The phagocytic function of leukocytes from peripheral blood of patients with classical rheumatoid arthritis was investigated using nitroblue-tetrazolium reduction of formazan. One control group (A) and two groups of RA-patients were compared. Group B was treated with nonsteroidal antirheumatics, the third group C additionally with low dose steroids (5,0-7,5 mg/d). Reduction to formazan was increased in group B in relation to the control, whereas treatment with cortico-steroids produced decreased phagocytic function of peripheral leukocytes.

Light and electron microscopy were used to analyze in vitro the interaction of Fonsecaea pedrosoi with in vivo activated-macrophages. Adherence of the fungi to the surface of activated macrophages triggers the respiratory burst as revealed by reduction of nitroblue tetrazolium. Twelve stable nondiabetic patients on chronic hemodialysis were studied for their immunobiologic competence while receiving vitamin C supplement and 1-2 months following its withdrawal. 5 children with chronic granulomatous disease had a disorder of tryptophan metabolism, resulting in excretion of gross excess of hydroxykynurenine, kynurenine, and xanthurenic acid, with or without tryptophan loading. Hydroxykynurenine predominated, and in this and other ways the findings were different from those present in most defects of tryptophan metabolism, but were similar to those in pyridoxine deficiency. Oral pyridoxine 30 mg./day corrected this defect, but had no effect on the nitroblue tetrazolium reaction, or bacterial killing by polymorphonuclear leucocytes.

X-linked chronic granulomatous disease (CGD), a defect of leukocyte bactericidal capacity, was seen in three generations of a large kindred. The association of discoid lupus erythematosus (DLE) with CGD was noted. Recurrent antigenic stimulation leading to autoantibody formation may explain the apparently increased frequency of DLE in female carriers of CGD. A screen for CGD, the nitroblue tetrazolium test, is suggested for females with DLE who have experienced suppurative infections or who have a family history of early childhood deaths or recurrent infections.

A new defect of chemotaxis and random motility of polymorphonuclear leukocytes is described in a patient with severe recurrent bacterial infections since early childhood. Signs of infection with a central venous access device in situ raise the possibility of catheter sepsis. We evaluated three tests for diagnosis of infection in infants with suspected catheter sepsis. The acridine orange leucocyte cytospin (AOLC) test was 87% sensitive and 94% specific in the diagnosis of catheter-related sepsis defined by quantitative blood culture. The C-reactive protein and nitroblue tetrazolium tests were not as useful. Using the AOLC results, available in an hour, we now remove fewer catheters on suspicion of sepsis alone.

Chronic granulomatous disease is a rare, inherited disease occasionally found in adults. It is characterized by repeated infections of the skin, lymph nodes, and viscera. The underlying cause is a metabolic inability of the leukocyte to destroy certain ingested bacteria and fungi that normally are saprophytes. The effect of the combination of retinoic acid (RA) and calcium ionophore A23187 on cellular differentiation was assessed in promyelocytic leukemia cell line HL-60. One effective treatment against sporotrichosis is the oral administration of potassium iodide (KI). 30 patients with definite seronegative juvenile rheumatoid arthritis and 3 patients with tendosynovitis and unclear arthralgia respectively were examined by the nitroblue tetrazolium (NBT) test (46 tests). Using a system of sequential daughter cell transfers in semisolid medium, we have analyzed self-renewal and differentiation of human promyelocytic leukemia cells (HL60) in presence of all-trans retinoic acid and human placental conditioned medium (HPCM). We find that retinoic acid induces coordinated losses of self-renewal potential which are followed by phenotypic differentiation. The latter occurs as an all-or-none event and is reversible in the presence of HPCM. A study of leucocyte response to infection, polymorphonuclear leucocyte chemotaxis and bactericidal activity, and nitroblue tetrazolium (NBT) reduction in children with kwashiorkor was undertaken and compared with a control group. The results show that total leucocyte counts were depressed in children with kwashiorkor, and lymphopenia was not infrequent. NBT reduction was normal.
