Research Progress and Future Perspectives of Orexin Receptor Antagonists in Insomnia Therapy.

Insomnia is a common sleep disorder that significantly impairs quality of life and increases the risk of various chronic diseases. Although conventional sedative-hypnotic agents are effective, their clinical use is limited by tolerance, dependence, and residual next-day effects. The orexin system plays a critical role in sleep-wake regulation through orexin receptor 1 (OX1R) and orexin receptor 2 (OX2R), with OX2R serving as the primary mediator of wakefulness. Based on this mechanism, orexin receptor antagonists (ORAs) have been developed to promote sleep by suppressing wake-promoting signali

Safety of dual orexin receptor antagonists: a real-world pharmacovigilance study.

BACKGROUND: Dual orexin receptor antagonists (DORAs) are widely used for treating insomnia. However, real-world data on the adverse events (AEs) induced by DORAs are lacking. METHODS: Data related to daridorexant, suvorexant, and lemborexant were collected from the US Food and Drug Administration Adverse Event Reporting System between the first quarter of 2022 and the third quarter of 2024. Two established signal quantification methods, the reporting odds ratios and proportional reporting ratios, were applied. RESULTS: A total of 2665 reports on daridorexant (1738, 65.22%), suvorexant (667,

Orexin Receptor Antagonism: Normalizing Sleep Architecture in Old Age and Disease.

Sleep is essential for human well-being, yet the quality and quantity of sleep reduce as age advances. Older persons (>65 years old) are more at risk of disorders accompanied and/or exacerbated by poor sleep. Furthermore, evidence supports a bidirectional relationship between disrupted sleep and Alzheimer's disease (AD) or related dementias. Orexin/hypocretin neuropeptides stabilize wakefulness, and several orexin receptor antagonists (ORAs) are approved for the treatment of insomnia in adults. Dysregulation of the orexin system occurs in aging and AD, positioning ORAs as advantageous for thes

Cholinergic modulation of narcoleptic attacks in double orexin receptor knockout mice.

To investigate how cholinergic systems regulate aspects of the sleep disorder narcolepsy, we video-monitored mice lacking both orexin (hypocretin) receptors (double knockout; DKO mice) while pharmacologically altering cholinergic transmission. Spontaneous behavioral arrests in DKO mice were highly similar to those reported in orexin-deficient mice and were never observed in wild-type (WT) mice. A survival analysis revealed that arrest lifetimes were exponentially distributed indicating that random, Markovian processes determine arrest lifetime. Low doses (0.01, 0.03 mg/kg, i.p.), but not a hig

Unveiling the binding mechanism of orexin 2 receptor antagonists with computational chemistry.

A detailed understanding of ligand recognition and binding at the orexin receptor 2 (OXR2) is essential for the rational design of selective therapeutics for sleep and neuropsychiatric disorders. In this study, we investigated the molecular interactions of three dual orexin receptor antagonists (daridorexant, lemborexant, suvorexant) and the selective OXR2 antagonist seltorexant using an integrated computational approach that combined classical molecular docking, explicit-membrane molecular dynamics (MD) simulations of 700 ns, and quantum-based molecular fractionation with conjugate caps (MFCC

Activation of orexin receptor 2 plays anxiolytic effect in male mice.

Anxiety disorders are the most common psychiatric illnesses. Present drugs can provide temporary relief for anxiety, however, they also come with side effects and safety concerns such as dependence, suicide, overdose and so on. Therefore, it is critical to discover new anxiolytic targets. An ongoing area of interest in the field of psychiatric diseases is the orexin system. Emerging body of evidences show that orexin receptor 1 (OX1R) has promising potential as novel anxiolytic target. However, little attention has been paid to orexin receptor 2 (OX2R) in anxiety. In this study, by using behav

Found in translation: orexin receptor antagonism for the treatment of opioid use disorder.

Existing pharmacological treatment options for opioid use disorder (OUD) face challenges that limit their efficacy. Accumulating preclinical evidence implicates the orexin/hypocretin system in numerous aspects of OUD development and symptomology: opioid use, opioid-associated cue-driven drug seeking, and opioid withdrawal states that lead to relapse. Further, these studies indicate that selective and dual orexin receptor antagonists (SORAs and DORAs) could constitute promising treatments for OUD and other substance use disorders. Several DORAs have already been approved by the FDA for the trea

Dual orexin receptor antagonists in insomnia: Toward a new therapeutic paradigm.

Although three dual orexin receptor antagonists (DORAs), suvorexant, lemborexant, and daridorexant, are currently available and widely used to treat insomnia, the differences in their elimination half-lives are not sufficient. As a result, clinicians have limited ability to tailor therapy to individual sleep complaints. The emergence of vornorexant, with a notably short half-life comparable to that of zolpidem, may substantially expand the clinical utility of DORAs. This broader spectrum of pharmacokinetic profiles enables more individualized treatment strategies that align with patients' spec

Urinary excretion profiles of the orexin receptor antagonist suvorexant and its metabolites.

PURPOSE: Suvorexant is an orexin receptor antagonist used in the treatment of insomnia. In this study, we investigated the urinary excretion profiles of suvorexant and its major metabolites, including conjugates, to obtain fundamental information for proving exposure to suvorexant in criminal cases. METHODS: Urine specimens were collected from three subjects for maximum 168 h after a single oral ingestion of suvorexant (10 mg), and suvorexant and its metabolites in urine were determined using liquid chromatography-tandem mass spectrometry with a C18 semi-micro column. RESULTS: The carboxylic

Anxiolytic-like effects of the dual orexin receptor antagonist daridorexant in rats.

