Summary The growing number of women of childbearing age joining the job market and remaining work active during pregnancy determines the importance of preventing the potential hazards of the workplace during pregnancy and while breastfeeding. We must take into account that regular working practices may not be acceptable for women during pregnancy or while breastfeeding. One of the daily risks of operation theatre nurses is exposure to inhaled anaesthetics used during surgery In most cases, nurses are unaware if exposure is permitted during pregnancy or while breastfeeding. In this article, it

For the last 40 years transtracheal ventilation has been suggested as a means of bypassing the glottis in emergency in patients unable to be intubated or ventilated by mask. A catheter has been designed to be easily inserted into the crico-thyroid membrane. The outer part of the device, with its dual attachment system, can be connected to conventional resuscitation equipment by its 15 mm male end or to a high-pressure oxygen source by its Luer-Lock fitting. The advantages of transtracheal ventilation are: a clear vision of the operative field, a good gas exchange, an elimination of airway trau

The results of 101 pyloromyotomies performed from 1972 to 1980 have been analysed; 34 operations were performed using local anaesthesia and 67 using general anaesthesia. The use of local anaesthesia was not associated with any advantage related to postoperative or other complications. A high postoperative complication rate of 20% was found (excluding postoperative vomiting), and there were 2 deaths; 11 patients suffered wound infection, with Staphylococcus aureus isolated from 5 of these wounds. The use of prophylactic antibiotics active against Staphylococcus aureus is suggested.

INTRODUCTION: A significant proportion of surgeons listen to music in the operating room (MOR) during a surgical procedure. Over the last twenty years, this practice has been widely studied. The aim of this article is to analyze the effects of MOR on surgical performance, postoperative history and, more generally, on communication in the operating theater.
METHOD: A comprehensive review of the factual literature (randomized trials and systematic reviews) was carried out.
RESULTS: Given the heterogeneity of the published studies, it was not possible to achieve results with a high level of evi

The development of inhaled anesthetics (IAs) has a rich history dating back many centuries. In modern times they have played a pivotal role in anesthesia and critical care by allowing deep sedation during periods of critical illness and surgery. In addition to their sedating effects, they have many systemic effects allowing for therapy beyond surgical anesthesia. In this narrative review we chronicle the evolution of IAs, from early volatile agents such as ether to the contemporary use of halogenated hydrocarbons. This is followed by a discussion of the mechanisms of action of these agents whi

PURPOSE: In this Continuing Professional Development module, we review the pathophysiology and clinical manifestations associated with massive hemorrhage as well as laboratory investigations and appropriate therapeutic measures. In addition to reviewing the available blood/plasma products and adjunct therapy, we also explore the role of the anesthesiologist in a massive transfusion protocol scenario.
PRINCIPAL FINDINGS: Massive hemorrhage can be either anticipated or unexpected. The coinciding presence of acidosis, hypothermia, and hypotension contribute greatly to a poor outcome. Red blood c

Most patients undergoing surgical procedures need to be mechanically ventilated, because of the impact of several drugs administered at induction and during maintenance of general anaesthesia on respiratory function. Optimization of intraoperative mechanical ventilation can reduce the incidence of post-operative pulmonary complications and improve the patient's outcome. Preoxygenation at induction of general anaesthesia prolongs the time window for safe intubation, reducing the risk of hypoxia and overweighs the potential risk of reabsorption atelectasis. Non-invasive positive pressure ventila

The acutely septic patient is a multifaceted challenge for the anesthetist. Unlike most elective surgery patients, acutely septic patients have severe systemic disease before the physiologic insults of anesthesia and surgery. The decision to operate is usually informed by the urgent or emergent need to correct a severe surgical problem and weighed against the higher risks of morbidity and mortality from the procedure itself. The care of the septic patient in the intensive care unit can help guide operating room management. However, the acuity and time course of intraoperative events, including

