BACKGROUND: Far from being benign, somnambulistic episodes can be frequent and/or severe and potentially injurious. Episodes may also be accompanied by sleep mentation with variable degrees of retrograde amnesia. The present study investigated how somnambulistic episodes unfold from childhood through adulthood, a topic that remains understudied.
METHODS: Adult sleepwalkers with a diagnosis of primary somnambulism and a childhood onset of the disorder (n = 113) were assessed for changes in frequency of their episodes, recall of episode-related sleep mentation and aggressive episodes during chi

Nightmares are a prevalent disorder leading to daily impairments. Two cognitive-behavioural self-help interventions--imagery rehearsal and exposure--recently showed short-term efficacy compared to a waiting-list and a group that recorded their nightmares. This paper reports the long-term results of the imagery rehearsal (n=103) and exposure (n=95) interventions. Participants were assigned randomly to a condition after completing baseline measurements; they received a 6-week self-help intervention and completed questionnaires 4, 16 and 42 weeks after end of treatment. Initial effects on nightma

Previously undescribed disorders of nutrition of thiamin, niacin and pyridoxin were observed among the poor people of North China during periods of prolonged deprivation. These disorders were often elicited or exacerbated by physical exertion. Thiamin deficiency syndromes included great toe pain, heel pain, temporomandibular joint click, and painful click of the knee. Syndromes of niacin deficiency included pellagral scrotal dermatitis, hypersecretion of ear wax, and night terrors. Episodic nocturnal motor hyperfunction was caused by pyridoxin deficiency. Several cases with cardiovascular dise

Dreams occur during all stages of sleep. Nightmares are common. They can be associated with poor sleep and diminished daytime performance. Frequent nightmares are not related to underlying psychopathology in most children and in some "creative" adults. However, recurrent nightmares are the most defining symptom of post-traumatic stress disorder and may be associated with other psychiatric illnesses. Night terrors are arousal disorders that occur most often in children and usually occur early in the sleep period. Patients with rapid-eye-movement behavior disorder often present with nocturnal in

This paper tries, after a phenomenological description of the nightmare, to expose the different aetiologies that have been attributed to the origin of nightmares and night terrors by the literature of the past hundred years. External and physiological causes for bad dreams are preferred in the older medical and psychological literature, whereas younger authors consider nightmares as psychogenic or mediated by psychic trauma. New impulses to aetiological theories came from the rise of the psychoanalytic theory, but also from ego-psychological developments that base its concepts on the actual c

To enhance physicians' knowledge of the prevalent problem of sleep disorders, a 50-minute videotape program designed as a learning and self-assessment experience in evaluating and treating these disorders was coproduced with the Network for Continuing Medical Education. The program has been presented nationally to about 25,000 medical professionals. The mean correct score was 50% for the pretest, 69% for the instructional section, and 79% for the posttest; the composite score on the posttest was 59% higher than that on the pretest. Physicians' responses indicated strengths and weaknesses in th

Sleep disturbances are highly prevalent in patients with tic disorders and Tourette syndrome, with up to 80% of patients experiencing sleep difficulties. Insomnia is notably present, and pediatric patients often experience parasomnias, including sleepwalking and night terrors. Disruptions in circadian rhythms are also reported, with a pronounced evening chronotype. Concurrently, patients may also suffer from sleep-related movement disorders, and tics are reported to often persist during sleep, further contributing to significant sleep fragmentation. The presence of comorbid attention-deficit h

There are strong associations between childhood sleep disorders and behavioural, concentration and mood problems. Sleep disorders caused and maintained by behavioural factors (eg, sleep-onset association disorder) are common in young children, and have a significant impact on families. Evaluation should include a medical history, a physical, neurological and developmental examination, a description of any nocturnal events or daytime effects of the child's disturbed sleep, and a good understanding of the family situation and parental management of the child. Management involves recognising the

Parasomnia is a common pediatric sleep disorder that can cause parents or caregivers distress when experienced by their children. Based on the International Classification of Sleep Disorders, parasomnias can be divided into two subgroups: non-rapid eye movement (NREM) parasomnias and rapid eye movement (REM) parasomnias. REM sleep parasomnias include nightmares, REM behavior disorder, and sleep paralysis, whereas NREM sleep parasomnias include disorders of arousal such as confusional arousals, sleepwalking, sleep talking, night terrors, and sleep-related eating disorder. This review focuses on