Dual orexin receptor antagonists (DORAs) belong to a novel class of sleep medications that function by blocking the actions of wakefulness-promoting orexin neuropeptides in sleep-wake centers of the brain. Orexins also transmit signals to brain nuclei that regulate emotions and stress responses. The effects of DORAs on anxiety-like reactions requires further exploration. The hyperarousal theory of insomnia suggests an underlying overactivation of the body's stress response systems, and a considerable proportion of insomnia patients suffers from concurrent anxiety disorders. Hence, it is import

Abuse potential assessment of the dual orexin receptor antagonist daridorexant in rats.

BACKGROUND: Drugs that act on the central nervous system (CNS) and have sedative effects can lead to abuse in humans. New CNS-active drugs often require evaluation of their abuse potential in dedicated animal models before marketing approval. Daridorexant is a new dual orexin receptor antagonist (DORA) with sleep-promoting properties in animals and humans. It was approved in 2022 in the United States and Europe for the treatment of insomnia disorder. AIMS: Nonclinical evaluation of abuse potential of daridorexant using three specific rat models assessing reinforcement, interoception, and with

Dual Orexin Receptor Antagonists (DORAs) as an Adjunct Treatment for Smoking Cessation.

Smoking is recognized as the most avoidable cause for multiplicity of chronic diseases. However, smoking cessation rates remain low, in part due to the limited target engagement of the currently approved medications for smoking cessation. Sleep is a promising focus for increasing smoking cessation rates because smokers' sleep problems are exacerbated during the first week of smoking abstinence and are associated with poor smoking cessation outcomes. Furthermore, the currently approved smoking cessation pharmacological agents varenicline and nicotine replacement treatment exacerbate sleep probl

Design and Synthesis of Non-Peptide, Selective Orexin Receptor 2 Agonists.

Orexins are a family of neuropeptides that regulate sleep/wakefulness, acting on two G-protein-coupled receptors, orexin receptors 1 (OX1R) and 2 (OX2R). Genetic and pharmacologic evidence suggests that orexin receptor agonists, especially OX2R agonist, will be useful for mechanistic therapy of the sleep disorder narcolepsy/cataplexy. We herein report the discovery of a potent (EC50 on OX2R is 0.023 μM) and OX2R-selective (OX1R/OX2R EC50 ratio is 70) agonist, 4'-methoxy-N,N-dimethyl-3'-[N-(3-{[2-(3-methylbenzamido)ethyl]amino}phenyl)sulfamoyl]-(1,1'-biphenyl)-3-carboxamide 26.

Effects of orexin 2 receptor activation on apnea in the C57BL/6J mouse.

BACKGROUND: The hypothesis was that an orexin 2 receptor (OX2R) agonist would prevent sleep-related disordered breathing. METHODS: In C57BL/6J (B6) mice, body plethysmography was performed with and without EEG monitoring of state (wakefulness, NREM and REM sleep). Outcome was apnea rate/h during sleep-wake states at baseline and with an intracerebroventricular administration of vehicle, 4 nMol of agonist OB(DL), and 4 nMol of an antagonist, TCS OX2 29. RESULTS: A significant reduction (p=0.035, f=2.99) in apneas/hour occurred, especially with the agonist. Expressed as a function of the chang

Examining the role of endogenous orexins in hypothalamus-pituitary-adrenal axis endocrine function using transient dual orexin receptor antagonism in the rat.

The orexin neuropeptide system regulates wakefulness and contributes to physiological and behavioral stress responses. Moreover, a role for orexins in modulating hypothalamus-pituitary-adrenal (HPA) axis activity has been proposed. Brain penetrating dual orexin receptor (OXR) antagonists such as almorexant decrease vigilance and have emerged as a novel therapeutic class for the treatment of insomnia. Almorexant was used here as a pharmacological tool to examine the role of endogenous orexin signaling in HPA axis endocrine function under natural conditions. After confirming the expression of pr

Successful Methods for Switching from a Benzodiazepine Receptor Agonist to a Dual Orexin Receptor Antagonist for the Treatment of Insomnias.

OBJECTIVE: Benzodiazepine receptor agonists (BZRAs) are still prescribed for insomnia to many patients in clinical practice, even though dual orexin receptor antagonists (DORAs) are an effective insomnia pharmacotherapy. It is important to establish appropriate methods of switching from BZRAs to DORAs for insomnia treatment. METHODS: We performed a secondary analysis of our prior retrospective study of the rate of DORA (suvorexant or lemborexant) continuance at 3 months after the introduction of these agents in 210 patients under long-term BZRA treatment. We investigated the effects of the cl

Sex differences in interactions between the muscarinic-1 and orexin-1 receptors on cognitive flexibility in rats.

Previous research has shown that cognitive flexibility, the ability to adjust to different rules, is affected by manipulations of orexin and acetylcholine (ACh). Because there are strong neural connections between orexin and ACh, these two neurotransmitter systems may interact to impact cognitive flexibility. In the present experiment, the effects of pharmacological manipulations of orexin-1 and muscarinic-1 receptors in rats were tested on cognitive flexibility. Rats received intraperitoneal injections of VU0453595, a positive allosteric modulator for the M1 subtype of ACh receptors, for seve
