Wernicke's encephalopathy after gastric partitioning for morbid obesity.

Two weeks after gastric partitioning for morbid obesity, a 45-year-old woman experienced persistent vomiting that led to a weight loss of 30 kg over 6 weeks. Wernicke's encephalopathy and peripheral neuropathy developed. The Wernicke's encephalopathy responded well to the administration of thiamine. This is one of very few reported cases of Wernicke's encephalopathy following gastric partitioning.

Surgical treatment of morbid obesity with reflux esophagitis.

Experience with six morbidly obese patients with symptomatic reflux esophagitis associated with hiatal hernia, the diagnoses confirmed by upper GI x-ray contrast studies and endoscopy, has been reported. Truncal vagotomy with Heineke-Mikilicz pyloroplasty and gastric bypass with Roux-en-Y gastrojejunostomy has controlled all symptoms of esophagitis and produced effective weight loss. It is suggested that if the proximal gastric pouch is no larger than 25 ml there is no need to perform truncal vagotomy and pyloroplasty.

Surgery for morbid obesity in a provincial centre.

Forty-eight patients underwent surgery for morbid obesity, 20 had gastric bypass and 28 had gastric partitioning or stapling. Gastric bypass was effective in producing satisfactory weight loss. There were few long term sequelae. However, it was complex and difficult technically, with a substantial mortality--three deaths. Gastric partitioning was simpler and safer, no mortality or serious complications but it was less reliable in producing satisfactory weight loss. Eleven patients showing no sign of getting within 20 percent of their ideal weight; this includes four with staple disruption. A s

Tuberculosis after jejuno-ileal bypass for morbid obesity.

A patient contracted tuberculosis after 2 operations for morbid obesity. The difficulty in diagnosis and treatment is described. Jejuno-ileal bypass is a non-physiological operation, with many reported complications and side effects. Following this short experience the author and his colleagues have now abandoned this operation. Patients suffering from malnutrition as a result of slimming operations should be carefully monitored for tuberculosis.

Jejunoileal bypass as a treatment of morbid obesity.

Seventy-five morbidly obese patients underwent jejunoileal bypass between December 1968 and October 1975. The average weight of 45 patients who had had the bypasses for over two years stabilized at 62.4% of the maximum preoperative value. Postoperative complications included recurrent diarrhea in 11% (8/74) and serum electrolyte deficits in 64% (48/75), most of which were transient. Potassium and calcium deficiencies were usually correctable by oral supplementation, but hypomagnesemia persisted in 23% (16/71) despite supplementation. Nine percent (4/43) had biopsy-proved hepatic cirrhosis afte

Gastric bypass for morbid obesity: results and complications.

Seventy-five patients underwent a 90 percent distal gastric bypass for morbid obesity. The average weight was 121.4 kilogram, height 164 centimeters, and age 31.4 years in these 70 women and five men. There was a total of 20 surgical complications in the 75 patients, with wound infection being the most common. Sixteen chronic complications were noted and consisted of vomiting, diarrhea, reflux esophagitis, dysphagia, and vitamin deficiencies. None of the above complications were life-threatening or required dismantling of the bypass. Of 54 patients followed for 12 months or more after gastric

Maternal obesity and morbid obesity: the risk for birth defects in the offspring.

BACKGROUND: The objective of this study was to assess, in a large data set from Swedish Medical Health Registries, whether maternal obesity and maternal morbid obesity were associated with an increased risk for various structural birth defects. METHODS: The study population consisted of 1,049,582 infants born in Sweden from January 1, 1995, through December 31, 2007, with known maternal weight and height data. Women were grouped in six categories of body mass index (BMI) according to World Health Organization classification. Infants with congenital birth defects were identified from three sou

The risk of herpes zoster is positively associated with obesity, especially morbid obesity.

This study aimed to investigate the association between obesity and herpes zoster (HZ) occurrence. This study used data covering 2 million people in Taiwan in 2000, which were obtained from the National Health Insurance Research Database. The cohort study observed aged 20-100 years with obesity from 2000 to 2017 (tracking to 2018). Obesity was indicated by the presence of two or more outpatient diagnoses or at least one admission record. And, obesity was categorized into non-morbid obesity and morbid obesity. Patients with HZ before the index date were excluded. The obesity cohort and control

Significance of GNAS mutations for morbid obesity in children.