Scalpel injuries can expose surgeons, nurses, and other OR personnel to bloodborne pathogens. Direct and indirect costs of managing exposure include time spent reporting, treating, and following up on the injuries; salaries and benefits for injured staff members; laboratory testing of exposure sources and exposed personnel; and postexposure prophylaxis. Standard precautions, training and awareness for those at risk, the use of neutral passing zones, and safety-engineered devices have helped decrease the incidence of injury for specific categories of sharps. One new safety device is a hand piec

Fire is a rare but potentially disastrous operating room misadventure. In this article, the authors describe the elements of operating room fires and present an illustrative case. The risk of fire can be minimized when the operating room team understands the interactions among the three sides of the classic fire triangle: oxidizers, fuels, and ignition sources. Lists of fire prevention techniques and steps to take in the event of an operating room fire are provided.

Recent events around the world have emphasized the need for health care facilities to prepare to deal with biological threats, including smallpox. At Walter Reed Army Medical Center, Washington, DC, administrators recognized the need for a policy on handling patients with smallpox in the OR and asked a group of students to create a template policy for care of patients with smallpox in need of surgery. This article provides a brief history of smallpox, concerns surrounding smallpox today, and smallpox characteristics with which perioperative personnel should be familiar, as well as a guideline

The risk of postoperative infection is determined by a number of factors including the underlying health of the patient, the type of surgery and the environment in which the procedure take place. The predominant infective organisms are Staphylococci. Coagulase-negative staphylococci are regarded as emerging pathogens, especially in clean surgery. The operating team is the major source of the contaminated bacteria. Bacteria dispersed from the skin is of greater importance than bacteria from the respiratory tract, so masks are not always necessary in the theatre during an operation except by tho

Several cases of occupational HIV transmission during surgical procedures have been reported all over the world in the last few years. Orthopaedic surgeons are at particularly high risk for blood contamination and skin injuries. This article addresses the risk of HIV transmission in the surgical setting with a focus on specific risk factors: the prevalence of the infection in general population, the stage of the disease, virulence, inoculum size, the nature and frequency of blood contacts. It also shows how the last two factors depend on surgical specialty, duration and kind of the procedure,

BACKGROUND: Long-term occupational exposure to volatile anesthetic agents may result in various adverse health effects. Additionally, certain surgical procedures involve exposure to the other agents such as X rays. Identification of chromosome damages in peripheral blood lymphocytes serves as a useful indicator of exposure to mutagenic agents.
METHODS: The frequency of chromosomal aberrations was assayed in peripheral lymphocytes obtained from 129 subjects working in operating theatres and 41 control subjects.
RESULTS: The results show an increased rate of chromosome aberrations in the expos

ICU patients can undergo elective or emergency surgery. Optimising the patient and arranging for the necessary blood components is as important as the fully monitored transport to the OR. Monitoring usually includes radial artery and vena cava access routes, pulmonary artery catheters help to check on oxygen transport parameters and cardiac output. Echocardiography is a new feature to determine cardiac volumes and left ventricular function as well as possible regional ischaemia. Anaesthesia induction can be performed using thiopentone along with diazepam and opioids. In shocked patients ketami

The efficacy of resuscitative thoracotomy in the trauma patient has been questioned. Survival rates are variable, but a review of resuscitative thoracotomy in the emergency department of our institution documented an overall survival rate of only 1.8%. Higher survival rates may be anticipated in patients initially presenting with signs of life who can be transported directly to the operating room prior to the need for resuscitative thoracotomy. To test this hypothesis, the clinical course of all injured patients undergoing urgent or exigent thoracotomy in the operating room between July 1983 a

We prospectively studied 684 operations from all surgical specialties to describe the frequency and character of blood contact and exposure during the procedures. Blood contact was defined as percutaneous, mucous membrane, nonintact skin or intact skin contact of patient blood with any member of the operative team. Blood exposure was defined as contact in any of the preceding categories excluding intact skin. Over-all, 28 per cent of the patients had one or more blood contact events that involved 293 operating room personnel. Risk of blood contact was significantly greater for cardiothoracic (
