BACKGROUND: Tyrosine kinase inhibitors (TKIs) constitute the standard first-line systemic therapy for patients with unresectable epidermal growth factor receptor (EGFR)-mutant non-small cell lung cancer (NSCLC). Despite their initial efficacy, the majority of patients inevitably develop acquired resistance or experience local recurrence. This study aimed to evaluate the safety and feasibility of minimally invasive radical salvage surgery following TKI-based therapy in a cohort of patients with initially unresectable NSCLC.
METHODS: This retrospective single-center study evaluated patients who

BACKGROUND: Brachial artery injuries are rare. The objectives of the study are to report our experience and identify predictors of outcome. The hypothesis of the study is that maintaining ischemic times less than six hours results in improved outcomes.
METHODS: This is a retrospective 118-month study. The outcome measure is total operative time from admission to restoration of blood flow, and outcomes are survival and limb salvage. Statistical analyses used in the study are univariate and multivariate stepwise logistic regression.
RESULTS: There were 124 patients with 131 brachial artery inj

OBJECTIVE: To evaluate systematically the safety and efficacy of intra-operative cell salvage (ICS) in urology.
METHODS: A search of Medline, Embase and Cochrane Library to August 2017 was performed using methods pre-published on PROSPERO. Reporting followed the Preferred Reporting Items for Systematic Review and Meta-analysis guidelines. Eligible titles were comparative studies published in English that used ICS in urology. Primary outcomes were allogeneic transfusion rates (ATRs) and tumour recurrence. Secondary outcomes were complications and cost.
RESULTS: Fourteen observational studies

BACKGROUND: Mechanical autotransfusion systems for washed shed blood (WSB) were introduced to reduce the need for postoperative allogenic blood transfusions (ABTs). Although some authors have postulated decreased requirements for ABT by using autologous retransfusion devices, other trials, mostly evaluating retransfusion devices for unwashed shed blood (USB), verified a small or no benefit in reducing the need for postoperative ABT. Because of these contradictory findings it is still unclear whether autologous retransfusion systems for WSB can reduce transfusion requirements.

The technical complexity of salvage lung resection after epidermal growth factor receptor (EGFR)–tyrosine kinase inhibitor (TKI) therapy remains unclear, in contrast to the well-recognized technical difficulty of salvage surgery after chemoradiotherapy. We retrospectively reviewed 15 patients with Stage IIIB/IV lung cancer who underwent anatomical lung resection after EGFR–TKI therapy and compared perioperative outcomes according to clinical nodal (cN) status (cN − vs. cN+). Although postoperative complication rates were similar, cN+ cases demonstrated greater technical complexity, including l

STUDY DESIGN: Retrospective case-control study.
OBJECTIVE: To compare the efficacy of 3 blood management strategies in patients undergoing posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS) in reducing donor blood transfusion.
SUMMARY OF BACKGROUND DATA: Although intraoperative cell salvage and predonated banked blood may be effective in reducing donor blood transfusion in the perioperative period, the optimal blood management strategy is unclear. A combined cell salvage strategy holds several potential advantages but has not yet been investigated.

BACKGROUND: With improved safety of allogeneic blood supply, the use of preoperative autologous donations (PADs) and perioperative autologous cell salvage (PACS) has evolved. This study evaluated temporal trends in PAD and PACS use in the United States.
METHODS: The National Inpatient Sample database, a stratified probability sample of 20% of hospitalizations in the United States, was used to compare temporal trends in hospitalizations reporting use of PADs and PACS from 1995 to 2015. Factors associated with their use were examined between 2012 and 2015 with use of multivariable Poisson regre

BACKGROUND: One of the leading causes of maternal death worldwide is severe obstetric haemorrhage after childbirth. Use of intraoperative cell salvage is strongly recommended by international guidelines on patient blood management. Recent data provide strong evidence that use of cell salvage in obstetrics is effective and safe in women with postpartum haemorrhage resulting in fewer transfusion-related adverse events and shorter hospital stay. We retrospectively analysed the use of cell salvage in bleeding women during delivery for a period of 10 yr in German hospitals.

