Massive plexiform neurofibroma with associated meningo-encephalocoele and occipital bone defect presenting as a cervical mass.

The commonest skull manifestations in neurofibromatosis involve the orbit, with very few reports about occipital defects. We report a rare case of a 54-year-old lady with a massive plexiform neurofibroma extending from the auricular region down her left neck and into her shoulder, with an associated large left occipital and left petrous bone defect and extensive cerebellar meningo-encephalocoele, which presented with a relatively asymptomatic cervical mass and was treated with resection of the neurofibroma and advancement and rotational skin flaps.

Rotational vertebral artery occlusion from occipital bone anomaly: a rare cause of embolic stroke. Case report.

Vertebral artery (VA) occlusion by rotation of the head is uncommon, but can result from mechanical compression of the artery, trauma, or atlantoaxial instability. Occipital bone anomalies rarely cause rotational VA occlusion, and patients with nontraumatic intermittent occlusion of the VA usually present with compromised vertebrobasilar flow. A 34-year-old man suffered three embolic strokes in the vertebrobasilar system within 2 months. Magnetic resonance imaging demonstrated multiple infarcts in the vertebrobasilar territory. Angiography performed immediately after the third attack displayed

Atlanto-axial approach for cervical myelography in a Thoroughbred horse with complete fusion of the atlanto-occipital bones.

A 2-year-old Thoroughbred gelding with clinical signs localized to the first 6 spinal cord segments (C1 to C6) had complete fusion of the atlanto-occipital bones which precluded performing a routine myelogram. An ultrasound-assisted myelogram at the intervertebral space between the atlas and axis was successfully done and identified a marked extradural compressive myelopathy at the level of the atlas and axis, and axis and third cervical vertebrae.

Soft tissue Reconstruction Using Pedicled Trapezius Musculocutaneous Flap for Cranial Bone Flap Infection in the Occipital Region.

When managing cranial bone flap infections, infected bone flaps are typically removed and subsequently replaced with artificial bones 6 to 12 months after the inflammation subsides. However, defects in the occipital region pose challenges due to concerns regarding brain protection when patients lie in the supine position. Herein, the authors report the case of a 73-year-old woman with an occipital bone flap infection, which was successfully managed by reconstruction with a trapezius musculocutaneous flap immediately after removing the infected bone flap. One year and 2 months postoperatively,

Aggressive evolution of an occipital bone ABC-like lesion with dural invasion and intradural extension: a case report.

BACKGROUND: Aneurysmal bone cysts (ABCs) of the skull are rare, particularly those arising in the occipital bone. Intracranial extension or dural destruction is extremely uncommon. Here, we report an occipital ABC-like lesion that recurred 19 months after gross-total resection with dural defects, intradural extension, and molecular evidence supporting aggressive neoplastic evolution. CASE DESCRIPTION: A 37-year-old woman presented with 2 months of headache and 1 month of binocular diplopia. CT and MRI demonstrated a large expansile lytic lesion of the occipital bone with poorly defined interf

Assessment of differences in mechanical properties and thickness of temporal and occipital bones and correlation with age in Japanese forensic samples.

The mechanical properties and thickness of adult temporal and occipital bones were examined in modern Japanese forensic samples. Cranial bones were obtained from 293 Japanese corpses (179 men and 114 women). During autopsy, left temporal (LT), right temporal (RT), and occipital (O) bone samples were extracted from each skull. Sample thickness (ST) was measured using multidetector computed tomographic imaging. The fracture load (FL) of each sample was measured by a bending test, in which the flexural strength (FS) was calculated. The FL and ST values for O were significantly greater compared wi

Morphology of the Occipital Bones and Foramen Magnum Resulting From Premature Minor Suture Fusion in Crouzon Syndrome.

To identify skull-base growth patterns in Crouzon syndrome, we hypothesized premature minor suture fusion restricts occipital bone development, secondarily limiting foramen magnum expansion. Skull-base suture closure degree and cephalometric measurements were retrospectively studied using preoperative computed tomography (CT) scans and multiple linear regression analysis. Evaluation of multi-institutional CT images and 3D reconstructions from Wake Forest's Craniofacial Imaging Database (WFCID). Sixty preoperative patients with Crouzon syndrome under 12 years-old were selected from WFCID. The c

MRI-based quantification of primary ossification centers in the lateral and basilar parts of the human fetal occipital bone.

BACKGROUND: The ossification of the occipital bone begins in the basilar part and progresses anteriorly, which is vital for the normal development of both the cranial and facial skeleton. Abnormal ossification of the occipital bone is associated with congenital anomalies such as Chiari malformation and anencephaly. Despite this, morphometric data on the ossification centers of the occipital bone, particularly in the lateral and basilar parts, remain limited. PURPOSE: The aim of this study was to provide a detailed morphometric analysis of the primary ossification centers in the lateral and ba

Resolution of a chronic occipital wound with exposed skull bone with a fish skin graft: a successful treatment approach.

