Der Hautarzt; Zeitschrift fur Dermatologie, Venerologie, und verwandte Gebiete2010ger
A 31-year-old patient presented with a new, rapidly growing pigmented skin tumor on her left thigh. Several years previously a level 2 superficial spreading melanoma (SSM) with a tumor thickness of 0.45 mm had been removed from her left thigh. Dermatoscopic examination revealed asymmetric pigmentation of the tumor with branched streaks of variable thickness and irregular pseudopodia. Histological analysis showed it to be a severely dysplastic melanocytic nevus.

Pigmented epithelioid melanocytoma (PEM) is a tumor encompassing epithelioid blue nevus of Carney complex (EBN of CNC) and was previously termed animal-type melanoma. Histologically PEMs are heavily pigmented spindled and epithelioid dermal melanocytic tumors with infiltrative borders, however, their origin remains unclear. Stem cells for the epidermis and hair follicle are located in the bulge area of the hair follicle with the potential to differentiate into multiple lineages. Multiple cutaneous carcinomas, including follicular cutaneous squamous cell carcinoma (FSCC), are thought to arise f

Melanoma arising in blue nevus (MBN) is a rare, aggressive malignancy that can develop from a preexisting blue nevus or resemble a cellular blue nevus without a clear precursor. We present a diagnostically challenging case of MBN on the foot of a 13-year-old female, with two sentinel lymph nodes (SLNs) showing heavily pigmented cells within the capsule and trabeculae. In contrast to the primary tumor, the cells within the SLNs contained bland spindled to epithelioid melanocytes with low proliferation and no significant cytologic atypia. Chromosomal microarray analysis was utilized to compare g

Cutaneous neurocristic hamartoma (CNH) is a rare tumor that arises from cells of neural crest origin. This tumor typically proliferates within the dermis or subcutaneous adipose tissue and usually exhibits pigmentation, which requires differentiation from conditions such as blue nevus and congenital nevus. We report CNH in a 5-month-old female infant. A subcutaneous mass in the parietal region was identified 4 days after birth. Imaging studies revealed that the subcutaneous mass was approximately 7 cm in diameter and involved the intracranial space. An open biopsy specimen obtained at 3 months

Pigmented spitz nevus is a rare benign melanocytic lesion that may clinically mimic malignant melanoma, particularly when occurring at unusual sites such as the genitalia. We report a case of a child presenting with congenital phimosis associated with blackish discolouration of the foreskin. Circumcision was performed and histopathological examination of the excised foreskin revealed a pigmented spitz nevus, a rare benign melanocytic lesion. This case highlights the importance of careful evaluation of pigmented lesions of the foreskin and the role of histopathology in establishing a definitive

Acquired and congenital melanocytic naevi are common benign neoplasms. Understanding their biology and genetics will help clinicians and pathologists correctly diagnose melanocytic tumours, and generate insights into naevus aetiology and melanomagenesis. Genomic data from published studies analysing acquired and congenital melanocytic naevi, including oncogenic driver mutations, common melanoma associated mutations, copy number aberrations, somatic mutation signature patterns, methylation profile, and single nucleotide polymorphisms, were reviewed. Correlation of genomic changes to dermoscopic

We observed histopathologic changes previously described in dysplastic melanocytic nevi in association with a dermal component characteristic of other types of melanocytic nevi or overlapping with features of other varieties of nevi. In order to determine the frequency of these changes, we studied 2,164 cases of compound melanocytic nevi that fulfilled the histopathologic criteria for the diagnosis of compound dysplastic nevus, including architectural pattern, cytologic features, and mesenchymal changes. Of the 2,164 compound dysplastic melanocytic nevi, 1,895 (87.6%) had the histopathologic c

Spitz tumours comprise a spectrum of melanocytic proliferations that share a set of distinct cytological features and molecular pathways. They include benign naevi, intermediate or indeterminate tumours and rare melanomas. Spitz tumours are notorious for the difficulty of distinguishing benign neoplasms with atypical features from melanomas and the related diagnostic uncertainty. Advances in the knowledge of the molecular pathways and genomic aberrations associated with these neoplasms have permitted opportunities for a reduction in the number of uncertain diagnoses and a more objective distin

Since their first description by Sophie Spitz, Spitz nevi have been a subject of controversy among clinicians for many decades, and remain a clinical conundrum until today as their etiology, morphology, biological behavior and natural evolution is still not totally clear. This is because their clinical, dermoscopic and histopathologic features sometimes overlap with those of melanoma, rendering the management of spitzoid lesions particularly difficult. In addition, cases of histopatologically equivocal lesions do exist and their classification might sometimes be very challenging. Among several

