Obesity management before and after pancreas transplantation.

PURPOSE OF REVIEW: We provide a review on the incidence, consequences, and management of obesity in patients before and after pancreas transplant. RECENT FINDINGS: Obesity is common in patients with both type 1 and type 2 diabetes. Obesity at the time of pancreas transplant is associated with worse graft and patient survival, while weight gain after transplant is associated with insulin resistance and posttransplant diabetes. Currently, lifestyle interventions are the backbone of obesity management and can improve insulin sensitivity, but result in only modest weight loss. Metabolic and baria

Medical management of obesity: unlocking the potential.

After a long and challenging history, there have finally been major breakthroughs in the development of effective obesity medications. Agents that act at receptors of one or more gut hormones are achieving unprecedented weight reductions and improvements in cardiovascular risk factors, comparable to some bariatric surgical procedures. Importantly, there is evidence of beneficial effects on a growing range of conditions, including type 2 diabetes, fatty liver, chronic kidney disease, obstructive sleep apnea and cardiovascular disease. Barriers to access need to be overcome to allow the standard

Commercial products and programmes in obesity management.

RECOMMENDATIONS 1. For adults living with overweight or obesity, some commercial programmes exist that should achieve mild-to-moderate weight loss over the short or medium term, and a mild reduction in glycated haemoglobin values over a short term in adults with type 2 diabetes compared with usual care or education. 2. We do not recommend the use of over-the-counter commercial weight loss products for obesity management, owing to lack of evidence (Level 4, Grade D). 3. We do not suggest that commercial weight loss programmes be used for improvement in blood pressure and lipid control in adults

Food Addiction in Application to Obesity Management.

Nascent and rapidly growing research into defining the concept of "food addiction" (FA) in relation to obesity is currently underway. Food addiction is present in a subset of patients who are overweight or have obesity, and its presence is associated with other psychosocial disorders such as depression, anxiety, eating disorders, impulse control disorders, and lower quality of life. Food addiction is associated with higher intake of and preference for highly processed foods that have addictive-like properties such as cravings, reward sensitivity, and impaired control. Food addiction is associa

ER stress in obesity pathogenesis and management.

Given the unprecedented global pandemic of obesity, a better understanding of the etiology of adiposity will be necessary to ensure effective management of obesity and related complications. Among the various potential factors contributing to obesity, endoplasmic reticulum (ER) stress refers to a state of excessive protein unfolding or misfolding that is commonly found in metabolic diseases including diabetes mellitus, insulin resistance (IR), and non-alcoholic fatty liver disease, although its role in obesogenesis remains controversial. ER stress is thought to drive adiposity by dampening ene

Family Management of Severe Obesity in Adolescents.

PURPOSE: Severe obesity, defined as a body mass index (BMI) ≥120th percent of the 95th BMI percentile for age and sex, is the fastest growing subcategory of obesity among youth, yet little is known about how this group understands and incorporates weight management strategies. The aims of this study were to explore how parents and adolescents understand severe obesity and incorporate management into their daily lives and evaluate the applicability of the Family Management Styles Framework (FMSF) to better understand the impact of severe obesity for adolescents. DESIGN AND METHODS: Directed co

Obesity and male infertility: Mechanisms and management.

Obesity is considered a global health problem affecting more than a third of the population. Complications of obesity include cardiovascular diseases, type 2 diabetes mellitus, malignancy (including prostatic cancer), neurodegeneration and accelerated ageing. In males, these further include erectile dysfunction, poor semen quality and subclinical prostatitis. Although poorly understood, important mediators of obesity that may influence the male reproductive system include hyperinsulinemia, hyperleptinemia, chronic inflammation and oxidative stress. Obesity is known to disrupt male fertility an

Obesity: long-term management after bariatric surgery.

For many patients, bariatric surgery results in enduring weight loss and comorbidity resolution, but it is not without risk of complications. Complications necessitating possible surgical intervention include small bowel obstruction, internal hernia, intussusception, anastomotic ulcer, and anastomotic leak. Medical complications include dumping syndrome; osteoporosis; substance abuse; nephrolithiasis; high suicide rates; and nutritional deficiencies, some of which can result in neurologic disorders. Patients need to take daily multivitamins with minerals (containing iron, copper thiamine, and

Obesity management--an opportunity for cancer prevention.

There is increasing evidence of an association between obesity and the development, morbidity and mortality of cancers of the colorectum, (post menopausal) breast, endometrium, kidney, pancreas and oesophagus. In addition to obesity per se, waist circumference is now emerging as a clear indicator of disease risk. Weight gain during adult life also appears to increase risk for breast and colon cancers. Major causative factors which are influenced by excess energy storage include hormones involved in metabolic control (insulin and leptin), cell growth (IGF-I and IGF-binding proteins) and reprodu

Management of obesity--the clinical nutritionist's experience.

Overweight patients are common in veterinary medicine, just as they are in human medicine. Although animals also suffer from diseases in the general categories of cancer, hypertension, diabetes, and digestive diseases, many of the specific problems of obese humans do not afflict obese pets. Of tumors, only adenocarcinoma of the breast is a significant problem in dogs and cats. Moreover, a high intake of dietary fat and table food has been reported to be protective in adult dogs; in women, increasing dietary fat has been associated with increased breast cancer risk. Two experimental studies in

Morbid obesity: problems associated with operative management.

A review of the problems associated with extensive jejunoileal bypass for morbid obesity in a series of 175 carefully selected patients is presented. Five postoperative deaths occurred (3%). Nonfatal complications occurred in 21%, with wound infections (14 patients) being the most common. Good results marked by weight reduction to the range of ideal weight without significant electrolyte or metabolic aberrations was observed in 82% of the patients receiving the current dimensional modificatiom of end-to-end jejunoileal bypass (30 cm to 20cm). An additional 13% had fair results and only 5% had

[Changes in obesity management: The clinician's perspective].

Obesity now affects nearly one in five French people, and its prevalence has more than doubled since 1997. It is defined as a chronic, progressive disease responsible for numerous mechanical complications (osteoarthritis, low back pain, incontinence, gastroesophageal reflux disease), metabolic complications (diabetes, fatty liver disease, cardiovascular disease), and psychosocial complications (anxiety, depression, stigmatization), as well as an increased risk of numerous cancers. A new definition, developed in 2025, distinguishes preclinical obesity from clinical obesity by incorporating the

Obesity and hypertension new insights in management.

PURPOSE OF REVIEW: Obesity-associated hypertension is a pressing and ever-growing public health concern. The prevalence of obesity has increased four-fold over the four preceding decades, with concomitantly rising rates of hypertension not far behind. Importantly, the interplay between these conditions exacerbates cardiovascular disease (CVD) risk, and optimal management strategies remain an evolving challenge. This review synthesizes recent advancements in understanding obesity-associated hypertension pathophysiology and explores emerging therapeutic options, highlighting their relevance in s

Management of obesity in adult Asian Indians.

The prevalence of obesity in India is increasing and ranges from 8% to 38% in rural and 13% to 50% in urban areas. Obesity is a risk factor for development of type 2 diabetes mellitus (T2DM), hypertension, dyslipidemia, coronary heart disease and many cancers. In Asian Indians excess abdominal and hepatic fat is associated with increased risk for T2DM and cardiovascular disease. There is higher risk for development of obesity related non-communicable diseases at lower body mass index levels, compared to white Caucasians. Despite being a commonly encountered medical problem, obesity poses chall
