1. Conventional manual measurement of blood pressure (BP) in clinical practice is no longer considered to be the best method for evaluating a patient's BP status. Home BP and 24 h ambulatory BP monitoring are now recommended for the diagnosis and management of hypertension. 2. Recent studies provide an alternative to conventional office BP, namely automated office (AO) BP, which involves multiple BP readings taken with a fully automated device with the patient resting quietly alone. Automated office BP is preferable to routine manual office BP in that it exhibits improved accuracy and a stron

BACKGROUND AND OBJECTIVES: Precise BP measurement to exclude hypertension is critical in evaluating potential living kidney donors. Ambulatory BP monitoring is considered the gold standard method for diagnosing hypertension, but it is cumbersome to perform. We sought to determine whether lower BP cutoffs using office and automated BP would reduce the rate of missed hypertension in potential living donors.
DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: We measured BP in 578 prospective donors using three modalities: (1) single office BP, (2) office automated BP (average of five consecutive aut

Clinical practice guidelines have traditionally recommended manual blood pressure (BP) measurement in the office setting as the standard method for diagnosing hypertension. In reality, manual BP in routine clinical practice is relatively inaccurate, over-diagnoses hypertension by provoking office-induced increases in BP and correlates poorly with both the awake ambulatory BP and target organ damage. The most recent guidelines recommend 24-h ambulatory BP and home BP for diagnosing hypertension. The advent of automated office BP (AOBP) represents a third alternative to conventional manual BP me

This study was a preliminary exploration of gynandromorphophilia, that is, sexual interest in cross-dressed or anatomically feminized men. Subjects were male subscribers to a voice mail system devoted to personal advertisements for sexual or romantic partners. These comprised 51 gynandromorphophiles who sought cross-dressers, transvestites, transsexuals, or she-males for such relationships, 37 gynandromorphophilic cross-dressers who identified themselves as cross-dressers and sought similar men, and 31 residual cross-dressers who sought masculine or unspecified male partners. During the past 15 years, clinical outcome studies have consistently reported that home and 24-hour ambulatory blood pressure recordings provide a significantly better measure of cardiovascular risk than do manual blood pressure readings taken in the office or clinic. The advent of automated sphygmomanometers that record blood pressure with the patient alone in the examining room will be the next major change in our approach to recording blood pressure. These automated devices virtually eliminate the white coat response and their readings correlate significantly better with the ambulatory bloo

Refractory hypertension is a recently described phenotype of antihypertensive treatment failure defined as uncontrolled blood pressure (BP) despite the use of ≥5 different antihypertensive agents, including chlorthalidone and spironolactone. Recent studies indicate that refractory hypertension is uncommon, with a prevalence of ≈5% to 10% of patients referred to a hypertension specialty clinic for uncontrolled hypertension. The prevalence of white-coat effect, that is, uncontrolled automated office BP ≥135/85 mm Hg and controlled out-of-office BP <135/85 mm Hg, by awake ambulatory BP monitor in

BACKGROUND: Regular physical activity has been highlighted as an important factor in decreasing about 25 factors harmful to health, such as obesity, diabetes, heart failure, depression and osteoporosis. Female office workers, because of the long hours they stay seated in their workplace, are seriously threatened by immobility. The Theory of Planned Behavior has been successfully used several times to predict PA behaviorphysical activity (PA). The TPB studies also have identified perceived behavioral control as the key determinant of walking intentions.

This study examined the dose-response characteristics of aprocitentan, a dual endothelin A/endothelin B receptor antagonist, in patients with essential hypertension. In a randomized, double-blind, parallel study design, eligible patients with a sitting diastolic blood pressure (BP) of 90-109 mm Hg received aprocitentan 5, 10, 25, or 50 mg, placebo, or lisinopril 20 mg as a positive control once daily for 8 weeks. Multiple automated office BP readings were obtained with patients resting unattended (unattended automated office BP) at baseline, weeks 2, 4, and 8. Ambulatory BP was monitored for 2

Factors related to the physician/nurse and patient and their interaction are potential sources of error in manual office blood pressure (MOBP). The use of automated sphygmomanometers to record blood pressure (BP) with the patient alone reduces measurement error and minimizes anxiety-related increases in BP, thus eliminating the "white-coat" response. Comparative studies have shown the cut-point for a normal automated office BP (AOBP), awake ambulatory BP, and home BP (<135/85 mmHg) to be similar, providing the patient does not rest for a prolonged period before the first AOBP reading, as recom

Hypertension is a potent risk factor for cardiovascular morbidity and mortality. High blood pressure (BP) correlates closely with all-cause and cardiovascular mortality. Although the gold standard remains office BP (auscultatory or automated), other methods (central or out-of-office) are gaining popularity as better predictors of CV events. In this review, we investigated the prognostic value of each method of BP measurement and explored their advantages and pitfalls. Unattended automated office BP is a novel technique of BP measurement with promising data. Ambulatory BP monitoring, and to a l

