Clinical Risk and Regulatory Oversight for Office-Based Cosmetic Procedures: A State-By-State Analysis.

BACKGROUND: Office-based cosmetic procedures are increasingly delegated by physicians to nonphysicians across a range of practice settings. These procedures vary substantially in technical complexity and complication risk. OBJECTIVE: To determine whether state-level scope-of-practice, delegation, and supervision standards for office-based cosmetic procedures scale with procedural risk. MATERIALS AND METHODS: Statutes, administrative codes, and medical, nursing, and cosmetology board guidance were reviewed for all 50 US states and the District of Columbia. Five office-based cosmetic procedure

The lifecycle of a grant: Award acceptance to dissemination.

Securing grant funding is an important achievement for nurse practitioners and academic faculty, yet the postaward phase introduces complex administrative, regulatory, and scholarly responsibilities essential to project success. This article is the second in a two-part series examining the lifecycle of a grant. Building on part 1, which focused on preaward planning and proposal development, this manuscript addresses postaward activities from award acceptance through dissemination. Drawing on the authors' shared experience as principal investigators and project directors, the article provides p

Prevalence of white coat effect in treated hypertensive patients in the community.

Patients receiving drug therapy for hypertension in the tertiary care setting frequently exhibit higher office readings compared to ambulatory blood pressure values (white coat effect). In this study, the prevalence of a white coat effect was determined in an unselected population of 147 hypertensive patients receiving treatment from their family physicians in the community. The proportion of patients with a white coat effect (defined as office - ambulatory blood pressure > or = 20/10 mm Hg) was significantly (P < .001) higher when based upon the family physician's routine blood pressure readi

Refractive cataract surgery: office systems to improve results.

As we approach the second millennium, state-of-the-art cataract surgery means not only improved corrected vision, but also the opportunity to create better uncorrected distance and near visual acuity for patients by taking advantage of new technology and techniques. Patient satisfaction will ultimately depend on our ability and success in effectively meeting the challenge to reduce the need for spectacles after cataract surgery. A systemic and coordinated effort by various members of the office staff and the surgeon to monitor the results of IOL calculations will enhance the overall success fo

Self-sufficiency in the healthcare workforce: a system dynamics model of the domestic and foreign educated nursing and midwifery workforce in Ireland.

BACKGROUND: World Health Organization (WHO) projections point to an increasing global demand for nurses and midwives, leading to shortages in many countries, particularly in less developed regions. Ireland, the context for this study, currently relies heavily on foreign educated nurses and midwives to meet its demand, with Government policy moving towards a domestic recruitment model. This paper estimates the recruitment requirement and associated nursing and midwifery student intake over time under different reform scenarios. It also highlights policy considerations for countries, like Irelan

The Limping Child: Could It Be Lyme Arthritis? Information and Planning for School Nurses.

Lyme disease is the most common vector-borne disease in the United States with Lyme arthritis becoming one of the most common symptoms of Lyme disease. It is imperative that school nurses become familiar with vector-borne disease and promptly consider Lyme arthritis to be contemplated as a differential diagnosis of a child presenting to the office with a limp and joint pain and swelling with no history of acute injury. The following article will discuss the overview, implications and management of Lyme arthritis in the school setting and includes a relevant IHP for school nurses to utilize in

Back pain: a case study.

Musculoskeletal complaints, especially back pain without trauma, are frequent health problems seen by nurse practitioners in community emergency centers and office settings. Back pain can be a symptom of serious health problems. This article presents the case of a Caucasian male in his early sixties who reported sudden onset of back pain after pushing a heavy object. Careful clinical assessment led the nurse practitioner with the collaborating physician to pursue diagnostic tests, which revealed thoracic and abdominal aortic aneurysms.

"Missing in Action" in Psychiatric Nursing History: The Role of Chief Nurse Adele S. Poston and Her Band of Nurses During World War I.

This is the first nursing journal article to introduce the pioneering work of American psychiatric nurse leader, Adele S. Poston. Poston supervised a team of nurses as they cared for soldiers serving with the American Expeditionary Forces during World War I in France. Poston and her nurses worked in the first American specialized neuropsychiatric hospital in a war. The soldiers they treated primarily had functional nervous disorders described at that time as "shell shock" or "war neuroses." The traumatized officers and enlisted men were considered capable of being cured and returned to active

Bedbugs: an equal opportunist and cosmopolitan creature.

Currently there is a rise in bedbugs seen in schools. The wingless, blood-sucking insects can arrive at school via student book bags, student clothing, or other personal items. If a student presents in the health office with a bite mark, the school nurse will need to differentiate between a bedbug bite and other insect bites, such as fleas, spiders, and scabies in order to provide the most accurate and appropriate intervention measures. This article will further provide school nurses with information about the etiology of the bedbug, mode of transmission, and eradication options for the school

Gilbert syndrome.

Two or three times a year, a healthy patient (> 1 year old) with mild unconjugated hyperbilirubinemia as the only abnormality on routine liver function testing will be referred to the gastroenterologist. A diagnosis of Gilbert Syndrome (GS) is made, and the patient may or may not be seen in the clinic or office because GS is a benign condition. Either way, the nurse needs to understand the pathophysiology of this condition so he/she can provide necessary patient/family education. The purpose of this article is to review the existing literature on GS and to summarize it in a manner useful to bo

From the field: a maternal-child health nursing competence validation model.

This group of Vermont community health nurses from different agencies collaborated to develop a competence validation framework for maternal and child health nursing in the practice areas of perinatal client teaching, breastfeeding, and prenatal, postpartum, and newborn nursing care. The framework is based on the work of Benner, using the "competent" level of nursing practice, and delineates three parameters of competence: technical skills, interpersonal skills, and critical thinking skills. Learning resource materials, including newborn and maternal assessment guidelines, were developed for e

[Legionellae in dental units--a hygienic risk?].

Water samples were collected from the hot water systems and from dental units in 12 dental offices of Dresden and cultured for legionellae. From seven (58%) hot water supplies and from dental units in 6 (50%) offices legionellae could be isolated. To asses the hygienic risk for dental staff we studied the prevalence of antibodies against Legionellae in serum samples of 113 dentists, 105 dental nurses and 17 dental technicians. In comparison with a control group of healthy people (132 women, 161 men, 20-80 years) living in the Dresden area, dentists have had a higher prevalence of antibodies ag
