We studied the feasibility of intrathecal ACNU perfusion therapy against subarachnoid dissemination of malignant glioma. Intrathecal perfusion was performed in adult dogs by constant drip administration of 1 to 2 mg ACNU dissolved in 10 to 20 ml of lactate Ringer solution into the lateral ventricle and cerebrospinal fluid drainage through the lumbar puncture. The de novo leukemic transformation of essential thrombocythemia is a rare event, and usually associated with previous treatments. Five patients with malignant pleural effusion were treated with intrapleural administration of ACNU. Three of 5 patients showed cytologically negative in the pleural fluid and the fluid was eliminated in two patients. Vaginal melanoma is a rare tumor, accounting for <1% of all melanomas and ~1-5% of all vaginal malignant tumors. The prognosis of vaginal melanoma is extremely poor, as it is often resistant to chemotherapy and radiotherapy, and metastases may develop in the early stages of the disease. Meningioma is one of the popular benign brain tumors. However, the recurrence of this tumor is not infrequently encountered. In an attempt to establish the useful adjuvant therapy for the recurrent meningioma, in vitro chemosensitivity study for meningioma was conducted. Among various chemotherapeutic agents tested here, cisplatin showed highest cytotoxicity on cultured meningioma cells. In conclusion, cisplatin may be useful in adjuvant chemotherapy for the recurrent meningioma.

A total of 62 patients with metastatic or locally advanced non-small-cell lung cancer were entered in a phase II study of ACNU. Initially, the drug was given i.v. at a dose of 100 mg/m2 every 6 weeks, but due to observed haematological side effects in chemotherapy-pretreated patients, the dose was lowered in this group to 75 mg/m2. A monoclonal antibody, MRK20, in F(ab')2 form [MRK20-F(ab')2], which reacts with 85-kDa membrane protein in a doxorubicin (ADM)-resistant subline (K562/ADM) of human myelogenous leukemia cell line, K562, was examined for reactivity with 41 cultured human leukemia and lymphoma cell lines. B16 melanoma (mouse melanoma) and C24 melanoma (human malignant melanoma) transplanted in mice were treated by a combined therapy of ACNU [1-(4-amino-2-methyl - 5 - pyrimidinyl) - methyl - 3 - (2 - chlorethyl) - 3 - nitrosourea hydrochloride] (10 mg/kg) and hyperthermia (43 degrees C, 30 min). In both types of melanoma, a marked synergistic effect of the combined therapy was noted. Particularly in C24 melanoma a reduction in the size of tumor was observed. Histopathologic findings revealed a strong degeneration such as destruction of the tumor structure and vacuolization of nuclei.

The effect of a combination therapy of hyperthermia (43 degrees C, 30 min) and ACNU (10 mg/kg) on B 16 melanoma and C 24 melanoma in vivo was studied.
