In Part II, this paper addresses ocular motions, their causes (forces), and the governing laws, beginning with the fundamental question: Why do the eyes move? Ocular rotations and different types of translations (ocular, orbital, and corporeal) are reviewed. The discussion then turns to how the eyes move, where concepts such as the plane of muscular action, torque, and arc of contact provide possible explanations for the anatomical arrangement of the ocular muscles within the orbit. Sherrington's law of reciprocal innervation is used to explain the distribution of muscular active forces in a c

A 54-year-old man developed an acquired monocular elevation paresis. Forced lid closure or oculocephalic reflexes failed to elevate the affected eye in contrast to the contralateral eye, which turned fully upward using either maneuver. A mass lesion compressing the lateral aspect of the right oculomotor nerve at the exit of the brainstem was found. The monocular elevation paresis in this patient reflects damage of the oculomotor nerve fascicles that supply the inferior oblique and superior rectus muscles. This case supports the accepted topographical fascicular arrangement of the oculomotor ne

The pattern of innervation of the extraocular muscles is highly conserved across higher vertebrate species and mediates sophisticated visuomotor processes. Defects in oculomotor development often lead to strabismus, a misalignment of the eyes that can cause partial blindness. Although it has been intensively studied from a clinical perspective, relatively little is known about how the system develops embryonically. We have therefore mapped the development of the oculomotor nerve (OMN) in chick embryos by using confocal microscopy. We show that OMN development follows a series of stereotyped st

Oculomotor disturbance resulting from orbital floor fractures have different etiologic factors, sometimes damage of one of the ocular motor nerves, caused by direct injury to the orbit; this damage occurs also to one or more of the extrinsic ocular muscles, especially the obliques; frequently, the diplopia is caused by prolapsed orbital tissues with or without muscle entrapment or by a muscle fibrosis; when the diplopia appears after orbital floor reconstruction there is often a palsy of the inferior rectus muscle in front of silicone implant or bone graft on the orbital floor. PURPOSE: The etiology of third nerve palsy is usually diagnosed by history, motility examination, and presence of lid and pupil involvement, as well as cranial and vascular imaging. We used high-resolution magnetic resonance imaging (hrMRI) of the oculomotor nerve and affected extraocular muscles (EOMs) to investigate oculomotor palsy.
DESIGN: Prospective, noncomparative, observational case series in an academic referral setting.
METHODS: Twelve patients with nonaneurysmal oculomotor palsy of 0.75 to 252 months' duration were studied. In the orbit and along the intracranial oculomotor nerve,

The article describes the results of electromyographic examination of the muscles of the neck in 26 patients with spasmodic torticollis. Electromyography was performed before and after stereotaxic operative interventions on the nucleus of Cajal or in combination with the diencephalic structures. It is shown that electromyographic examination of patients, especially with the use of functional motor and oculomotor tests, widens essentially the possibilities of the diagnosis by facilitating the identification or specification of the predominantly involved group of muscles. The discovery of concur

Twenty-six patients with oculomotor nerve palsy due to cerebral aneurysms were examined. There were six males and twenty females with a mean age of 55 years. 25 of the 26 aneurysms were located at the junction of the internal carotid and the posterior communicating artery and one was at the junction of the basilar artery and the superior cerebellar artery. Twelve patients had associated subarachnoid hemorrhage (SAH); the other 14 did not. The initial symptoms in many patients were ptosis and double vision. Twenty-one of the patients had total oculomotor nerve palsy, one had a sparing of medial

Neurogenic ptosis refers to upper eyelid drooping resulting from disrupted innervation of the eyelid retractor muscles. The differential diagnoses of neurogenic ptosis include oculomotor nerve palsy, Horner's syndrome, and neuromuscular junction disorders. Oculomotor nerve palsy and Horner's syndrome result from damage to the oculomotor and oculosympathetic pathways, respectively. The oculomotor nerve pathway has four segments: intraaxial, subarachnoid, cavernous, and orbital. The oculosympathetic pathway consists of three orders of neurons extending from the base of the skull to the upper che

The fatal disease amyotrophic lateral sclerosis (ALS) is characterized by the loss of somatic motor neurons leading to muscle wasting and paralysis. However, motor neurons in the oculomotor nucleus, controlling eye movement, are for unknown reasons spared. We found that insulin-like growth factor 2 (IGF-2) was maintained in oculomotor neurons in ALS and thus could play a role in oculomotor resistance in this disease. We also showed that IGF-1 receptor (IGF-1R), which mediates survival pathways upon IGF binding, was highly expressed in oculomotor neurons and on extraocular muscle endplate. The