Spitz nevus is a type of melanocytic nevus that can arise as a solitary lesion or as multiple lesions either disseminated or agminated (grouped) in different skin backgrounds (eg, grossly normal, hyperpigmented, or hypopigmented). Agminated Spitz nevi have been rarely reported and are even rarer in a background of hypopigmented skin. We present the case of a 2-month-old girl with multiple, grouped, dome-shaped, red papules arising on a hypopigmented patch with a segmental distribution. Biopsy of 2 lesions showed findings characteristic of Spitz nevus, confirming the diagnosis. We also review 4

The patient is a 13-year-old female who had had a patchy lesion on her back since birth which increased in size proportional to her age. When she visited our hospital at the age of 13, the lesion was a 30 x 43 mm, slightly depressed, well defined, irregularly shaped, atrophic plaque. Differential clinical diagnoses at that time included macular atrophy, congenital melanocytic nevus, and mastocytosis. Histopathologically, the lesion was characterized by patchy and scattered infiltrates of oval to spindle cells throughout the dermis extending into the subcutaneous fat tissue. Nerve fibers were f

Atypical fibroxanthomas (AFX) are rare cutaneous tumors, which typically present as a solitary ulcerated papule or nodule on sun-damaged skin. Despite malignant-appearing features on histology, AFX typically pursue a benign clinical course. In rare instances, AFX can form collision tumors with other lesions. However, to the best of our knowledge, AFX in collision with a nevus has never been previously reported. In this study, we describe such a lesion for its novelty and challenge in diagnosis, as this case was originally considered to be melanoma arising in a nevus. On histologic examination,

Cutaneous melanoma occurs only rarely in children under 10 years of age. Mimics of melanoma, including Spitz naevi and proliferative nodules in congenital melanocytic naevi are much more frequent in this age group. Melanoma arising in congenital melanocytic naevus is uncommon, but can show aggressive behaviour. Although spitzoid lesions constitute the majority of 'diagnostically challenging' cases, they are an uncommon cause of mortality in this age group. Among lesions with undoubted metastatic potential, there are biologically distinct tumours which differ significantly in behaviour from the

Multiple Spitz nevi are rare and may occur in agminated, widespread, or dermatomal distributions. Agminated Spitz nevi usually arise in children, presenting on grossly normal, hyperpigmented, or most rarely, hypopigmented skin. We present a child with Langerhans cell histiocytosis who developed bilateral agminated Spitz nevi in the inguinal area. Unusual features included the multifocal distribution, bilateral inguinal location, and co-occurrence with Langerhans cell histiocytosis.

Spitzoid tumors (ST) are a group of melanocytic tumors of high diagnostic complexity. Since 1948, when Sophie Spitz first described them, the diagnostic uncertainty remains until now, especially in the intermediate category known as Spitz tumor of unknown malignant potential (STUMP) or atypical Spitz tumor. Studies developing deep learning (DL) models to diagnose melanocytic tumors using whole slide imaging (WSI) are scarce, and few used ST for analysis, excluding STUMP. To address this gap, we introduce SOPHIE: the first ST dataset with WSIs, including labels as benign, malignant, and atypica

Spitzoid melanocytic neoplasms are a diagnostically challenging class of lesions in dermatopathology. Recently, molecular assays and immunohistochemical markers have been explored as ancillary methods to assist in the diagnostic workup. Specifically, preferentially expressed antigen in melanoma (PRAME) immunohistochemistry is a nuclear stain commonly positive in melanomas, but not in nevi. This study investigates PRAME immunoreactivity (≥75% positive nuclear staining in tumor cells) in a set of 59 spitzoid melanocytic neoplasms with known clinical outcomes. We compared PRAME status with (1) th

An 11-year-old boy had multiple agminated pigmented lesions distributed over the left scapula since the age of 2 years. Shave excision of four grouped lesions showed Spitz nevi (benign juvenile melanoma, spindle or epithelioid cell nevi) that were not totally excised basally. Three years later, a tumorous, keloidlike nodule developed on that site. To the best of our knowledge, giant recurrence within an agminated Spitz nevus has not previously been reported.

To investigate the status of insulin resistance, metabolic syndrome, dyslipidemia, and serum adiponectin levels in patients with uveal melanoma and choroidal nevus were investigated. Our study included 86 patients with uveal melanoma, 38 patients with choroidal nevus, and 86 controls. Uveal melanomas were classified as small, medium, and large on the basis of Collaborative Ocular Melanoma Study (COMS) criteria. Patients with uveal melanoma had significantly higher homeostatic model assessment scores compared with patients with choroidal nevus (P<0.001). Patients with uveal melanoma and choroid

Pediatric melanoma is rare and often poses diagnostic challenges because benign and malignant melanocytic tumors in children can share overlapping clinicopathologic features. Epidemiologic and genomic studies have provided important insights into the etiology of pediatric melanocytic tumors and the genetic alterations associated with malignant transformation. This review summarizes environmental, host, and genetic factors associated with the development of the major pediatric melanoma subtypes, including Spitz melanoma, low-cumulative solar damage (superficial spreading) melanoma, melanoma ari