UNLABELLED: Sleep terrors are a non-rapid eye movement (NREM) sleep-related parasomnia characterized by abrupt terror with a frightening scream. However, it remains unknown whether sleep terrors occur only from NREM sleep. We evaluated 3 cases of a sleep terrors-like parasomnia that occurred exclusively during arousals from rapid eye movement (REM) sleep. These parasomnia events occurred at REM sleep-wake transitions, manifesting with screaming or crying, similar to sleep terrors, without alertness or clear orientation. The patients were all young-adult females without notable medical conditio

A meta-analysis of treatments for nightmares is reported. The studies were identified by database searches and by an inspection of relevant reference lists. The inclusion criteria were: nightmares as a target problem, studies published in English, use of a randomized controlled trials and reporting of nightmare-relevant outcomes. A total of 19 studies, published between 1978 and 2012 were identified, which included 1285 participants. Effect sizes were calculated as Cohen's d. A statistically significant improvement for all studies combined (d = 0.47, 95% CI = 0.33-0.60, fixed effects model; d

BACKGROUND: Sleep complaints and various sleep symptoms are common in elderly persons with cardiac diseases. Nightmares are associated with profound sleep disturbances.
METHODS: The present questionnaire survey with questions on sleep symptoms, nightmares and cardiac symptoms comprised 6103 elderly subjects (39.5% men).
RESULTS: Nightmares occurred rather often in 6.9% and very often in 2.1% of the men. The corresponding frequencies in women were 9.6 and 2.3%, respectively. Irregular heart beats were reported by 11.8% of the men and 131% of the women (NS). Spasmodic chest pain occurred in 12

Narcolepsy is clinically associated with cataplexy, sleep paralysis and hypnagogic hallucinations. It is treated by reassurance (that there is no physical disease) and by stimulants such as ephedrine and amphetamine on an intermittent basis. The special tricyclic antidepressant clomipramine is also used, and mono-amine oxidase inhibitors (MAOIs) are useful in theory. Obstructive sleep apnoea is an important and often unrecognised cause of daytime somnolence. It is treated by weight reduction (pickwickian syndrome), hormones, or recently, with continuous positive pressure apparatus. Night terro

Akinetopsia is a rare neurological syndrome characterized by an impaired perception of movement, often resulting from brain damage due to ischemia, epilepsy, or medication. It is also known as visual motion blindness, and patients with this condition are unable to perceive motion normally even with perfect visual acuity. This report aims to present a case of a patient in their late 40 s who developed akinetopsia and also an impairment in movement perception of objects without emitting sounds, after experiencing a late relapse of breast cancer with the occurrence of multiple brain metastases. OBJECTIVE: The disparity of problems, impairments, and disorders among children in foster care is well-documented and spans virtually every domain of functioning. Sleep, however, has received minimal attention among this vulnerable group, which is concerning given the multitude of ways sleep affects children's development, health, and behavior.
METHODS: A total of 485 foster caregivers from across the United States completed a survey including quantitative items and qualitative, open-ended questions about sleep and related health and behavior for one child (M = 6.4 years, SD = 2.2; range 4-11

There is a noted bidirectional relationship between sleep and posttraumatic stress disorder (PTSD), often associated with nocturnal behavior disorders contributing to sleep disturbances and impaired daytime functioning. Furthermore, disruptive nocturnal behavior (DNB) significantly impairs quality sleep among servicemembers and their sleep partners. Psychotherapy, pharmacotherapy, and alternative, integrative health interventions, such as meditation, are not particularly useful in achieving significant and durable remission of DNB. The use of oral dental appliances has demonstrated clinical su

Graves' disease may rarely present with predominant neuropsychiatric symptoms, such as severe, treatment-resistant panic attacks, leading to misdiagnosis as a primary psychiatric disorder. We describe a 30-year-old woman with refractory panic attacks and night terrors who was initially treated with anxiolytics and antidepressants. Despite fluctuating thyroid function test results, which raised diagnostic uncertainty between evolving hyperthyroidism and thyroiditis, the presence of ophthalmopathy and autonomic hyperactivity prompted further evaluation. Thyroid scintigraphy demonstrated diffuse
