targets in use are the subthalamic nucleus (STN) and the globus pallidus internus (GPi) for PD, GPi for dystonia, and ventralis intermedius (Vim) nucleus of the thalamus for ET. Following target selection, procedures include the generation of lesions or the placement of deep brain stimulating electrodes in the selected target. Additionally, transplantation has been used in the treatment of PD. The indications, outcom

Prenatal diagnosis and pediatric neurosurgery.

Advances in realtime ultrasound imaging and a greater availability of high-quality ultrasound equipment have resulted in an increased number of congenital abnormalities being diagnosed prenatally in the last 10-15 years. In addition, testing for maternal serum alpha-fetoprotein, beta-human chorionic gonadotropin, and serum unconjugated estriol has allowed for more sensitive screening for congenital abnormalities. In response to this, in 1989 the Prenatal Diagnosis and Treatment Center was established at our institution to coordinate the care of fetuses and newborns with congenital malformation

[Functional neurosurgery of cerebral palsy].

In 1983, approximately 40 000 patients in France and 5 760 patients in Switzerland suffered from cerebral palsy, representing more than 0.1% of their respective populations. The functional disability of these patients is particularly impressive and emphasizes the medical, social and economic importance of this problem. The term cerebral palsy is restricted to non-progressive disorders of motor function, already observed at an early age and due to cerebral lesions. These motor disorders can be of paretic, dystonic and dyskinetic nature. Their epidemiology, classification, etiology, pathology, e

[Transcranial Doppler sonography in neurosurgery].

Using a 2 MHz transcranial Doppler sonographic system, it has become possible to measure flow velocities in the large basal vessels of the circle of Willis through thin areas of the cranial bone. After having used the method for one year on patients with neurovascular diseases, we now know that it can be employed as a matter of routine, after one has become familiar with it, and that it is of great diagnostic value. Knowing the measuring depth, achived by range gating, and with the aid of compression tests and standardised recording angles, it is possible to reliably identify the investigated

Topical antibiotic prophylaxis in neurosurgery.

Topical antibiotics have been used prophylactically in clean operations by many neurosurgeons. Many others have not used them, however. Review of the experimental literature shows that topical antibiotics will reduce the rate of wound infection in experimentally contaminated wounds. Experience in other surgical disciplines suggests that the infection rate of operations with an intrinsically high risk of infection (greater than 15%) can be reduced substantially by the use of topical antibiotics. There is no sound scientific evidence documenting the benefit of prophylactic topical antibiotics in
