Aetiology of viral gastroenteritis: a review.

Over the past seven to eight years, several virus groups have been shown to be associated with gastroenteritis. They are adenoviruses, astroviruses, caliciviruses, coronaviruses. Norwalk-like viruses and rotaviruses. In infants and young children, rotaviruses are the single most important aetiological agents of acute gastroenteritis in terms of numbers of cases and also of patients requiring admission to hospital. In older children and adults, the aetiology is less clear, but the present state of our knowledge indicates that Norwalk-like viruses are one of the major causes. Most of these gastr

25- to 30-nm virus particle associated with a hospital outbreak of acute gastroenteritis with evidence for airborne transmission.

Between November 1 and 22, 1985, an outbreak of acute, nonbacterial gastroenteritis occurred in a 600-bed hospital in Toronto, Ontario, Canada. Illness in 635 of 2,379 (27%) staff was characterized by fatigue, nausea, diarrhea, and vomiting and had a median duration of 24-48 hours. The finding of virus-like particles measuring 25-30 nm in six stool specimens and low rates of seroresponse to Norwalk virus (3/39) and Snow Mountain agent (1/6) suggest that a Norwalk-like virus was responsible for the outbreak. The outbreak was of abrupt onset and high incidence, affecting 79 people in a single da

Chapter 9 Infections caused by rubella, reoviridae, retro, Norwalk and ronaviruses.

This chapter focuses on infections caused by rubella, reoviridae, retro, Norwalk and coronaviruses. High incidence of acute gastroenteritis caused by rotaviruses calls for prophylactic and therapeutic measures. Although no vaccine is presently available, it seems likely that vaccines will be developed in the next few years. There are also several rotavirus enzymes useful as targets for antiviral drugs. However, no antiviral drugs have shown therapeutic effects against rotavirus infections. The newly discovered human retrovirus (HTLV) has not yet been investigated in such detail as to predict t

An outbreak of Norwalk gastroenteritis associated with swimming in a pool and secondary person-to-person transmission.

In June 1977 an outbreak of acute gastroenteritis affected 103 students and teachers at an elementary school in Ohio. The illness typically lasted 24 hours or less and was characterized by vomiting (86%) and cramping (70%), but more than half of the persons involved also reported having nausea, diarrhea, and headache. Similar illness frequently followed in household members (29%) of families with primary cases. Investigation revealed that 70% of the children and teachers who swam in a pool at an all day outing June 1 (4 classrooms) and 55% of those who swam during a similar outing June 2 (2 cl

Major incidents. Norwalk on the wild side.

An outbreak of the Norwalk-like virus at an acute hospital in October affected 147 patients and 200 staff. It was brought under control within two weeks, following the establishment of a team to manage the incident. Some day surgery was cancelled, but no inpatient operations were affected. The public was discouraged from visiting the hospital during the outbreak. Local GPs were co-operative about managing patients with the virus at home. Some doctors came to work with symptoms, despite the trust urging affected staff to stay at home.

The annual incidence of food-borne infections in the Netherlands is estimated to be 250,000 or more; registration, however, is lacking. Meat, poultry, milk and eggs are contaminated primarily by intestinal animal commensals (Salmonella, Campylobacter, E. coli O157:H7, Yersinia enterocolitica) or secondarily by animals, humans and the environment during processing (typhoid fever, Shigella, Listeria, Clostridium, hepatitis A virus, Norwalk virus, parasites). The guidelines for the prevention of contamination are insufficient. Intensity of production and the economic importance of fast, large-sca

Virion concentration method for the detection of human enteric viruses in extracts of hard-shelled clams.

A method to extract and concentrate intact human enteric viruses from oyster extracts for detection using reverse transcription-polymerase chain reaction (RT-PCR) was applied to hard-shelled clams (mercenaria mercenaria). Fifty-gram clam samples were processed by an adsorption-elution-precipitation method and then seeded with 10(1) to 10(5) PFU to poliovirus 1 (PV1) and/or hepatitis A virus (HAV). Seeded viruses in extracts were purified by fluorocarbon (Freon) extraction and concentrated by polyethylene glycol (PEG) precipitation and elution. Efficiency of virion recovery from PEG precipitate

[Rotavirus and other viruses causing acute childhood gastroenteritis].

Acute gastroenteritis is one of the most common diseases, affecting children worldwide. Viruses are recognized as a major cause of this disease, particularly in children. Since the Norwalk virus was identified as a cause of gastroenteritis, the number of viral agents associated with diarrheal disease in humans has progressively increased. Rotavirus is the most common cause of severe diarrhea in children under 5 years of age. Human astroviruses, caliciviruses and enteric adenovirus are also important etiologic agents of acute gastroenteritis. Other viruses such as toroviruses, coronaviruses, pi

Cold weather seasonality of gastroenteritis associated with Norwalk-like viruses.