In the present review, we summarized the blood pressure (BP) measurement protocols of contemporary outcome trials in hypertension. In all these trials, clinic BP was used for the diagnosis and therapeutic monitoring of hypertension. In most trials, BP was measured in the sitting position with mercury sphygmomanometers or automated electronic BP monitors by trained observers. BP readings were taken on each occasion at least twice with a 30-to-60-s interval after 5 min of rest. Details regarding the arm side, cuff size, and the timing of BP measurement were infrequently reported. If clinic BP co

The incidence of patients who present to the office with arrhythmia and hemodynamic instability is unknown. Emergency medical systems data, based on ambulance runs, are available only for patients who have had a cardiac arrest. When faced with an unstable or potentially unstable patient, however, we must be prepared to act quickly, safely, and accurately. This article addresses the general approach to such a patient; provides necessary information on office emergency preparation, including training, rapid response team protocol, and the use of automated external defibrillators; and addresses t

PURPOSE OF REVIEW: For the accurate diagnosis and management of hypertension, out-of-office blood pressure evaluation using ambulatory (ABPM) or home monitoring (HBPM) is currently recommended. In children, there is considerable evidence on the clinical utility of ABPM, whereas the evidence on HBPM is limited. This systematic review presents (i) the benefits of HBPM in children; (ii) the evidence on normal range, diagnostic accuracy, and relationship with preclinical organ damage; and (iii) guidance for devices, monitoring schedule, and interpretation.
RECENT FINDINGS: HBPM is a useful adjunc

Three main methods of blood pressure (BP) measurement are routinely used: office-based BP measures (OBPM), 24-hour ambulatory BP measures (ABPM), and home BP measures (HBPM). OBPM can lack precision, ABPM gives exhaustive information but is not comfortable, HBPM requires a device at home and the result is not immediate. The automated (unattended) office blood pressure measurement (AOBP) is a more recent method, simple to implement in the physician's office, permitting to largely avoid the white coat effect. The result is immediate and the readings are similar to those obtained by ABPM, the ref

BACKGROUND: Auscultatory measurement of office blood pressure (BP) by mercury sphygmomanometers (AuscBPM) is still the gold standard in clinical trials and registration studies for antihypertensive drugs. The increasing availability of accurate automated oscillometric BP measuring devices has offered new perspectives in this field, although their usefulness in drug studies has not been systematically tested yet.
METHODS: During the course of Valsartan 320 mg EU Registration Study we used an electronic automated oscillometric BP measuring device (eBPM) as an alternative to conventional AuscBPM

The objective of this study was to compare serial readings from an in-pharmacy automated blood pressure (BP) kiosk to mean daytime ambulatory BP. A total of 100 community-dwelling adults with hypertension underwent (1) three baseline automated office readings; (2) three in-pharmacy readings on each of four visits (12 total) using the PharmaSmart PS-2000 kiosk; and (3) 24-hour ambulatory BP monitoring between in-pharmacy visits two and three. Paired t-tests, Bland-Altman plots, and Pearson correlation coefficients were used for analysis. Mean BPs were 137.8 ± 13.7/81.9 ± 12.2 mm Hg for in-pharm

BACKGROUND: Patients often exhibit higher blood pressure (BP) readings in the doctor's office, a phenomenon known as the white coat effect. This study examines the presence of a physician in the examining room as a possible factor in provoking a white coat effect.
METHODS: Blood pressure measurements taken by an automated BP recording device, the BpTRU (VSM MedTech Ltd., Vancouver, BC, Canada) with the patient alone in the examining room, were compared with the following: (1) BP taken by the patient's family physician; (2) BP taken on the first visit to a hypertension specialist; (3) BP measu

BACKGROUND: Similar to adults, in children automated cuff devices are recommended for office, home, and ambulatory blood pressure (BP) measurement. However, a device validated in adults may not be accurate in children, and separate investigation is necessary. Moreover, current pediatric guidelines recommend hypertension detected by an automated device to be confirmed using auscultation. The evidence on validated automated BP devices for children (aged 3-12 years) is presented.
METHODS: STRIDE BP (www.stridebp.org) performs periodic systematic PubMed searches for validation studies of automate

Results obtained at the University of California in San Francisco with the Remler M-2000, patient-activated, semi-automatic portable blood pressure recorder were reviewed. The automated recordings were accurate and reproducible in comparison with simultaneous readings obtained with a conventional sphygmomanometer. Daytime pressures in ambulatory patients were correlated reasonably well with office blood pressures (r = 0.63-0.69), but in 80% of the patients ambulatory blood pressures were lower than office blood pressures, and the disparity between the two tended to increase as office blood pre

Modern home automation systems include features that enhance security, such as cameras and radars. This paper proposes an innovative home security system that can detect burglars by analyzing acoustic signals and instantly notifying the authorized person(s). The system architecture incorporates the concept of the Internet of Things (IoT), resulting in a network and a user-friendly system. The proposed system uses an adaptive detection algorithm, namely the "short-time-average through long-time-average" algorithm. The proposed algorithm is implemented by an IoT device (Arduino Duo) to detect pe