BACKGROUND: Hereditary forms of obesity are characterized by early severe heterogeneous manifestations of the phenotype along with a rapid progression to morbid obesity, primary due to pathogenic variants of certain genes. Most forms are characterized by moderate to severe neuropsychic developmental delays, dysmorphic features and organ-specific developmental anomalies. RESULT: We searched for hereditary causes of morbid obesity in children by exome sequencing. As a result, we have identified 5 variants in the GNAS locus, two of which were identified for the first time: NM_000516.7(GNAS):c.20

[Treatment of ventral hernias in patients with morbid obesity].

The review is devoted to the treatment of ventral hernias in patients with morbid obesity. This issue is important due to significant number of such patients and no unambiguous clinical recommendations. The advantages of simultaneous surgery (with bariatric intervention) are obvious, i.e. lower risk of postoperative hernia incarceration and no need for re-hospitalization with another intervention. High risk of bariatric population makes it necessary to minimize surgery time and surgical trauma. A staged approach with reducing body weight surgically or conservatively before hernia repair is oft

Abdominal Fat Tissue Echogenicity: A Marker of Morbid Obesity.

PURPOSE: Menopause-related changes may affect regional but also morphological characteristics of adipose tissue. We sought to assess the clinical value of echogenicity of subcutaneous adipose tissue (SAT) and preperitoneal adipose tissue (pPAT) in postmenopausal women without cardiovascular disease. METHODS: In 244 consecutively recruited postmenopausal women, subclinical atherosclerosis was assessed in the femoral and carotid arteries by intima-media thickness (IMT) and atheromatous plaques using high-resolution ultrasonography. In 41 women with a second visit (median follow-up 41.5 months),

Gastroesophageal reflux disease and morbid obesity: evaluation and treatment.

Gastroesophageal reflux disease (GERD) is markedly increased among the obese population being recognized as one of the many obesity-related comorbidities. This concept should raise awareness, making physicians investigate more profoundly about this disease in this kind of patients. Currently, bariatric surgery is considered the gold standard treatment for morbid obesity. However, not all the operations are appropriate for the treatment of GERD and not all the patients are willing to receive bariatric surgery for the treatment of GERD. Even though sleeve gastrectomy has emerged as a suitable tr

Morbid obesity and hypertension: The role of perirenal fat.

Accumulation of fat in renal sinus and hilum is associated with hypertension development. We evaluated the relationship between perirenal fat and hypertension in a population of morbidly obese patients and the potential variations after sleeve-gastrectomy. Two hundred and eighty-four morbidly obese patients were included in the study, and 126 underwent sleeve-gastrectomy. At baseline and 10-12 months after surgery, we evaluated anthropometric parameters, blood pressure, glycometabolic, and lipidic assessment, and performed an ultrasonographic evaluation of visceral fat area and perirenal fat t

Nephrological aspects of surgical weight correction in morbid obesity.

Obesity, including morbid obesity, is a growing worldwide problem. The adverse effect of obesity on the kidneys is associated with the development of comorbid conditions, such as insulin resistance (IR), metabolic syndrome (MS), diabetes mellitus (DM), arterial hypertension (AH), which are the recognized risk factors of chronic kidney disease (СKD). Obesity also causes direct kidney damage with the development of non-immune focal segmental glomerulosclerosis. The leading pathophysiological mechanisms of kidney damage in obesity are intrarenal hemodynamic disorders with the formation of hyperfi

[Acute internal hernia following gastric bypass for morbid obesity].

BACKGROUND: Approximately 3,000 bariatric operations are performed in Norway annually. Laparoscopic gastric bypass is the most widely used procedure. Internal hernia is a potentially serious cause of acute abdominal pain after a gastric bypass. MATERIAL AND METHOD: The review is based on a discretionary selection of articles found in PubMed coupled with the authors' personal clinical experience. RESULTS: Internal hernias due to protrusion of the bowel through mesenterial defects, or slits, occur in approximately 0-10% of patients after a gastric bypass. The incidence depends on follow-up tim

Delayed diagnosis of abdominal mass due to morbid obesity.

Obesity has become an increasing problem in developed countries. Obesity is associated with many of the most common causes of morbidity and mortality, including diabetes mellitus, coronary artery diseases, sleep apnoea, and many types of cancers. Not only is morbid obesity associated to a greater extent to the many medical problems related to obesity, but the body habitus of a morbidly obese patient causes limitation to physical examination, radiologic evaluations, and therapeutic interventions that can delay diagnosis and treatment of the patient. We present a case of a morbidly obese patient
