Collagen in histologic stages of oral submucous fibrosis: A polarizing microscopic study.

BACKGROUND: Oral submucous fibrosis (OSF) is a precancerous condition in which there is excessive deposition of collagen in connective tissue. The purpose of present study was to compare changes in birefringence of collagen fibers in histopathological stages of OSF. MATERIALS AND METHODS: Collagen in connective tissue of 91 cases of OSF was studied by staining 5 μm thick sections with picrosirius red. The polarization colors of thin (0.8 μm or less) and thick (1.6-2 μm) collagen fibers were recorded. RESULTS: The birefringence of thin collagen fibers showed no difference in both histopatholo

One-stage reconstruction of a large defect of the lower lip and oral commissure.

Large lower lip defects including the oral commissure are very difficult to reconstruct and obtain an aesthetic appearance and an acceptable function. We report two patients who had a large defect in the lower lip including oral commissure and were treated using free radial forearm flap and temporal muscle transfer in one-stage. For the cutaneous defects, in one a local flap was elevated from the adjacent cheek and in the other a turned over area of the forearm flap was used. In each case, the reconstructed lower lip could maintain adequate elevation of oral the commissure using the temporal m

Decoding the Dynamic Landscape of Sequential Disease Stages from Oral Submucous Fibrosis to Oral Squamous Cell Carcinoma.

Oral submucous fibrosis (OSF) is a chronic and progressive disease of the oral mucosa that is considered potentially malignant. However, the specific mechanism of OSF occurrence and malignant transformation into oral squamous cell carcinoma (OSCC) is still unclear. This study aimed to elucidate the spatiotemporal changes in cellular biological behaviour during the occurrence and malignant transformation of OSF, providing new insights for understanding the underlying mechanism and developing novel therapeutic strategies. Sets of normal, OSF, margin area, and OSCC tissue samples were collected,

A matched cohort comparison of mTHPC-mediated photodynamic therapy and trans-oral surgery of early stage oral cavity squamous cell cancer.

Photodynamic therapy (PDT) of early stage oral cavity tumors have been thoroughly reported. However, statistical comparison of PDT to the surgical treatment is not available in published literature. We have identified and matched cohorts of patients with early stage oral cavity cancers undergoing surgery (n = 43) and PDT (n = 55) from a single institute experience. The groups are matched demographically and had the same pre-treatment screening and follow-up schedule. Both groups consisted only of tumors thinner than 5 mm to ensure comparability. The endpoints were local disease free survival,

Stroma AReactive invasion Front Areas (SARIFA) in early-stage oral tongue squamous cell carcinoma.

BACKGROUND: Stroma AReactive Invasion Front Areas (SARIFA) has recently been introduced as a prognostic indicator. The prognostic value of SARIFA has not been studied in early-stage oral tongue squamous cell carcinoma (OTSCC). MATERIAL AND METHODS: A total of 287 cases of early-stage OTSCC were included in this multicenter study. SARIFA was defined as direct contact between tumor cells and the adipose tissue, and was assessed in hematoxylin and eosin (HE)-stained sections. RESULTS: In early-stage OTSCC, SARIFA-positive tumors were associated with worse disease-specific survival with a hazard

Neck dissection improves the prognosis of patients with early-stage oral squamous cell carcinoma.

For early-stage oral squamous cell carcinoma (OSCC), the primary treatment modality generally entails surgical resection. However, the role of neck dissection in early-stage OSCC remains controversial. The aim of this study was to evaluate the effect of neck dissection on the prognosis of patients with early-stage OSCC. We identified patients with early-stage OSCC from the Surveillance, Epidemiology, and End Results database spanning from 2004 to 2021. We collected demographic and clinical data, including age, sex, tumor site, marital status, race, residence, income, grade, records of radiothe

Machine Learning Predicts Lymph Node Metastasis in Early-Stage Oral Tongue Squamous Cell Carcinoma.

PURPOSE: Early-stage oral tongue squamous cell cancer (OTSCC) has a rate of metastasis to the cervical lymph nodes of 20 to 50%. This study aimed to build and validate 4 machine learning (ML) models to predict the occurrence of lymph node metastasis before and after surgery for early-stage (cT1N0 to cT2N0) OTSCC. MATERIALS AND METHODS: We designed a retrospective cross-sectional study and reviewed the clinical and pathologic records of patients with early-stage OTSCC. The sample was composed of 2 groups with different node status (positive or negative) and was randomly split into training (70

Assessment of Tumor-infiltrating Lymphocytes Predicts the Behavior of Early-stage Oral Tongue Cancer.

Tumor-infiltrating lymphocytes (TILs) have shown a promising prognostic value in many epithelial cancers. We sought to assess the prognostic value of TILs in a multicenter cohort of early oral tongue squamous cell carcinoma (OTSCC). The percentage of TILs was assessed on the surgical resection slides stained with hematoxylin and eosin. The assessment of TILs was performed in the stromal compartment and in the intraepithelial compartment (at the invasive front and at the center of the tumor). We followed the method that was described recently by the International Immuno-Oncology Biomarker Worki

Surgical defect coverage in oral submucous fibrosis patients with single-stage extended nasolabial flap.

PURPOSE: Long standing oral submucous fibrosis (OSMF) is associated with involvement of the oral submucosa and the muscles of mastication leading to difficulty in mouth opening. Various surgical modalities are mentioned for release but each has its own limitations. The aim of the study was to evaluate the postoperative mouth opening in patients of OSMF after excision of fibrous bands followed by coronoidotomy and surgical defect coverage by extended nasolabial flap. METHODS: We evaluated the use of extended nasolabial flaps and coronoidectomy in the management of 11 randomly selected patients

Cervical lymph node metastasis from early-stage squamous cell carcinoma of the oral tongue.

CONCLUSIONS: The findings of this study demonstrated that the wait-and-watch strategy for neck metastasis from squamous cell carcinoma (SCC) of oral tongue is a reliable option and that salvage by surgical treatment is effective. However, younger patients should be closely monitored for recurrence. Adjuvant therapy may be recommended for patients with pathologically advanced disease. OBJECTIVES: Metastatic involvement of cervical lymph nodes is the most important prognostic indicator in patients with oral tongue SCC. With the objective of determining the most appropriate treatment strategy fo

Tumour growth kinetics and stage progression during treatment delay in oral squamous cell carcinoma.

BACKGROUND: Oral squamous cell carcinoma (OSCC) frequently progresses during delays before definitive treatment, yet the biological behavior of tumours during this interval remains poorly characterized. We prospectively evaluated tumour growth kinetics, stage progression, and anatomic pathways of spread using serial pre-treatment imaging. METHODS: This prospective observational study included 483 patients with treatment-naïve OSCC treated between July 2020 and December 2023. Eligible patients underwent at least two clinically indicated pre-treatment imaging studies separated by ≥ 3 weeks. Gro