Chronic skin defects in the head, face and neck pose challenges for closure, especially after multiple surgeries or radiation therapy. We report the case of a woman in her 70s with a chronic occipital wound following squamous cell carcinoma resections, resulting in exposed skull bone. Despite various options, we successfully treated the 4 cm x 5 cm wound with a Kerecis fish skin graft (FSG), observing significant improvement within a week. The FSG promoted granulation tissue formation, enabling subsequent full-thickness skin grafting from the patient's groin. Complete wound closure was achieve

Pathogenesis and treatment of a giant occipital bone defect with meningoencephalocele in an NF1 child: case report and review of the literature.

Autosomal dominantly inherited neurofibromatosis type I (NF1) is a systemic disorder caused by a mutation of a gene on chromosome 17q11.2 and characterized by multiple café-au-lait spots, lentiginous macules, Lisch nodules of the iris, and tumors of the nervous system. Bony manifestations such as scoliosis, dysplasia of the greater sphenoidal wing, tibial pseudoarthrosis, short stature, and macrocephaly have been reported in approximately 50% of patients. However, calvarial bone defects are rare. After screening 324 articles, 23 cases (12 adult and 11 pediatric patients) of occipital bone defe

Parathyroid adenoma diagnosed on the basis of a giant cell tumor of parieto-occipital region and multifocal bone injuries.

Brown tumors are rare skeletal manifestations of hyperparathyroidism (HPT) that may mimic cancer metastases. Histopathologically, they are difficult to differentiate from other giant cell lesions. A case is presented of 41-year-old woman with giant cell tumor in parieto-occipital region with injury of external bone lamina, growing into the skull cavity. The mass was suspected of being neoplastic. Numerous osteolytic lesions in the skull skeleton and multifocal bone injuries were observed, also. Elevation in calcium (5.91 mEq/L) and parathormone (1188 ng/mL) concentrations and hypercalciuria (5

Occipital Bone Erosion Caused by C1 Lateral Mass Screw-Rod Construct Used for C1/2 Arthrodesis: A Case Report.

C1 lateral mass screw (C1LMS) is used as a standard implant for fixation of the upper cervical spine. The most significant benefit of the fixation using C1LMS is that it can obtain an excellent mechanical stability immediately after surgery without a massive structural bone graft and strict orthosis. Meanwhile, several studies have already referred to complications, which have not been seen in conventional devices. As for one of the complications, occipital bone erosion (OcBE) induced by repetitive impingement between the occipital bone and the protruded metal implant on the C1 posterior has b

Late complication of surgically treated atlantoaxial instability: occipital bone erosion induced by protruded fixed titanium rod: a case report.

OBJECTIVE: Polyaxial screw-rod fixation of C1-C2 is a relatively new technique to treat atlantoaxial instability, and there have been few reports in the literature outlining all possible complications. The purpose of this case report is to present the occurrence and management of occipital bone erosion induced by the protruded rostral part of a posterior atlantoaxial screw-rod construct causing headache. CLINICAL FEATURES: A 70-year-old Asian man with rheumatoid arthritis initially presented to our institution with atlantoaxial instability causing progressive quadraparesis and neck pain. INT

Atlanto-occipital assimilation: the role of ex-, basi-, and suboccipital bones in the physiopathology of type I basilar invagination. Patient series.

BACKGROUND: Atlas assimilation is the most common congenital anomaly of the craniocervical junction and is consistently associated with type I basilar invagination (BI1). Its precise bone composition and distribution, however, remain poorly defined. OBSERVATIONS: All BI1 patients demonstrated atlas assimilation, which was absent in controls. The condyle-mass complex was the most consistent pattern, forming a continuous monoblock in all cases. Atlas anterior arch-basion fusion was universal but asymmetrical in 2 patients, while posterior arch fusion was complete in 5 patients and partial in 3.

Occipital bone erosion induced by C1 pedicle screw as a late complication of atlantoaxial fixation: a case report and literature review.

PURPOSE: The objective of this case report is to highlight occipital bone erosion as an unusual late complication of C1-C2 instrumented fixation. CASE PRESENTATION: A 60-year-old man of a displaced Anderson type II odontoid fracture was surgically treated by C1-C2 pedicle screw fixation. Occipital bone erosions, caused by a repeat irritation of the end of rod to the occipital bone, were detected on multiplane reconstructed computed tomography at 3 months after surgery. The lesion progressed over time with increasing the C2 anteversion on radiological evaluations. Eventually, the bony shell ha

Occipital Bone Angiomatosis and Chronic Subdural Hematoma Causing Severe Cerebellar and Brainstem Compression and Obstructive Hydrocephalus in a Young Adult Cat.

A 2-year-old cat was presented for a 1-month history of cautious ambulation, head shaking, and lethargy, with severe spinal pain on neurologic examination. MRI and CT demonstrated marked endophytic thickening of the occipital bone diploe with widened intertrabecular spaces, irregular internal cortical margins, relative preservation of the outer cortex, and strong internal and perilesional contrast enhancement. Histopathology revealed an anomalous blood vessel proliferation consistent with angiomatosis. An associated chronic subdural hematoma caused severe cerebellar and brainstem compression,
