Impact of a short online course on the accuracy of non-ophthalmic diabetic retinopathy graders in recognising glaucomatous optic nerves in Vietnam.

PURPOSE: To test an online training course for non-ophthalmic diabetic retinopathy (DR) graders for recognition of glaucomatous optic nerves in Vietnam. METHODS: This was an uncontrolled, experimental, before-and-after study in which 43 non-ophthalmic DR graders underwent baseline testing on a standard image set, completed a self-paced, online training course and were retested using the same photographs presented randomly. Twenty-nine local ophthalmologists completed the same test without the training course. DR graders then underwent additional one-to-one training by a glaucoma specialist an

Optic Nerve Sheath Diameter and Retinal Artery Resistive Index Measurements with Bedside Ophthalmic Ultrasound in Pediatric Patients with Pseudotumor Cerebri Syndrome.

Pseudotumor cerebri syndrome (PTCS) is characterized by raised intracranial pressure (ICP) with no neuroradiological abnormalities. Ocular ultrasound has been in use to measure optic nerve sheath diameter (ONSD), and retinal artery Doppler indices have been used for indirect assessment of ICP by pediatric intensivists. Here, we aimed to evaluate the correlation of the lumbar puncture (LP) opening pressure with the ultrasonographic ONSD and retinal resistive index (RRI) measures in patients with PTCS. And we wanted to find an answer to the following question: Can ultrasonographic ONSD measures

Color Doppler imaging evaluation of blood flow parameters in ophthalmic and posterior ciliary arteries and optic nerve diameter in chronic optic neuritis in multiple sclerosis patients.

BACKGROUND AND PURPOSE: Optic neuritis (ON) is a common clinical manifestation of Multiple Sclerosis (MS), which is associated with reduced visual acuity, blurred vision, blindness and retro-bulbar pain. In several studies, specific relations between optic nerve diameter and hemodynamics of the eye bulb arteries with ON have been reported. However, no consensus has been reached in this regard. We aim at determining the alterations in optic nerve diameter and eye bulb arteries hemodynamics in ON in MS patients. METHODS: This case-control study includes 40 patients, who at least had experienced

Impact of ophthalmic draping on end-tidal CO₂, cerebral oximetry, and optic nerve sheath diameter in cataract surgery: an observational comparison by COVID-19 pneumonia history with randomized oxygen flow allocation.

BACKGROUND: Ophthalmic draping during cataract surgery creates an isolated surgical field but may lead to carbon dioxide (CO₂) accumulation beneath the drape. Elevated CO₂ can cause hypercapnia, cerebral vasodilation, and increased intracranial pressure, potentially reflected by changes in cerebral oximetry and optic nerve sheath diameter (ONSD). Patients with a history of COVID-19 pneumonia may exhibit residual pulmonary dysfunction that influences these parameters during surgery. This study aimed to investigate the effects of ophthalmic draping on end-tidal CO₂, cerebral oximetry, and ONSD i

Case report: computed tomography scan-guided Gasserian ganglion injection of dexamethasone and lidocaine for the treatment of recalcitrant pain associated with herpes simplex type 1 infection of the ophthalmic division of the trigeminal nerve.

We describe the case of a 17-year-old boy with dermatologic herpes simplex virus-1 outbreaks with incapacitating facial pain requiring multiple hospitalizations. He failed to respond to aggressive treatments including antiviral drugs, opioid analgesics, stellate ganglion, and supraorbital and supratrochlear nerve blocks. The patient elected to undergo dexamethasone and lidocaine Gasserian ganglion block under computed tomography-scan guidance. He had immediate and complete relief of his pain for the first time in almost 2 years. The patient remained pain free during 6-month follow-up visits. T

Elongated, finger-like ophthalmic segment aneurysms: Implications for selection of treatment modality.

BACKGROUND: Microsurgical treatment of ophthalmic segment aneurysms often requires anterior clinoidectomy and optic nerve mobilization prior to successful clipping. OBJECTIVE: We hypothesize that ophthalmic segment aneurysms that are elongated and finger-like grow unconstrained, lateral to the optic nerve. We note that this avoids the need for clinoid resection and optic nerve mobilization. METHODS: Three cases with up-pointing aneurysms were reviewed. Patient and aneurysm characteristics were collected. RESULTS: The first two patients with elongated ophthalmic segment aneurysms were found

Transsphenoidal optic nerve decompression: an endoscopic anatomic study.

The objective was to study the relationships among the optic nerve, posterior paranasal sinuses, and ophthalmic artery. The identification of reliable anatomic landmarks would provide an endoscopic anatomic basis for optic nerve localization and effective prevention of ophthalmic artery injury during transsphenoidal optic nerve decompression surgery. The Messerklinger technique was performed to open the posterior ethmoid sinus and sphenoid sinus in 8 adult-head specimens. The opticocarotid recess and optic canal were identified under endoscopy, and the relationships between the optic nerve and

Histometric study of myelinated fibers in the human trigeminal nerve.

The trigeminal ganglion, roots and the initial portion of the ophthalmic, maxillary and mandibular nerves were dissected in 3 cadavers, to study the number, area and composition of the fascicles, and the density and diameter spectra of myelinated fibers. The total number of fibers (x 1000) was 26 in the ophthalmic, 50 in the maxillary, and 78 in the mandibular division, 7.7 in the motor root and 170 in the sensory root. In all nerves, the histograms of fiber diameter had a bimodal distribution. Cutaneous and muscle nerve fascicles clearly differed in the fiber density and diameter. The ophthal

[Endoscopic anatomy research related to transsphenoidal optic nerve decompression].

OBJECTIVE: To investigate the anatomical relationship among optic nerve, posterior ethmoid sinus and ophthalmic artery to further provide surgical instruction for endoscopic transsphenoidal optic nerve decompression. METHOD: Messerklinger technique was adopted in eight cases of adult cadaveric head to expose the posterior ethmoid sinus and sphenoid sinus. The optic-carotid recess and optic canal were identified. The adjacent structure of optic canal was observed. After removal of the bony wall of optic canal, the relationship between the optic nerve and the ophthalmic artery was disrupted. R

Raeder's syndrome. A case with an unusual localization.

We describe a patient with a Raeder's paratrigeminal neuralgia consisting of left-sided frontal and orbital pain, Horner syndrome (including anhydrosis of the forehead), and sensory loss in the territory of the first division of the trigeminal nerve. The involvement of the ophthalmic nerve is not consistent with the usual localization of this syndrome to the pericarotid sympathetic plexus. Oculosympathetic and sympathetic fibers supplying the sweat glands of the forehead join the ophthalmic nerve in the cavernous plexus localized in the cavernous sinus. Therefore, this seems to be the most lik
