The role of laparoscopy in the case of nonpalpable cryptorchidism is both diagnostic and therapeutic. Laparoscopic orchiopexy for nonpalpable testes in the pediatric population has become the preferred surgical approach among pediatric urologists over the last 20 years. In contrast, laparoscopic varicocelectomy is considered one of several possible approaches to the treatment of a varicocele in an adolescent; however, it has many challengers and it has not gained universal acceptance as the gold standard. This article reviews the published evidence regarding these surgical techniques.

Cryptorchidism is a known risk factor for testicular cancer and the secular increase in testicular cancer incidence might have been paralleled by a similar increase in cryptorchidism. Data on trends in prevalence of cryptorchidism are however conflicting and decreases have recently been reported. To analyse Swedish trends in rates of orchiopexy, we used the Swedish Hospital Discharge Register to identify all cases of orchiopexy carried out for cryptorchidism between 1977 and 1991, that is, before the era of outpatient orchiopexies in Sweden. Observed trends were analysed in 5-year age groups.

From the results of examination of 60 healthy boys the authors suggest a thermographic characteristics of the scrotum in different age groups. It is shown with high authenticity that the temperature of the scrotum reduces with age. Besides, there is an authentic increase of temperature of the scrotum on the left side in children aged from 11 to 14 years. The various steps in a urological orchiopexy are described in detail; alignment of an everted testis, funiculolysis, retroperitoneal dissection and medial displacement of the cord. A 29-year-old man had been operated on for bilateral cryptorchism at the age of 11. The right testis was not found and was considered to be aplastic. The left testis was placed surgically in the scrotum. When he entered a follow-up study for early detection of testicular cancer, the right testis was found intraperitoneally and was removed. A left testicular biopsy showed carcinoma in situ and orchiectomy was performed. This testis contained an invasive seminoma.

Ten of 12 intra-abdominal testes transferred to the scrotum in 10 patients were successfully revascularised by microvascular techniques. It is proposed that orchiopexy should be performed early, thus reducing the risk of malignant transformation, and preferably by 2 years of age before the onset of testicular damage. A careful long-term follow-up through puberty and into adulthood is required to determine the place of microvascular orchiopexy in the management of high undescended testes.

Over a 14-year period, a total of 40 patients underwent surgical revision after an unsuccessful orchiopexy attempt. Overall, 34 reorchiopexies (1 bilateral) and 7 orchiectomies were performed. All reorchiopexies were carried out using extensive mobilization of the testis and spermatic cord. This approach was sufficient for scrotal placement of the testis in 23 instances; however, it was necessary to transpose the spermatic vessels medially in the remaining 11 (incision of the transversalis fascia in 5, incision of the transversalis fascia and division of the inferior epigastric vessels in 6).

Testicular torsion occurring three years after bilateral orchiopexy is described. The antecedent orchiopexy was accomplished using 2-0 proline sutures and transseptal fixation of the contralateral testicle. A history of bilateral orchiopexy should not interfere with diagnosing torsion in the acute scrotum. Doppler ultrasound and nuclear imaging scan are auxiliary tools that should be used when available, but should not preclude expeditious surgical treatment of suspected torsion.

A series of 141 prepubertal boys with undescended testes operated on in a provincial teaching hospital has been analyzed five years after operation. The cases of spermatic cord torsion reported here show that this condition may occur even some time after fixation. Three men, aged 22, 30 and 35 years, visited the urologist with complaints of an acute scrotum following earlier orchiopexy for spermatic cord torsion. Different techniques are employed to fixate the testicles after spermatic cord torsion. These cases confirm that Jaboulay's bottleneck procedure is the method of choice for bilateral fixation in patients presenting with this condition.
