Pancreatic cancer is the ninth common malignancy in South Korea. It has a dismal prognosis with a 5-year overall survival rate of less than 10%, and pancreatic cancer is associated with cancer cachexia, which is defined as the loss of muscle mass that is not reversible by conventional nutritional support. Cachexia is noted in over 85% of all pancreatic cancer patients and it is strongly related with the disease's mortality. Nearly 30% of pancreatic cancer deaths are due to cachexia rather than being due to the tumor burden. Therefore, it is crucial to discover the mechanisms behind the develop

Nutritional care in the Eating Disorder unit of Sainte-Anne general hospital in Paris, is organised around a care model based on cognitive behavioural therapy. Hospitalisation is generally prepared beforehand and aims to draw on patients' resources enabling them to clarify a request for help. A care contract can be drawn up to provide step-by-step support for the patient in terms of the goals to achieve.

UNLABELLED: Systemic sclerosis (SSc) is a chronic multisystem autoimmune disease which involves the gastrointestinal tract in about 90% of cases. It may contribute to nutritional deterioration.
OBJECTIVE: To assess whether the application of a nutritional support protocol to these patients could improve their nutritional status and quality of life.
METHODS: Single center prospective study, performed on an outpatient basis, in a county hospital. The Malnutrition Universal Screening Tool (MUST) was used to screen risk for malnutrition. Health questionnaire SF-36 and the Hospital Anxiety and De

Many factors affect wound healing, including nutritional status and recent nutritional intake. Patients with infected wounds have increased requirements for nutrients and often have a reduced food intake. It is therefore important that nutritional status is carefully considered, and all patients should be screened for malnutrition using a nutrition screening tool. Nutrition plans should aim to provide sufficient energy while maintaining good glycaemic control, adequate protein and hydration, and a minimum of 100% reference nutrient intake for micronutrients. Measures required to achieve this w

Chylothorax is a well-known, albeit infrequent, complication in certain neoplasias including lymphomas. The continuing loss of chyle leads to a profound deterioration in patients' nutritional and immunological status, prevented only by appropriate early nutritional support. There is currently some dispute over its handling, particularly with regard to the most appropriate type of nutritional support and the suitability of conservative treatment versus surgery. The present paper describes the case of a 41-year-old patient diagnosed as having secondary chylothorax following B-cell lymphoma where

BACKGROUND: Malnutrition continues to be a significant problem in patients undergoing surgery. The relationship between a poor nutritional status and subsequent post-operative morbidity and mortality is well recognised. It is logical, therefore, that malnourished patients should be given nutritional supplementation in the peri-operative period. However, the benefits of this approach have been debated and as we enter the new millennium there are many questions regarding then role of peri-operative nutritional support that remain unanswered.
AIMS: This review aims to determine how we decide whi

The child with a malignancy frequently will have associated cachexia with significant weight loss and malnutrition. The reasons for this are multifactorial and may be related directly to the tumor, such as increased metabolic rate, circulating peptides leading to anorexia, and decreased intake due to poor appetite or gut involvement. There appears to be other reasons involved, including increased whole body protein breakdown, increased lipolysis, and increased gluconeogenesis. Release of certain cytokines, such as tumor necrosis factor, interleukin-1, interleukin-6, and others may increase the

Interventional nutritional support is a complex therapeutic entity. Metabolic complications associated with this therapy are numerous. It is imperative to monitor electrolytes very closely during parenteral and enteral nutritional support and to correct deficiencies or to compensate for increases in serum concentrations when appropriate. It is also critical to observe patients receiving drug therapy to avoid untoward drug-nutrient interactions and to be able to compensate for adverse metabolic effects of medications. To achieve successful nutritional support, careful monitoring of electrolytes

Ever since methods of artificial nutritional support became available, attempts have been made using this form of treatment to reduce mortality and morbidity in surgical patients. Many trials have addressed this question, but very few have given a meaningful answer because of conceptual and methodological flaws. We therefore undertook a prospective randomised trial investigating the effects of at least 10 days pre-operative total parenteral nutrition (TPN) (n = 51) or total enteral nutrition (TEN) (n = 50) providing 150% basal energy expenditure (BEE) non-protein energy, to reduce major postop

