Ophthalmological and Interdisciplinary Surgical Approaches to the Orbit.

The treatment of orbital masses often requires an interdisciplinary approach. Ophthalmological surgical approaches include the anterior transcutaneous approach for removal of dermoids or taking biopsies. Anterior transconjunctival orbitotomy is suitable for masses of the inferior orbit or the intraconal compartment, but is inappropriate for masses of the orbital apex. The lateral transosseous orbitotomy often fits for masses of the lateral or intraconal compartment, especially for neoplasms of the lacrimal gland e.g. pleomorphic adenoma. Cryoextraction is often the technique of first choice fo

Fabrication of orbital prosthesis by two different methods in patients with post-COVID-19 rhino-orbital maxillary mucormycosis: A case series.

Neoplasms, congenital disorders, fungal infections, and traumatic injuries are the predominant causes of orbital defects. Various retentive mechanisms such as application of adhesive, utilization of mechanical undercuts, and implant-supported attachments are generally used in the maxillofacial prosthesis. In the orbital region, the result of magnet-retained attachments is favorable compared with other mechanisms. Different advantages of the magnet-retained prosthesis are less manual dexterity needed during insertion or removal and better maintenance of hygiene. The skin-implant interface and t

Unusual progression of pleomorphic adenoma of the lacrimal gland: case report.

A 72-year-old female complained of acute pain on left eye movement followed by progressive exophthalmos. Neuroimaging revealed a large well-demarcated lesion consisting of solid and cystic parts, as well as bone destruction and hemorrhage, within the left orbital cavity. The preoperative diagnosis was pleomorphic adenoma with or without malignant transformation, or cavernous angioma. En bloc excision including adjacent tissues was planned to resolve the progressive symptoms and to obtain a histological diagnosis. The transcranial route was chosen since tumor invasion to the cranial base was po

Pediatric intraorbital wooden foreign body mimicking an orbital mass: Delayed diagnosis and imaging considerations on MRI.

Wooden intraorbital foreign bodies (IOFBs) are uncommon in children and may present a significant diagnostic challenge because their imaging appearance can mimic orbital neoplasms or inflammatory lesions, particularly when the trauma history is absent or unrecognized. We report the case of a 10-year-old boy presenting with progressive left orbital swelling and pain. Initial magnetic resonance imaging (MRI) demonstrated a rim-enhancing extraconal lesion with a central non-enhancing component and was initially interpreted as an orbital neoplasm, including a possible orbital schwannoma. No trauma

Neoplasms of the nasopharynx in children.

Neoplasms of the nasopharynx are rare in children, but they threaten the child's life when they do occur. The nasopharynx tends to harbor dysontogenetic neoplasms. After classification into benign and malignant groups, nasopharyngeal neoplasms in children can be further characterized according to the age of the patients in which the clinical manifestations usually appear. Dermoids and teratomas are the most frequently encountered neoplasms of the nasopharynx in infants and may produce airway obstruction and dysphagia. Among the benign tumors of the nasopharynx in children, the juvenile angiofi

Pathology of orbital bones. The XXXII Edward Jackson Memorial Lecture.

The orbital bones may show nearly all the pathologic changes observed in the skull and in the face. The congenital anomalies in this area are numerous and involve various forms of craniostenoses. Among the benign osseous tumors the osteoma is most frequently encountered in the orbit. Fibrous dysplasia is a tumefaction of indeterminate behavior that often involves the orbit. Osteosarcoma or other malignant neoplasms are rarely seen in this area. Eosinophilic granuloma and Hand-Schüller-Christian disease are tumor-like lesions that may involve the orbit.

Primary orbital endodermal sinus tumor (yolk sac tumor).

Endodermal sinus tumor, or yolk sac tumor, is the most common malignant neoplasm of germ cell origin and usually occurs in infant testes or ovaries. On rare occasions, the tumor may arise from extragonadal sites, including sacrococcygeal region, uterus, vagina, prostate, retroperitoneum, liver, mediastinum, pineal gland, and third ventricle. The orbit is an unusual location for the primary development of this neoplasm. We report the case of a girl with primary orbital endodermal sinus tumor who was managed with exenteration and chemotherapy with the result of a disease-free survival of 9 years

[A case of sinonasal carcinoma with intracranial invasion treated by a multidisciplinary team].

We report a case of a 66-year-old male with diffuse infiltration of neoplasm from the paranasal sinuses to the orbit and left cerebral hemisphere, associated with prominent edema. Initial complaints were headache and swelling of the forehead, which were followed by progressive symptoms, such as epistaxis, dacryops, and severe pains. Neuroimaging showed marked invasion of a neoplasm from the left paranasal sinuses into the intracranial space and to the frontal skull. Radical removal of the neoplasm and left orbital content with reconstruction of the anterior skull base using musculocutaneous fl

Doppler imaging of the ophthalmic vasculature of the normal dog: blood velocity measurements and reproducibility.

The purpose of this study was to assess the feasibility and reproducibility of color Doppler imaging (CDI) of the vasculature of the normal canine orbit and eye. Eight normal Beagles were evaluated by Doppler imaging. The goals of the study were to determine the location, spectral waveform morphology, specific blood velocity parameters, and reproducibility for the ophthalmic and orbital vessels most frequently identified in the normal dog. Vessels identified a majority of the time (> 50%) included: external ophthalmic artery, dorsal external ophthalmic vein, ventral external ophthalmic vein, i

Gamut of Orbital Lesions in a Tertiary Neurocenter-A Clinicopathological Study of Lesions Seen Over a Period of One Decade.