Simple, complex or syndromic craniosynostosis may be responsible for ocular and especially oculomotor pathologies. Among simple craniosynostosis, anterior plagiocephaly is the most frequently associated with oculomotor disorders. Oculomotor disorders encountered in craniosynostosis are specific to this pathology. They may be related to orbital deformities or oculomotor muscle malformations. Early craniofacial surgery reduces the onset and severity of these oculomotor disorders which is very important for ophtalmological patient care. Indeed, these oculomotor disorders are difficult to treat fo

PURPOSE: To describe a patient with unusual findings after regeneration of the oculomotor nerve.
METHODS: Case report.
RESULTS: A 35-year-old woman developed complete right third nerve paralysis after neurosurgical ligation of internal carotid-posterior communicating and internal carotid-ophthalmic artery aneurysms. Permanent ipsilateral lagophthalmos appeared as third nerve function spontaneously recovered.
CONCLUSION: Lagophthalmos may rarely develop after aberrant regeneration of the oculomotor nerve, presumably caused by co-contraction of the levator and superior rectus muscles during t

The interdependence of the development of the eye and oculomotor system during embryogenesis is currently unclear. The occurrence of clinical anophthalmia, where the globe fails to develop, permits us to study the effects this has on the development of the complex neuromuscular system controlling eye movements. In this study, we use very high-resolution T2-weighted imaging in five anophthalmic subjects to visualize the extraocular muscles and the cranial nerves that innervate them. The subjects differed in the presence or absence of the optic nerve, the abducens nerve, and the extraocular musc

Based on 146 cases of basicranial aneurysm, the eye manifestations were pain in the eyes, diplopia, ptosis, and visual impairment. In cranial nerve paralysis, the oculomotor nerve was the most vulnerable, followed by the abducent nerve. The trochlear and trigeminal nerves were the least involved. In oculomotor nerve palsy, the levator muscle of the upper lid and the pupil muscles were first affected. Visual fields of typical bitemporal hemianopsia were not seen in cases of aneurysms near the optic chiasma.

After just a clinical examination, the experienced neurologist can assign specific symptoms quite precisely to distinct lesions within the brain and other parts of the nervous system, on the basis of his neuroanatomical knowledge. This also holds true for lesions affecting the oculomotor system. The aim of this article is to give a comprehensive overview of the neuroanatomical basis of the oculomotor system, in order to facilitate the precise spatial assignment of potential lesions affecting the control of eye movements. After a brief introduction, the components of the system are discussed, i

UNLABELLED: A case of partial oculomotor palsy due to midbrain infarction is reported.
CASE PRESENTATION: Fifty-one-year old man noted a sudden onset of double vision. He transiently presented right hemiparesis, right hemihyposthenia and cerebellar ataxia. The main symptom was the left oculomotor palsy selectively involving extraocular muscles (levator and pupil sparing), lasting for more than 6 months. The CT scan showed localized and well demarcated low-density areas at the left tegmentum of midbrain and left anteromedial thalamus, diagnosed as lacunar infarction due to occlusion of paramed

This paper aims to present various aspects of the anatomy of horizontal (i.e., lateral and medial) rectus muscles. It mainly compares morphology and detailed patterns of intramuscular innervation of those muscles. It is also one of the first reports that uses the Sihler's stain to examine human extraocular muscles. The study was conducted on 80 isolated cadaveric hemi-heads. Sihler technique of nerves staining served to expose the course of intramuscular branches of the oculomotor and abducens nerves. The lateral rectus was longer (48 mm versus 46.5 mm) and more distant from the limbus (6.8 mm

A patient with infarction of the paramedian part of the lower midbrain on the right side is described, in whom internuclear ophthalmoplegia and bilateral ptosis were associated with limitation of elevation of the contralateral eye from paresis of the superior rectus. Supranuclear paresis was suggested by partial dissociation between phasic and tonic components of upward gaze. It is suggested that the right-sided lesion involved the rostral median longitudinal fasciculus before it reached the oculomotor nucleus, the central caudal nucleus medially situated in the lower end of the oculomotor nuc

Accessory rectus muscles have rarely been reported as muscular 'bands' or 'slips' originating from the common tendinous ring (annulus of Zinn) and inserting in atypical location. This group of muscles is innervated by the inferior branch of the oculomotor nerve, lies on lateral side of the optic nerve and inserts in rectus muscles. Since there are only few descriptions of such unusual findings in the medical literature, the anatomical data on accessory rectus muscles is limited. Furthermore, existing reports vary in terms of studied objects (cadavers or living subjects), medical history (absen
