BACKGROUND AND OBJECTIVES: Trauma to the central midface may result in complex nasoethmoid orbital fractures. Due to the intricate anatomy of the region, these challenging fractures may often be misdiagnosed or inadequately treated. The purpose of this article is to aid in determining the appropriate exposure and method of fixation.
METHODS AND MATERIALS: This article presents an organized approach to the management of nasoethmoid orbital fractures that emphasizes early diagnosis and identifies the extent and type of fracture pattern. It reviews the anatomy and diagnostic procedures and prese

Along with the neurosurgical difficulties, the management of fronto-orbital fractures poses problems of aesthetical nature, especially those that arise in restoring the trauma-deformed face to its original, or close to original, appearance. The cranioplasty using duracryl to deal with defects secondary to impression fractures affecting the frontal sinuses presents a serious therapeutic problem because of the risk of suppuration. We report 48 patients treated in the Department of Neurosurgery of the University of Medicine, Plovdiv between 1981 and 1990. Particular attention was paid to impressi

When large portions of the internal orbit are destroyed (two to four walls), standard bone-grafting techniques for immediate and late orbital reconstruction may not yield predictable eye position. Critical bone support is most often deficient inferomedially. CT analysis of orbital volume in cases where eye position was unsatisfactory reveals that displacement of bone grafts is one mechanism of the unsatisfactory result. Other mechanisms include undercorrection and bone-graft resorption. In order to minimize postoperative bone-graft displacement, titanium implants were used to span large defect

Two cases of patients with isolated medial orbital wall fractures are described. Presenting physical findings were minimal; however, both patients were noted to have bleeding into the nasopharynx from unidentified sources. Routine radiographs failed to disclose the fractures, which were clearly shown by computed tomography. Reconstructive surgery was planned for one patient. Frequently overlooked, this injury may be the cause of delayed enophthalmos, meningitis, or blindness. The presentation, diagnosis, and management of medial orbital wall fractures are reviewed.

Orbital floor fractures can occur with or without any associated fractures of the middle third of the facial skeleton. Fifty-one patients with a unilateral "black eye" after motor vehicle accidents were reviewed. The fractures involving the orbital floor were analysed. The clinical signs and symptoms, with results of radiological examination, are discussed. The various treatment modalities and their postoperative complications are discussed.

Between 1973 and 1974, 96 zygomatic fractures were diagnosed of which 89 were surgically treated. 75 fractures were set openly and of these 65 were joined by osteosynthesis. 75 patient were examined later. Results show that posttraumatic complications occurred more frequently in fractures set without opening than in fractures set by osteosynthesis. Open reposition is therefore recommended, along with stabilization by osteosynthesis.

Six patients with blowout fractures of the orbital floor, not associated with infraorbital rim fractures, are presented. Diplopia, infraorbital anesthesia and enophthalmos following blunt trauma to the eye should alert the examiner to the possibility of a blowout fracture with incarceration of extraocular tissues. Early roentgenographic examinations are often difficult to evaluate because of periorbital edema, mucosal swelling and antral hematoma. In cases in which stereoscopic Waters projections are equivocal, laminograms of the orbit, after the edema subsides, will aid in confirming the diag

Orbital fractures are frequent and can result in severe complications (visual impairment, diplopia and enophthalmos). Their management is challenging and remains controversial. Whereas immediate surgery is clearly indicated in children with trapdoor fractures associated with annoying diplopia and observational management is indicated in cases with no diplopia, a grey zone of therapy decisions concern a significant portion of cases. Surgical repair is not trivial and can be plagued by severe complications such as visual impairment, globe misplacement and diplopia. Thus, the final decision shoul

PURPOSE OF REVIEW: Orbital floor blowout fractures may lead to significant morbidity, including debilitating double vision and globe malposition. Thorough evaluation and timely surgical intervention may prevent long-term sequelae.
RECENT FINDINGS: New studies have demonstrated certain patients with orbital floor fractures to do well when managed conservatively. In those necessitating operative repair, precise surgical dissection and adequate fracture reduction are essential to restoring orbital anatomy and function. Novel surgical techniques and orbital floor reconstruction materials, describ

The natural course of several isolated and nonisolated orbital roof fractures is reported, by showing four cases in which a "wait and see" policy was followed. All four cases showed spontaneous repositioning and stabilizing of the fracture within less than a year. This might be explained by the equilibrium between the intraorbital and intracranial pressures.

The management of orbital floor fractures involves several issues. Different surgeons have diverse opinions about who needs surgery and how best to do it. This is especially so with regard to use of transantral diagnostic and therapeutic endoscopic surgery over traditional lower eyelid approaches. Transantral endoscopy provides precise and complete information about the orbital floor fracture and makes possible the repair of these fractures under unobstructed vision at all times, minimizing the risk of implant misplacement and, hence, enophthalmos. Contrary to popular belief, this technique is

Significant orbital trauma can result in orbital floor fractures with subsequent prolapse of the orbital contents into the paranasal sinuses. Prolapse of the periorbita can result in extraocular muscle entrapment, diplopia, enophthalmos, and even visual loss. Management of orbital floor fractures traditionally has been accomplished through transconjunctival and subciliary incisions. These approaches provide adequate visualization and cosmetically acceptable scars. Unfortunately, post-operative lid malposition can occur in a small percentage of cases. Another limitation is easy visibility of th

The nature and aetiology of orbital roof fractures is discussed and a strategy for their successful management outlined with reference to five clinical cases. Early recognition and treatment of these fractures reduces the high incidence of intra-cranial and ocular complications and the importance of a combined approach involving maxillofacial surgeons, neurosurgeons and ophthalmologists is emphasised. A coronal flap together with frontal craniotomy is recommended for surgical access to the orbital roof and overlying dura. When reconstruction is indicated, autogenous bone harvested from the inn

37 patients treated surgically for orbital blow-out fractures were reviewed retrospectively. 29 had diplopia before and four after operation. The diplopia was present only in extreme gaze positions and was not troublesome to the patients. Enophthalmus was present in 19 patients preoperatively and in six postoperatively. Of six patients treated conservatively one developed a moderate enophthalmus. There were no significant postoperative complications.