BACKGROUND AND OBJECTIVES: Intraoperative red blood cell (RBC) salvage is frequently used in contemporary spine surgery, despite clinical concern in its efficacy as a surrogate for blood-banked allogeneic packed RBCs (pRBCs). During spine surgery, salvaged RBCs (sRBCs) are exposed to injurious high-heat electrocautery, prolonged stasis, and abrasive pharmaceuticals, potentially making sRBCs a poor blood substitute. We therefore sought to scientifically and objectively define the quality of sRBCs in the context of complex spine surgery.
METHODS: This is a single-center, prospective, nonrandomi

BACKGROUND: Intraoperative blood salvage as a blood-saving strategy has been widely used in surgery. Considering its theoretic risk of malignant tumor cells being reinfused and the corresponding blood metastases, the safety of intraoperative blood salvage in cancer surgery remains controversial.
METHODS: Following the Preferred Reporting Items for Systemic Review and Meta-Analysis (PRISMA), we searched the Cochrane Library, MEDLINE and EMBASE to November 2017. We included only studies comparing intraoperative blood salvage with allogeneic blood transfusion.
RESULTS: This meta-analysis includ

BACKGROUND: Salvage radical prostatectomy (sRP) is a potentially curative treatment for localized recurrence after radiation therapy for prostate cancer; however, nationwide real-world perioperative outcomes remain unclear.
METHODS: We conducted a retrospective descriptive study using the Japanese National Clinical Database (NCD). Patients who underwent sRP after radiation therapy were identified, and perioperative, functional, and pathological outcomes were summarized descriptively according to surgical approach and prior radiation modality without comparative statistical analysis.
RESULTS:

PURPOSE: While multiple studies have evaluated the role of intraoperative red blood cell salvage (IOCS) in instrumented posterior spinal fusion as a method to reduce allogenic blood transfusion and its associated risks and cost, few studies have quantified the blood available for return to the patient when using IOCS. Given this paucity of data, the purpose of this study was to evaluate the relationship between estimated blood loss and cell saver volume returned.
METHODS: Patients aged 18 years or older who underwent elective spinal fusion for correction of deformity requiring six or more lev

PURPOSE: Salvage radical prostatectomy is associated with a higher complication rate than radical prostatectomy without prior radiotherapy but the magnitude of the increase is not well delineated.
MATERIALS AND METHODS: A total of 3,458 consecutive patients underwent open radical prostatectomy and 98 underwent open salvage radical prostatectomy from January 1999 to June 2007. Data were collected from prospective surgical and institutional morbidity databases, and retrospectively from billing records and medical records. Medical and surgical complications were captured, graded by the modified

Intraoperative cell salvage (IOCS) for high-risk obstetric hemorrhage is now endorsed by a number of obstetric organizations. Most previous studies have focused on the safety of IOCS from case series and small controlled studies. Here, we describe the effect of IOCS on rates of allogeneic blood transfusion (ABT) under different degrees of bleeding during cesarean section in women with placenta accreta, which has seldom been reported in the literature. We conducted a retrospective analysis on the introduction of routine application of IOCS for the management of hemorrhage during cesarean section

STUDY DESIGN: Retrospective review of high-dose tranexamic acid (TXA) use in consecutive patients.
OBJECTIVE: To determine the safety profile of a high-dose TXA protocol in complex adult spinal deformity patients.
SUMMARY OF BACKGROUND DATA: Adult spinal deformity (ASD) surgery may involve significant amounts of blood loss, especially when various osteotomy techniques are used. Antifibrinolytic agents such as TXA have been used to reduce intraoperative blood loss. However, there is no universally accepted dosing protocol for its use during complex ASD surgery.
METHODS: Consecutive patients

BackgroundLumbar interbody fusion is commonly performed to improve spinal stability in the context of degenerative, traumatic, and deformity-related pathologies. The axial lumbar interbody fusion (AxiaLIF) technique, also known as presacral interbody arthrodesis, is the only presacral interbody fusion technique approved by the US Food and Drug Administration. It is a rarely utilized approach to interbody fusion that aims to achieve fusion across L4 to L5 and/or L5 to S1 levels, which are the most susceptible to pseudoarthrosis and hardware failure. This case series describes the utility of the

BackgroundPelvic fixation failure (PFF) remains prevalent in adult spinal deformity correction, with failure rates up to 35%. Current revision surgeries are highly invasive and morbid, requiring extensive incisions and recovery times. Sacroiliac joint fusions (SIJFs) may offer a salvage option for PFF. Robotic navigation has previously been explored as a feasible method for primary SIJF. In this study, the authors aim to establish the viability of robot-navigated SIJF as a salvage surgery for PFF.ObservationsThis study examined that data of patients who underwent previous deformity correction
