Intervertebral disc degeneration is a major contributor to lower back pain worldwide, underscoring the urgent need for effective, minimally invasive regenerative therapies. One of the critical challenges in nucleus pulposus (NP) replacement lies in developing injectable hydrogels with optimal gelation behavior, radiopacity, mechanical properties, and biocompatibility. This study investigates the influence of varying concentrations of Na2HPO4 (0.3%, 0.4%, 0.5%) and BaSO4 (1%, 1.5%) in ALG/PVA hydrogels through a series of gelation time measurements, radiopacity analysis, mechanical testing, and

Intervertebral disc degeneration (IVDD) is a major contributor to lumbar diseases, including low back pain, herniation, and stenosis. Despite significant efforts, there have been limited improvements in treatments to alleviate IVDD. The nucleus pulposus (NP) is a crucial component of the intervertebral disc (IVD), responsible for secreting aggrecan, collagen II, and other extracellular matrix components. Programmed cell death (PCD) of NP cells is believed to play a central role in IVDD. RIPK1 is a key mediator of PCD and recently reported PANoptosis, playing essential role in kidney injury, ar

Intervertebral disc (IVD) degeneration, induced by aging and irregular mechanical strain, is highly prevalent in the elderly population, serving as a leading cause of chronic low back pain and disability. Evolving evidence has revealed the involvement of nucleus pulposus (NP) pyroptosis in the pathogenesis of IVD degeneration, while the precise regulatory mechanisms of NP pyroptosis remain obscure. Misshapen/Nck-interacting kinase (NIK)-related kinase 1 (MINK1), a serine-threonine protein kinase, has the potential to modulate the activation of NLRP3 inflammasome, indicating its pivotal role in

Low back pain is a leading cause of disability worldwide, often attributed to intervertebral disc (IVD) degeneration with loss of the functional nucleus pulposus (NP). Regenerative strategies utilizing biomaterials and stem cells are promising for NP repair. Human NP tissue is highly viscoelastic, relaxing stress rapidly under deformation. However, the impact of tissue-specific viscoelasticity on the activities of adipose-derived stem cells (ASC) remains largely unexplored. Here, we investigated the role of matrix viscoelasticity in regulating ASC differentiation for IVD regeneration. Viscoela

The loss of nucleus pulposus (NP) precedes the intervertebral disk (IVD) degeneration that causes back pain. Here, we demonstrate that the implantation of human iPS cell-derived cartilaginous tissue (hiPS-Cart) restores this loss by replacing lost NP spatially and functionally. NP cells consist of notochordal NP cells and chondrocyte-like NP cells. Single cell RNA sequencing (scRNA-seq) analysis revealed that cells in hiPS-Cart corresponded to chondrocyte-like NP cells but not to notochordal NP cells. The implantation of hiPS-Cart into a nuclectomized space of IVD in nude rats prevented the de

BACKGROUND: Intervertebral disc (IVD) degeneration is characterized by an early decrease in cellularity of the nucleus pulposus (NP) region, and associated extracellular matrix changes, reduced hydration, and progressive degeneration. Cell-based IVD therapy has emerged as an area of great interest, with studies reporting regenerative potential for many cell sources, including autologous or allogeneic chondrocytes, primary IVD cells, and stem cells. Few approaches, however, have clear strategies to promote the NP phenotype, in part due to a limited knowledge of the defined markers and different

Low back pain is the most common cause of disability under the age of 45. The annual incidence of back pain is estimated to be 5% and the lifetime prevalence is 80%. Majority of the patients with persistent symptoms are suffering from radiculopathy that is mainly caused by a herniated nucleus pulposus (HNP). HNP can heal spontaneously due to spontaneous resorption. Besides pressure nucleus pulposus, without any compression, may induce similar changes when applied epidurally to the nerve roots. Nevertheless, combination of chronic mechanical compression and application of nucleus pulposus cause

STUDY DESIGN: Nerve conduction velocity in pig nerve roots was assessed after application of various preparations of nucleus pulposus and control.
OBJECTIVE: To study whether cultured nucleus pulposus cells could reduce nerve conduction velocity after epidural application.
SUMMARY OF BACKGROUND DATA: It is known that nucleus pulposus applied epidurally may reduce the nerve conduction velocity of the adjacent nerve roots and that this reduction seems to be related to the cells of the nucleus pulposus.
METHODS: Nucleus pulposus cells and fibroblasts were cultured for 3 weeks, and various prep

