From Negative Duplex to Phlegmasia in Minutes: Bedside POCUS Identifies Rapid Thrombosis Unmasking Underlying May-Thurner Syndrome.

May-Thurner syndrome occurs from an anatomical variant in which the left common iliac vein is compressed against the lumbar spine by the overlying right common iliac artery. This often leads to chronic venous stasis predisposing individuals to left-sided iliofemoral deep venous thrombosis (DVT). Progression to phlegmasia cerulea dolens (PCD) is rare, but potentially limb-threatening. Here we describe rapid formation of DVT leading to PCD first discovered during a bedside point-of-care ultrasound (POCUS). A 50-year-old female presented to the emergency department (ED) with left leg swelling fol

The relationship between intraoperative teamwork and management skills in patient care.

BACKGROUND: Optimal team performance in the operating room (OR) requires a combination of interactions among OR professionals and adherence to clinical guidelines. Theoretically, it is possible that OR teams could communicate very well but fail to follow acceptable standards of patient care and vice versa. OR simulations offer an ideal research environment to study this relationship. The goal of this study was to determine the relationship between ratings of OR teamwork and communication with adherence to patient care guidelines in a simulated scenarios of malignant hyperthermia (MH). Work systems analysis of sterile processing: assembly.

BACKGROUND: Sterile processing departments (SPDs) play a crucial role in surgical safety and efficiency. SPDs clean instruments to remove contaminants (decontamination), inspect and reorganise instruments into their correct trays (assembly), then sterilise and store instruments for future use (sterilisation and storage). However, broken, missing or inappropriately cleaned instruments are a frequent problem for surgical teams. These issues should be identified and corrected during the assembly phase. OBJECTIVE: A work systems analysis, framed within the Systems Engineering Initiative for Patie

A checklist for endonasal transsphenoidal anterior skull base surgery.

OBJECT Approximately 250 million surgical procedures are performed annually worldwide, and data suggest that major complications occur in 3%-17% of them. Many of these complications can be classified as avoidable, and previous studies have demonstrated that preoperative checklists improve operating room teamwork and decrease complication rates. Although the authors' institution has instituted a general preoperative "time-out" designed to streamline communication, flatten vertical authority gradients, and decrease procedural errors, there is no specific checklist for transnasal transsphenoidal

Current Status of Clinical 177Lu-PSMA Radiopharmaceutical Therapies: Insights from a Survey of High-End Nuclear Medicine Theranostic Centers.

Radiopharmaceutical therapy (RPT) is becoming a cornerstone of cancer treatments with the approval of 177Lu-PSMA (Pluvicto; Novartis), 177Lu-DOTATATE (Lutathera; Novartis), and 223Ra-dichloride (Xofigo; Bayer). An even larger number of treatments with different isotopes and biologic targets are being studied and promise a revolution in oncology care. However, the Food and Drug Administration's approval of 177Lu-PSMA RPT in the prechemotherapy space has raised concerns regarding access, logistics, supply, quality of care, and workforce challenges. Improved lymphatic mapping technique for breast cancer.

Breast sentinel lymph node biopsy is becoming more common. However, the best injection technique is not well established. Currently the gold standard is peritumoral injection. However, for upper outer quadrant tumors there is considerable axillary "shine through" which makes the identification of the radioactive sentinel lymph node difficult. We undertook a study to compare an injection in Sappey's subareolar plexus to the gold standard of peritumoral injection. Between December 1997 and March 1998, 85 patients with breast cancer were enrolled in the study. All patients were injected with 2 cc

A Work Systems Analysis of Sterile Processing: Sterilization and Case Cart Preparation.

Achieving reliable instrument reprocessing requires finding the right balance among cost, productivity, and safety. However, there have been few attempts to comprehensively examine sterile processing department (SPD) work systems. We considered an SPD as an example of a socio-technical system - where people, tools, technologies, the work environment, and the organization mutually interact - and applied work systems analysis (WSA) to provide a framework for future intervention and improvement. The study was conducted at two SPD facilities at a 700-bed academic medical center servicing 56 onsite

Pre-analytical effects of pneumatic tube transport on impedance platelet aggregometry.