BACKGROUND: The orbital region is an anatomically complex area comprising crucial contiguous/adjacent structures. Since the eye has a neuroectodermal basis of embryogenesis, many of the lesions may be similar to those arising in the central nervous system. OBJECTIVE: To record and describe the clinicopathological spectrum of orbital lesions presenting to a neurology center. STUDY SETTING: The retrospective study included biopsy/resected specimens of patients with orbital/ophthalmic lesions referred to the Department of Neuropathology, between February 2007 and February 2018. Esthesioneuroblastoma Presenting With Orbital Hypertelorism and Foster-Kennedy Syndrome.

Esthesioneuroblastoma is a rare neoplasm originating from the olfactory neuroepithelium at the cribriform plate. The superior nasal cavity is primarily affected. Morbidity and mortality are related to locally destructive growth as well as metastatic potential. Orbital involvement is associated with decreased survival. The authors describe a case of advanced esthesioneuroblastoma with bilateral orbital involvement, presenting with a rare constellation of orbital hypertelorism and Foster-Kennedy Syndrome.

The 2017 Doyne Lecture: the orbit as a window to systemic disease.

A very large number of disorders affect the orbit, and many of these occur in the setting of systemic disease. This lecture covers selected aspects of orbital diseases with systemic associations in which the author has a particular clinical or research interest. Spontaneous orbital haemorrhage often occurs in the presence of bleeding diatheses. Thrombosis of orbital veins and ischaemic necrosis of orbital and ocular adnexal tissues occur with thrombophilic disorders, vasculitis, and certain bacterial and fungal infections. Non-infectious orbital inflammation commonly occurs with specific infla

Minimally invasive resection of a spheno-orbital tumor: Review of surgical techniques and a case report.

BACKGROUND: Resection of neoplasms of the cranio-orbital region represents a significant surgical challenge due to the extensive spread of the tumor into adjacent anatomical structures, deep regions of the skull base, the dense structure of the tumor, and the complexity of defect reconstruction. The aim of this study is to present a minimally invasive approach, using the example of a spheno-orbital tumor, to evaluate its effectiveness and cosmetic advantages. CASE DESCRIPTION: A 46-year-old patient with a spheno-orbital cranial neoplasm presented with exophthalmos and decreased visual acuity

Frontal sinus osteoma presenting with orbital emphysema.

Osteoma is the most common neoplasm of the paranasal sinuses usually involving the frontal sinus. Osteomas are benign, but can cause serious intracranial or orbital complications. The authors report a frontal sinus osteoma with orbital emphysema in a 16-year-old boy with a history of right-eye swelling after nose blowing. CT showed a right frontal sinus bony mass. Intraoperatively, strands of mucosa extending from the frontal sinus around the periphery of the mass in the right orbit were seen. The mass was excised and the orbital roof repaired. Histopathology was consistent with osteoma. The o

Palliative orbital decompression for metastatic melanoma to the orbit.

Although relatively rare, metastatic carcinomas to the orbit are the second most common orbital neoplasm in the adult. There are only ten cases reported in the literature of cutaneous melanoma metastatic to the orbit and, of these, only two had disease involving both orbits. This paper presents the third case of bilateral orbital melanoma; however, in this case, the primary site was the uveal tract in the first eye, with contralateral involvement being a metastasis of the orbital primary. The patient developed severe unilateral proptosis in his only seeing eye with progressive loss of vision.

Orbital metastases from endocrine tumours.

Orbital metastases (OM) are uncommon but clinically significant manifestations of systemic malignancy. Endocrine tumours represent a rare source of OM, often posing diagnostic and therapeutic challenges due to their variable presentation and indolent course. OM from endocrine tumours most commonly originate from differentiated thyroid carcinoma and neuroendocrine neoplasms. Presenting symptoms include proptosis, diplopia, pain and visual impairment, frequently mimicking inflammatory or primary orbital disorders. Imaging typically demonstrates mass-like orbital lesions, while in some cases defi

Giant orbital leiomyoma in a pediatric patient: diagnostic and therapeutic challenge.

This manuscript describes an exceptional case of a long-standing orbital leiomyoma in a 14-year-old male. The tumor was unusually large, causing severe proptosis and significant involvement of the ocular muscles. The patient presented with amaurosis, complete ophthalmoplegia, spontaneous eye pain, and the inability to close the eyelids, leading to psychological distress. Due to the tumor's size and progression, a right orbital exenteration was performed to remove all orbital contents, including the tumor and the eyeball. The surgical procedure aimed to prevent tumor recurrence and improve the

Multidisciplinary approach to the management of complex bony and soft tissue orbitocranial disorders.

Orbital reconstructive surgery is entering a new age as progress in surgical techniques and diagnostic imaging are applied to the patient with severe acquired orbital and periorbital deformities. The authors present a series of six patients with orbitocranial neoplasms and congenital and traumatic deformities whose management involved a multidisciplinary approach. Particular emphasis is placed on the use of reformatting two-dimensional orbital scans into three-dimensional images and surgical techniques using composite flaps and periorbital microsurgical vascular anastomoses.