Norwalk-like viruses (NLVs) are the most common cause of acute nonbacterial gastroenteritis in adults, but little is known about their seasonality. The lack of specific diagnostic tools impeded study of these viruses in the past, and surveys using electron microscopy often grouped NLVs with other unrelated viruses. A search of the scientific literature found eight surveys of gastroenteritis, which were conducted for at least 1 year, that specifically identified NLVs. Unpublished data from laboratories of 4 NLV researchers were also used. These surveys, which were conducted in eight countries,

New molecular methods for the detection of hepatitis A and Norwalk viruses in shellfish.

Outbreaks of viral enteric diseases after consumption of shellfish are a major health risk. Methodological problems (such as toxicity for cell cultures and low viral concentrations) and the unculturability of some strains (i.e. hepatitis A virus, Norwalk virus) have made it difficult to study those viruses in the environmental samples. Currently, the analysis of the hygienic quality of marketable shellfish is determined by the use of fecal indicator bacteria, but their reliability in determining viral pollution of shellfish is very low. Recent biotechnology developments are providing available

[Outbreaks of acute gastroenteritis caused by small round structured viruses in Tokyo].

Of 34 non-bacterial gastroenteritis outbreaks which occurred at day-care centers, kindergartens, elementary and secondary schools in Tokyo during the period from February 1985 to June 1991, 28 outbreaks from which small round structured viruses (SRSV) were detected in the patients' stool specimens by electron microscopy were subjected to an epidemiological investigation. The outbreaks tended to occur frequently in the cold season; twenty-two (79%) of these outbreaks from November through April. Though detailed epidemiological informations was not obtained from all outbreaks, the common source

Multi-state outbreaks of acute gastroenteritis traced to fecal-contaminated oysters harvested in Louisiana.

Norwalk-like viruses (NLVs), or small round structured viruses, are known to cause acute gastroenteritis associated with eating contaminated shellfish. Between 1993 and 1996, three oyster-related gastroenteritis outbreaks attributed to NLV occurred in Louisiana. Intensive trace-back and environmental investigations revealed that the overboard disposal of sewage by oyster harvesters into oyster-bed waters was the most likely source of contamination in at least two of the outbreaks. The small infectious dose of NLV, the large quantity of virus particles in stool, and the ability of oysters to co

Correlation of Genogroup I, Genotype 1 (GI.1) Norovirus Neutralizing Antibody Levels With GI.1 Histo-Blood Group Antigen-Blocking Antibody Levels.

BACKGROUND: The in vitro cultivation of human noroviruses allows a comparison of antibody levels measured in neutralization and histo-blood group antigen (HBGA)-blocking assays. METHODS: Serum samples collected during the evaluation of an investigational norovirus vaccine (HIL-214 [formerly TAK-214]) were assayed for neutralizing antibody levels against the vaccine's prototype Norwalk virus/genogroup I, genotype 1 (GI.1) (P1) virus strain. Results were compared with those previously determined using HBGA-blocking assays. RESULTS: Neutralizing antibody seroresponses were observed in 83% of 24

Seafood-associated disease outbreaks in New York, 1980-1994.

BACKGROUND: Seafood-associated disease outbreaks in New York were examined to describe their epidemiology and to identify areas for prevention and control efforts. METHODS: We reviewed reports submitted to the New York State Department of Health (NYSDOH) of seafood-associated outbreaks occurring from January 1, 1980, through December 31, 1994. RESULTS: During 1980-1994, 339 seafood-associated outbreaks were reported, resulting in 3959 illnesses, 76 hospitalizations, and 4 deaths. During this period, seafood-associated outbreaks accounted for 19% of all reported foodborne outbreaks and 10% of

Epidemiology of calicivirus infections in Sweden, 1994-1998.

Outbreaks of acute gastroenteritis are frequently caused by caliciviruses. Electron microscopy was used to search for these viruses in fecal samples from patients with acute gastroenteritis. Of 5800 samples collected and analyzed from November 1994 to June 1998, 3700 were associated with outbreaks. A total of 676 outbreaks were analyzed, and viruses were found in 67%. Caliciviruses, usually Norwalk-like viruses (NLVs), were found in 407 (89%) of 455 outbreaks, while Sapporo-like viruses were identified in nine outbreaks, including six that were suspected to include foodborne transmission. Outbreak of viral gastroenteritis due to drinking water contaminated by Norwalk-like viruses.

Heinävesi, a Finnish municipality with a population of 4860 inhabitants, had an outbreak of gastroenteritis in March 1998. On the basis of an epidemiologic survey, an estimated 1700-3000 cases of acute gastroenteritis occurred during the outbreak. Municipal water consumption was found to be associated with illness (risk ratio [RR]=3.5, 95% confidence interval, 3.11>RR>3.96). Norwalk-like virus (NLV) genogroup II (GGII) was identified in untreated water, treated water, and 4 tap water samples by use of reverse transcription-polymerase chain reaction. This was the first time NLVs had been detect