Once thought to be solely involved in vasculogenesis, tyrosine kinase with immunoglobulin-like and EGF-like domains 2 (TIE2) has emerged as a crucial marker of progenitor-like cells in the avascular nucleus pulposus (NP), a tissue with notoriously limited regenerative capacity. Recent evidence suggests that TIE2 + NP cells play a pivotal role in disc tissue homeostasis, influencing extracellular matrix maintenance, cellular renewal, and tissue integrity. However, despite the reported regenerative potential of TIE2 + NP cells, their precise function remains enigmatic. This review consolidates i

Degeneration of the lumbar intervertebral discs is a leading cause of low back pain, a condition that affects a substantial proportion of the global population. The earliest degenerative changes typically manifest in the central nucleus pulposus (NP) region of the disc and are characterized by elevated local inflammation and a cell-mediated reduction in proteoglycan content and transition to a fibrotic extracellular matrix. The application of adult mesenchymal stem cells (MSCs) to arrest and reverse degenerative changes in the NP has received significant interest due to the availability and su

The intervertebral disc (IVD) is the largest avascular organ of the human body and plays a fundamental role in providing the spine with its unique structural and biomechanical functions. The inner part of the IVD contains the nucleus pulposus (NP), a gel-like tissue characterized by a high content of type II collagen and proteoglycans, which is crucial for the disc's load-bearing and shock-absorbing properties. With aging and IVD degeneration (IDD), the NP gradually loses its physiological characteristics, leading to low back pain and additional sequelae. In contrast to surrounding spinal tiss

We have developed a simple, inexpensive and innovative device for reproducing the global mechanical behavior of spinal motion segments and the local mechanical environment experienced by lumbar intervertebral discs. The device has several broad functions: (1) exploration of the basic mechanics underlying this complex skeletal system, (2) connecting changes in tissue characteristics with overall motion segment function, and (3) evaluation of strategies for repair and replacement of disc components. This "disc emulator" consists of three main parts: (1) an artificial annulus fibrosus (AAF), made

Intervertebral disc (IVD) degeneration causes low-back pain through disc compression, prolapse and herniation. Inflammation of the IVD and subsequent degeneration produce altered glycosylation profiles in several animal models of IVD injury and ageing, although the function of this altered glycosylation pattern in a human is unknown. Altered N-glycome, specifically sialylated and fucosylated N-glycosylation motif expression, might play a role in inflammation and disease progression. Healthy (foetal and adolescent idiopathic scoliosis) and degenerated (lumbar degeneration) human IVD glycosylati

Lumbar disc hernia is common disease, affecting about 5% of the population. Many studies to date reported regression of disc herniation without surgical intervention.Medical records of the patients who applied to the spine clinic in an outpatient setting were retrospectively reviewed. Age, sex, radiological findings, neurological examinations, and medical treatments of the patients were evaluated.Male patients constituted 52.6% of the cases (n = 40) and 47.4% (n = 36) were female. The ages of the patients ranged from 25 to 82 years, with a mean of 48.5 ± 12.1 years. Visual analog scale (VAS) m

BACKGROUND: We theorized that active cervical extension should influence the position of the nucleus pulposus (NP) within the intervertebral disk (IVD) in the sagittal plane. Although several studies on the lumbar IVD have been conducted, there are no quantitative data for in vivo positional changes of the NP in the cervical IVD.
OBJECTIVE: To evaluate the influence and mechanism of cervical extension on the deformation and migration of IVD and NP in the sagittal plane and understand underlying mechanisms of the extension maneuver.
DESIGN: Asymptomatic subjects underwent magnetic resonance i

The causes of herniated disc in vivo vary, and recent studies stress the role of intradiscal pressure as an important factor. Identifying causes of increased intradiscal pressure is difficult. This case report describes the diagnosis of a herniated nucleus pulposus in a 20-yr-old healthy man, with sudden onset of emesis. The diagnosis was made by a complete history, physical examination, electrodiagnostic studies, and magnetic resonance imaging. This etiology of herniated nucleus pulposus has not been described in the medical literature, and the severity of the symptoms, clinical examination,

Growth in volume of the anulus fibrosus (AF) and nucleus pulposus (NP) was quantified using serial histological sections of human and kitten fetuses. Fetal intervertebral discs were studied that had clearly outlined AF and NP. Regression equations were calculated and graphs plotted by microcomputer. An increase in surface areas of these intervertebral structures was also recorded; however, volume was a better indicator of relative growth than was surface area. The AF volume of the fetal human increased more in proportion to the intervertebral disc than it did for the fetal kittens. There was s