Point-of-care platelet monitoring is increasingly used in cardiac patients treated with antiplatelet agents. The validity of a new assay needs to be evaluated not only for reproducible data in clinical samples, but also for other pre-analytical conditions that may influence measurements. The aim of this study was to evaluate the influence of a pneumatic tube system (PTS) for specimen transport on impedance platelet aggregometry. We evaluated 50 consecutive patients scheduled for coronary artery bypass surgery under oral therapy with 100 mg/d acetylsalicylic acid (aspirin). In each patient, thr

The impact of a multidisciplinary safety checklist on adverse procedural events during bedside bronchoscopy-guided percutaneous tracheostomy.

BACKGROUND: Bedside procedures are seldom subject to the same safety precautions as operating room (OR) procedures. Since July 2013, we have performed a multidisciplinary checklist before all bedside bronchoscopy-guided percutaneous tracheostomy insertions (BPTIs). We hypothesized that the implementation of this checklist before BPTI would decrease adverse procedural events. METHODS: A prospective study of all patients who underwent BPTI after checklist implementation (PostCL, 2013-2014, n = 63) at our Level I trauma center were compared to all patients (retrospectively reviewed historical co

Evaluation of accuracy of photogrammetry with 3D scanning and conventional impression method for craniomaxillofacial defects using a software analysis.

BACKGROUND: Facial mutilation and deformities can be caused by cancer, tumours, injuries, infections, and inherited or acquired deformities and has the potential to degrade one's quality of life by interfering with fundamental tasks like communication, breathing, feeding, and aesthetics. Depending on the type of defect, producing maxillofacial prostheses for the rehabilitation of patients with various defects can be challenging and complex. The prosthesis is used to replace missing or damaged parts of the cranium and face, like the nose, auricle, orbit, and surrounding tissues, as well as miss

Why and How Should Ethiopia Establish a Stem Cell Transplant Service? A Review Article.

Ethiopia is attempting to reduce cancer-related morbidity and mortality through a strategic national cancer control plan but according to Globocan 2020, hematologic malignancies particularly leukemia and non-Hodgkin's lymphoma rank among the top five leading causes of new cancer incidence and cause of death among all age groups in both sexes. Hematopoietic stem-cell transplantation (HSCT) is an advanced treatment modality that makes the only effective treatment for cancer and non-cancer-related hematologic diseases unresponsive to conventional therapy. Patients who need stem cell transplants m

Opioid use in patients with polymyalgia rheumatica.

INTRODUCTION: Recognizing and managing high-acuity, low-frequency pediatric conditions such as near-fatal bronchospasm with respiratory failure and uncompensated septic shock are essential skills for trainees. These emergencies require timely clinical judgment and familiarity with medications, respiratory support, and resuscitation strategies. METHODS: We implemented a high-fidelity, interprofessional simulation for pediatric residents focused on management of near-fatal bronchospasm complicated by septic shock. The simulation was delivered as 4 standalone sessions over 2 months. The scenario

Latest Clinical Approaches in the Ocular Management of Cystinosis: A Review of Current Practice and Opinion from the Ophthalmology Cystinosis Forum.

Cystinosis, a rare autosomal recessive disease caused by intracellular cystine accumulation, occurs in an estimated 1/100,000-200,000 live births. Ocular non-nephropathic cystinosis is typically diagnosed during adulthood, when patients present with corneal crystal deposition and no systemic involvement. Due to the rarity of the condition, diagnosis is often delayed and can have a significant impact on the overall prognosis of the disease. Early diagnosis is therefore imperative to ensure successful treatment and improve quality of life, as most of its clinical manifestations can be prevented

Factors that impact on healthcare workers' adherence with infection prevention and control (IPC) guidelines for COVID-19: a qualitative evidence synthesis.

BACKGROUND: The COVID-19 pandemic, declared in 2020, brought unprecedented challenges to our healthcare services in relation to the prevention and treatment of the disease. Additionally, preventing transmission within healthcare settings was critical. Infection Prevention and Control (IPC) guidelines are important for providing strategies on the use of personal protective equipment (PPE), the separation of patients with respiratory infections from others, and stricter cleaning routines. It is important to examine how the context of the pandemic impacts healthcare workers' (HCW) ability to adhe