The role of nutritional support in the management of the critically ill patient has been a topic of much concern and research. Malnutrition experienced by patients with cancer can be related to the nutritional status of the patient before the development of cancer, to the tumor itself, and to cancer therapy. Aggressive treatment may increase the degree of malnutrition, and the combination of the effects of therapy and progressive malnutrition may be a frequent cause of death. The use of intensive nutritional support for some patients may promote weight gain and positive nitrogen balance, incre

Nutritional and clinical responses to three nutritional regimens were retrospectively evaluated in 81 Crohn's patients with active disease. Group 1 (n = 42) received a low residue oral diet, group 2 (n = 15) received chemically defined diets, and group 3 (n = 24), parenteral nutrition (PN). Weight gain was observed in a similar percentage of patients, whereas serum albumin increase was significant only in group 3: 3.15 +/- 0.66 versus 3.54 +/- 0.61 g/100 ml (p less than 0.05). Mean activity index decreased significantly in all groups (p less than 0.001), and length of stay in hospital was simi

Our current ability to favorably influence the adverse consequences of malnutrition in adult patients with established cancer is quite limited. Nutrient provision alone has not been successful in this regard. In fact, the approach of managing cancer patients with weight loss by solely providing calories is almost entirely extrapolated from clinical situations in which the presence of cancer is not a confounding problem and, therefore, may well be seriously flawed. These conclusions may not apply to situations in which special considerations hold, such as childhood malignancies and bone marrow

Mechanically ventilated patients generally depend on artificial means for nutritional supplementation. In this article we review the magnitude, pathophysiology, and clinical significance of proteolysis, and evidence that nutritional repletion is beneficial. Methods for assessing nutritional status and response to nutritional intervention are briefly discussed, as well as basic principles of anabolic and anticatabolic therapy.

Hypoalbuminemia is considered one of the hallmarks of protein-calorie malnutrition and chronic liver disease. Recently, serum albumin has also been proposed as a critical predictor of the response to nutritional support and tolerance to enteral feeding in critically ill patients. Albumin is essential for maintenance of plasma colloidal osmotic pressure, prevention of edema, and transport of certain drugs and nutrients. Experimental studies have shown that rapid plasma expansion and reduced plasma protein concentration and osmotic pressure induce a net secretion of sodium and water into the sma

Although malnutrition is associated with poor clinical outcome, it cannot be inferred that better nutrition will improve clinical outcome. Efficacy of a proposed regimen is best established by prospective, randomised, controlled trials. Cost effectiveness is only an issue if efficacy exists. Patients with long term temporary, or permanent, inadequate bowel syndrome are candidates for parenteral nutrition. Most of the prospective, randomised, controlled trials testing the value of nutritional support in other diseases, however, have failed to show that this treatment has a beneficial clinical e

The administration of a complete and well-balanced diet commensurate with the requirements and limitations of individual patients is essential to obtain maximal benefit of nutritional therapy. Complications related to the provision of nutritional therapy must be considered when providing nutrition support to patients with malignant diseases. Some of these complications are not exclusive of the form of therapy, whereas others are inherent in the different components of nutrient solutions used. A sound knowledge of clinical nutrition as well as the establishment of nutrition support teams will h

Cancer patients in whom elective surgical intervention is planned are frequently malnourished. Moreover, the tumor itself may be responsible for additionally altering metabolism in the host, although the mechanisms by which this occurs are not clear. All preoperative cancer patients should be carefully surveyed for indices of malnutrition. Patients with a history of inadequate oral protein and calorie intake, an unintentional weight loss of greater than 10 pounds, or a serum albumin level of less than 3.5 gm per dl should undergo a thorough nutritional assessment, including anthropometric meas

Administration of nutritional support in acutely ill patients can be seen as a balance between the need to provide nutrients and the known complications associated with infusions of each of the three major nutrients--glucose, fat, and protein. It is prudent to maintain calorie intake within reasonable limits. If facilities for measurements of O2 consumption and hence metabolic rate are available, these should be utilized. Otherwise, metabolic rate should be estimated. In patients who are in satisfactory nutritional condition and who are being nourished to preserve lean body mass, diets designe
